Carried coats to plant, set them on a belt crossover. Turned to leave area when right foot caught a small pile of coal fines. Employee fell to the ground (same level) onto their right hip and elbow.

Individual was cleaning and raking along MN4 beltline XC96 when they tripped on wire rope falling to the ground. Put their arm out to catch themselves causing a fracture to the Greater Tuberosity.

While getting out of a tractor, hit head against the top in an area of low height. EE stumbled in the cab and fell, twisting left leg and felt a pop in left knee. EE continued working. A follow-up MRI revealed damage to the left knee that required surgery. EE began missing work when the procedure was performed 5/11/2020.

Our employee is a truck driver. Had just finished dumping coal, went to walk back to push in tailgate valve to relock the tailgate, when employee slipped in mud and fell on left side. Driver did not wish to seek medical attention immediately. Thought had just bruised side.

While exiting a tow tractor, slipped in an area of wet conditions. EE grabbed the tractor to keep from falling and felt a sharp pain in EE's left arm and shoulder. EE completed EE's shift before reporting the incident and continued working. Initially reported as first aid only, began missing work due to cervical fusion surgery on 08/15/2018.

While walking down the stairs at the refuge bin slipped on a step and caught self with both hands on the handrails. As ee did so ee felt a pop in left bicep that caused a burning pain. Diagnosed as a left bicep tendon tear and restricted to no use of left arm pending additional treatment.

While rushing to get into a ramcar caught his foot and fell forward into the opening. As he tried to catch himself struck his left arm against the plate behind the seat bruising his shoulder. X-rays were negative but he was placed on a no use of left arm restriction due to pain.

Our driver slipped and fell on ice in the parking lot by the stoker scale at Viper Mine. His right leg bent under his body when he fell and he landed on his right foot. Driver thought he was just sore and continued working, but went to the hospital the next morning and found out he had broken his ankle. Driver is off work but expected to return within a few weeks.

While going to her car in the parking lot slipped on ice and twisted her left ankle. EMTs applied ice to the ankle. She then went on her own to Taylorville Memorial Hospital where she was diagnosed with a fracture. She has not returned to work.

While working slipped in mud and fell landing on a large rock and striking the inside of his left forearm against a sharp edge. The impact resulted in a laceration that required sutures to close. He was treated and released to unrestricted duty reporting for his next scheduled shift.

While recovering Kennedy stopping material he stumbled and rubbed the back of his right arm against a rail where the clips had been snipped, leaving a laceration that required sutures to close. He was treated and released to unrestricted duty reporting to work his next scheduled shift.

At the end of his shift and as he finished showering and was walking to his locker he slipped in water on the floor. His feet went out from beneath him and he struck the lockers with the little toe of his left foot and big toe of his right foot causing lacerations that required sutures to close. He was treated and released to unrestricted duty.

While working on a tractor and climbing down missed a step falling about 6 inches and landing hard on his right leg. His right knee gave out and he felt a sharp pain. X-rays revealed no fractures or dislocations and he was released to the care of his own physician. Began missing work 1/24/2011 after an MRI revealed a medial and lateral tear of the right meniscus.

While working out of a scoop bucket slipped and fell landing on his right shoulder resulting in pain when pushing, pulling or lifting. He did not seek medical attention until after his shift. Initially a first aid only became a lost time after surgery to repair the shoulder. Began missing work 04/02/2011.

After bending down to place a magnet stepped through a hole in floor grating puncturing his left shin. Transported to St. John's ER Springfield for treatment including sutures and staples to close the wound. He returned to his next scheduled shift and has not missed any work.

While walking tripped and landed on his left heel. He continued working and completed his shift. Initially reported 6/3/2010 as a Minor First Aid became a Lost Time Accident 7/13/2010 after being diagnosed by his physician as a left calcaneal fracture although X-rays were inconclusive and directed to perform "no work until further notice".

While attempting to pull down a rib that MSHA had deemed a hazard and using a slate bar a small piece of the rib broke off allowing him to fall backwards striking the back of his head on an unknown object. The impact caused an open laceration approximately 1.5" long that required sutures to close. He returned to work after treatment without restrictions.

While working employee stepped into a hole and fell striking his right arm, shoulder and head and felt pain. Pain worsened and he called an EMT. Transport by ambulance to St. John's ER Springfield. No fractures were indentified and he was scheduled to undergo a series of physical therapy sessions making this a medical reportable event.

While walking on rocks the employee slipped and fell, striking his left knee on the rocks, causing pain. Transported to hospital for X-rays and no fracture identified. Advised not to work until released by physician.

After completing a repair job the injured employee was picking up wrenches, took a step and caught his boot on a rock and fell. He put his arm out to catch himself and felt something pop in his left shoulder causing pain when he tried to raise his left arm. Initially reported as a first aid accident on 12/6/2007 the employee began missing work after surgery 4/14/2008.

Employee was walking down refuse belt at prep plant. As he came to ground floor he stepped into hole caused by built up fly-ash causing right ankle to turn. He was taken to doctor and diagnosed with slight fracture to right ankle and advised not to work for three weeks.

Making pre-shift exam stepped in hole and twisted back. Back became sore and stiff sought medical attention on 10/31/06. Dr advised sitting work only. To follow up with doctor on 11/2/06.

Employee was repairing hose on scraper. As he climbed down ladder, foot slipped causing him to fall and twist left knee. He has continued to work since accident date but has had stiffness in knee. On March 29th 2007 he had orthoscopic procedure to repair knee. Due to work schedule he was able to return on normal shift 4/4/07.

Employee was walking beltline when he stumbled and fell to knees. Finished shift and worked the next two however pain and swelling persisted and he sought medical attention on June 10th. Doctor advised restricted duty work for one week with follow up visit on 6/16.

While hanging cable at power center, a piece of shale fell from between pins, knocking hard hat off and causing a laceration to back of head that required sutures. After treatment he was released to return to work. However, the following day he was complaining of tenderness and swelling in ankle and returned to Dr and was treated for a sprained ankle which kept him away from work.

Employee tripped carrying bolter plates and felt a pop in right side of neck/shoulder. He stated that earlier in shift he had walked into roof bolt jamming his neck. No medical attention was sought until November 17th at which time doctor diagnosed possible cervical strain and advised no work for seven days.

EE WAS WALKING ALONG BELTLINE AND STEPPED OFF INTO HOLE SPRAINING ANKLE, DOCTOR ADVISED RESTRICTED WORK ONLY

EE slipped getting off picker, fell between tire and steps bruising both shins. Continued to work the shift, went to doctor on 3-14-05 and was advised to work with restrictions for a week.

Employee was walking along belt line looking for water leak when he fell forward and strained left heel. Doctor recommended restricted activities which he was on for three days and then became lost time on 2/16/05.

Employee was working off a 4' step ladder cutting off low hanging roof bolts. The ladder was not set up properly and when he reached out the ladder slipped causing him to fall to the ground resulting in a muscle bruise in his lower right leg.
