Mostrando los 25Injured was driving 777C rock truck, when he hit a pot hole in roadway. His body left the seat when his body came back down in seat, the seat bottomed out. Injured stated he had pain in lower back.
Injured was pushing dirt and rock with a dozer, dozer ran on a rock, dozer hit the ground solid, when dozer came off rock. Injured cracked rib on left side. This accident was reported to management on 5/12/11.
Injured was taking end cap off of a push arm on a D-11 dozer blade. End cap came loose from chain striking injured on his foot bruised foot.
Injured was jump starting the battery on a 988B front-end loader. While disconnecting the cable, the cover fell, cutting left thumb. Three stitches required for cut to left thumb.
While putting the track back on a cat excavator, a chain became fouled between the segment and track. Another chain was attached to each side of the track so it could be lifted up to relieve pressure on the fouled chain. One of the hooks on the track chain came loose and struck the EE on the chin, causing two lacerations that required stitches.
EE operated various types of equipment on this date along with other manual labor tasks. He experienced severe back and leg pain after the shift ended. He sought medical treatment on 9-26-09. He reported this on 9-28-09 and has not returned to work as of this date 9-29-09.
The victim was assisting the mechanic in charging the air conditioning system. He placed his hand behind the operators seat at the air discharge area to see if it was cooling. Unknown to him the guard over the fan blades was damaged. He contacted the moving blades, cutting his finger which required stitches.
Employee was servicing a 988B Caterpillar loader. While descending the left rear step, his foot slipped causing him to strike the ground, breaking his right femur.
The employee fell while dismounting his dozer, brusining his leg and knee. He went to the dr on 2/12/09 and received medical treatment. Fluid was drained from the knee. The company was not aware this had taken place until 4/3/09. Injury did not file a comp claim which would have alerted us. It was discovered in the corporate office on the results of his cdl physical.
EE was injured 4/23/08 at another operation. He returned to work 7/7/08 and transferred to this site where he complained of back pain due to backing his dozer over a rock and creating a severe jolt.
While EE was dismounting his dozer he stepped on a rock, causing him to fall to the ground. He complained of back pain and numbness in one leg. He returned to his regular job the following day. While on his scheduled vacation he was diagnosed with a herinated disc. Placed under dr care and missed his first scheduled work day on 5/12/08.
Injured was in the process of removing a track roller on the left side of a D11R Dozer when as the last bolt was removed the roller fell approximately 6 inches onto a impact wrench the hit the right index finger. Injured returned back to work from the hospital where he was treated.
Employee was on top of bulk truck poking hardened prill out w/ a poke rod when his foot slipped and cut right thumb on catwalk.
Some employees were removing lower rails on a 8200 dragline. They had removed some manhole covers to get inside the tub to assist in the removal of the bolts. The injured employee was shoveling grease and fell backwards into a manhole. The injury resulted in bruised ribs and a sprained knee requiring prescriptions meds and light duty for five days.
Employee was dismounting service truck when a side door which was not latched swung around, striking knuckle on left index finger causing laceration.
EE WAS CHANGING HYDRAULIC OIL FILTER ON A CAT D11 DOZER. WHILE CHANGING THE FILTER HE STUCK A WIRE FIBER IN HIS LEFT THUMB. SMALL PUNCTURE WOUND TO LEFT THUMB.
EMPLOYEE WAS FUELING A ROCK TRUCK. HE HEARD A NOISE AND WALKED ACROSS FRONT OF TRUCK AND LOOKED UP AT BATTERIES THAT HAD SHORTED OUT AND EXPLODED. ACID SPRAYED IN HIS EYES AND FACE. EMPLOYEE RECEIVED MINOR BURNS.
EE was operating dozer in hollowfill. Dozer operating temperature was rising. EE stopped the dozer to check radiator and while raising cover the radiator cap blew off spraying coolanton ee burning his right wrist, shoulder, face and neck.
EMPLOYEES EXPOSURE TO RESPIRABLE DUST OVER HIS WORK HISTORY.
Submitted to comply with Part 50.20 30 CFR. EE was operating bulldozer in a hollow fill reclaiming. EE stated he was hurting all over & was sick. Taken to hospital; diagnosed as viral infection. At a later date 2nd physician diagnosed degenerative joint disease (arthritis). EE informed Dr. that he was jarred by the dozer & was work related. Invest. ongoing.
EE HAVING CHEST PAINS AND GOT HIS SON TO TAKE HIM TO DR
THE EMPLOYEE (A MECHANIC) WAS WORKING ON HYDROSEEDER ENGINE WHILE IT WAS RUNNING. THE WRENCH SLIPPED AND HIS HAND HIT THE FAN. IT BROKE 2 FINGERS AND HE RECEIVED A CUT ABOVE THE KNUCKLE.
Employee was operating a Cat articulated truck hauling spoil to dump area of the hollow fill. Upon second load he attempted to back into the dump site and for whatever reason backing over slope he exited or was ejected from truck suffering severed right foot, cut and abrasions to rest of body. Per company call 5-27-05, leg was amputated to 5 inches above ankle - coder 12.
EE is a consultant. he was taking pictures of dozer tracks and tires. he was standing on the left side of the haulroad. the grader was backing up on the right side. he had his camera to his face and backed up into the grader and was struck by the left front tire.
