Employee was descaling #2 Washer. While setting down the air-powered scaling tool, the employee lost footing resulting in a back strain.

Employee was stepping off of a skid steer. While doing so, the employee slipped and caught left foot between the bucket and frame resulting in a strain / sprain of the left knee. Restricted duty started 8/4/2016.

Employee was walking down a stairway carrying a trash bag. While doing so, the employee missed a step and grabbed the handrail while off balance resulting in a dislocated left shoulder.

Two employees were pulling a slack rope to secure a ship at the dock. While pulling, one employee tripped resulting in a hernia.

AT operator was standing in engine that was connected to empty rail cars. A KCS engine pushing empties coupled to the AT train causing a bump that knocked AT operator off balance.

The employee allegedly tripped on a cable as he was attempting to walk over it. The cable is used to secure one dock barge to another dock barge. The employee stated that he felt pain in his back following the alleged incident.

Precip operator stepped into 4" x 14" floor opening resulting in abrasions and contusions.

Employee dismounted from Gantry 101 bottom steps, slipped, and fell on hands and knees on 4/12/2015. Employee was placed on restricted duty on 4/23/2015.

Employee removing jib crane hook from a load, stepped backward onto wooden pallet and tripped. Employee attempted to cushion fall with right arm resulting in right wrist fracture.

Employee was stepping down off of #1 filter and felt something pop in left knee. Employee placed on restricted duty on 11/24/2014.

Associate was walking up the stairs from checking on the 2 belt tail pulley when he slipped and put his left hand out to break his fall. He felt something pop in his left wrist.

A Lab employee tripped on a floor mat in the lab. The employee reached for the exit's door frame to catch himself and felt a pain in his right bicep. The employee was directed to medical and restrictions were issued including lifting <=5 pounds, no pushing or pulling and no overhead work. The employee was released to return to lab work only. Additional diagnosis to be conducted.

EE was removing equipment. He climbed the ladder of the jib crane to top of the loader. The finder on right side is used as a landing support for ascending & descending the ladder. He fell & slipped off falling onto the tire then to the floor. Swelling in ankle. Diagnosis: Acute exacerbation osteoarthritis right knee: occupational illness. Rst on 6-11-13, Lost time on 9/4/13.

Employee was working in the Jamaica Building preparing to clean belt hopper. While descending stairs, he stepped into wet bauxite and slipped, hurting his right shoulder. He returned to work with no restrictions. Pain continued to worsen and on 4/8/13, he was diagnosed with a torn rotator cuff (right arm)and returned to work with restrictions.

Employee was bending down and twisting to step down from an 18 wheeler trailer when he felt his left knee pop. This caused him to lose his balance, falling to the ground, landing on his feet. At that point, his knee gave out on him. He was diagnosed with lateral meniscus sprain and on 3/22/13 his duties were restricted.

EE fell against scaffold while left foot went through cable tray and left elbow and arm jammed against scaffold. Prior to this accident he had injured his right knee years ago. Continued to complaint of pain, stiffness, and swelling of right knee. On 10/25/12 he was evaluated by an orthopaedic specialist. On 12/4/12, his work duties were RESTRICTED.

Employee was pulling on a lineup bolt to get the bolts back in a flange when he slipped, hitting the left side of his head on the corner of a drain valve. He sustained a 1" long laceration just above the ear. He received 3 staples as a result of the injury.

Employee was performing lubrication duties on #9 belt above West Silo. While moving from one lubrication point to the next, he slipped and fell backwards. He fractured his left fibula.

EE was assisting in replacing the long cylinder on #7 press. He climbed down to the shelf of #7 press to cut the nut holding the long cylinder. When he cut the nut the press jumped and he stepped back and slipped off the shelf and slid into the hopper about 8' down. He fractured a bone in his right leg. His first day of lost time is 10/12/10.

EE was walking behind the conveyor on the north side of the dock where scaffold boards were laying (in water) and tripped. As time went on, the pain progressed and knee began to swell. He saw a physician and was ordered to return to work with restrictions of no climbing from physician at Baton Rouge Orthopaedic Clinic on 9/28/10.

