EE was on ladder welding legs to stacker and 2 employees below holding legs. Legs shifting causing 1 employee to fall off the ladder and the other employee to hurt himself while holding the ladder.

EE was exiting loader, felt sharp pain in lower back
Employee was inspecting the jaw crusher grizzly feed when he tripped and fell.
EE was stepping over conveyor frame, foot slipped and cut shin above the boot
Employee getting out of the man lift and slipped on some sand - went to grab himself and according to employee "popped his right shoulder out of socket." Employee stated that this had happened before and he was able to pop back in place, did not want to go to docotr, however called next day and wanted to see a doctor.
Area posted DANGER Do not enter unstable ground. Employee walking across to berm to retrieve rope that stabilizes float pump in retaining pond. Settling of the ground caused major cracks on top of berm. Employee heel on edge of crack as he attempted to step over it. Dirt gave way and he fell into the crack up to groin. Complained of pain in wrist, elbow, thigh and abdomin.
While inspecting a walk around, employee slipped into ditch causing strain to right knee.
Operator was trying to turn off the main water supply at the crusher to change a valve. The operator was standing on top of pipe turning valve when his feet slipped. He fell into the hole that was dug under the pipe in order to gain access to the bolts that secure the pipe. He sprained left ankle - notified supervisor but continued to work a normal shift day.
