Mostrando los 6Employee was servicing a 988B Caterpillar loader. While descending the left rear step, his foot slipped causing him to strike the ground, breaking his right femur.
The employee fell while dismounting his dozer, brusining his leg and knee. He went to the dr on 2/12/09 and received medical treatment. Fluid was drained from the knee. The company was not aware this had taken place until 4/3/09. Injury did not file a comp claim which would have alerted us. It was discovered in the corporate office on the results of his cdl physical.
While EE was dismounting his dozer he stepped on a rock, causing him to fall to the ground. He complained of back pain and numbness in one leg. He returned to his regular job the following day. While on his scheduled vacation he was diagnosed with a herinated disc. Placed under dr care and missed his first scheduled work day on 5/12/08.
Employee was on top of bulk truck poking hardened prill out w/ a poke rod when his foot slipped and cut right thumb on catwalk.
Some employees were removing lower rails on a 8200 dragline. They had removed some manhole covers to get inside the tub to assist in the removal of the bolts. The injured employee was shoveling grease and fell backwards into a manhole. The injury resulted in bruised ribs and a sprained knee requiring prescriptions meds and light duty for five days.