EE was operating a CAT 325 Marsh Buggy and a hydraulic line broke. He got out of the cab to look and slipped on hydraulic fluid that was on the track. He hit his right knee on the cleat of the track.

Employee was unplugging a cyclone on Tank 38. When he loosened the bolts, liquor spewed out and while exiting the platform, twisted his right ankle. On 8/27/10 a hairline fracture was discovered. He has been placed on restricted duty.

Walking on wet concrete, EE slipped and caught himself with both hands in the push up position causing pain & swelling to his wrists. He was treated and returned to work with no restrictions. After continued complaints of pain, he returned for additional x-rays which revealed a fracture, restricting his work activity.

EE was stepping into filter house when his foot caught the bottom part of the door causing him to fall forward. He struck the left side of his face and ear on the angle iron that holds the filters in place, causing a laceration to the face and ear. He had surgery on his left ear on March 12, 2010. His first full day of lost time is March 11, 2010.

EE fell while in his office in the Engineering Bldg. During the fall he struck the back of his head on the edge of a table. EE received a laceration to the back of his head and indirectly experienced a subdural hematoma to the right frontal lobe.

EE lost his footing on the second rung of platform stand 3-4' off the concrete. He landed on his back, causing abrasions on his back and left elbow. First aid was administered. He was placed on restrictions on November 30, 2009.

EE was inspecting the temporary conveyor set up in Section IV. He was walking down a sloped area and lost his footing. He tried to regain his balance & twisted his left knee. His work duties were restricted.

On October 6, 2009 the heel of employee's left foot got caught on a step causing pain in left knee. Because of symptoms worsening, he was referred to an orthopedist and was diagnosed with a torn meniscus. He was admitted into the hospital for surgery on November 16, 2009 which is the first day of lost time.

Employee was taking settler overflow sample when he slipped and fell onto tank. Landed on his lower back and right arm. Received 2nd degree thermal burns to the lower back and right upper arm. Because of this injury, he did not report to work for one day of his shift. He returned to work on 9-24-09.

On 8/1/08, EE reported that he slipped while descending the stairs on 102 gantry. EE did not fall, but caught himself with his arms on the hand rails. As a result of this event, EE complained of shoulder pain that did not improve. On 10/30/08, EE was placed on work restrictions.

Employee was attempting to climb the 101 Gantry when he lost his footing causing him to slip & fall. He fell from the second step to the ground. Losing his footing caused a strain to the groin muscle. He was placed on restricted duty.

While walking to change #5 belt, employee slipped on wet bauxite and twisted his back.

Employee reported that he lost his footing after putting the #4 Decanter Recirculating Pump in service. He felt a sharp pain in his lower back. He stated that he sat down, and could not get up. He returned to work with restrictions.

Employee was working on ground floor in Slurry Mix Area & slipped & fell to the ground. Contusion to left wrist, elbow & side rib cage. On 7/12/06 he received a steroid injection which then classified it as a Medical Treatment.

EE slipped and hurt his right lower back when he was attempting to climb stairs at the platform by #3 washer underflow pump. The area he was trying to walk through was covered with mud. The injury was initially classified as a First Aid Case, but as treatment progressed, restrictions were provided.

Employee was walking down the ramp of a utility trailer used as work station storage. He slipped and fell, making contact with a piece of angle iron on the bottom, which caused a laceration requiring sutures to close the wround. He was then released to regular duty.

EE was standing on a process line to access a valve & close it. After closing the valve & preparing to leave the area, the ee slipped & fell, hitting his back against the flange of a 3" discharge line. He returned to his office where the pain in his lower back became intolerable. EE was examined at St James hospital where he was treated for a back strain/contusion & released.

Employee was closing purge water valve on West Relay Pump in Slurry Mix. In doing so, employee slipped, but caught himself with his right hand on the ground. He experienced no pain, but his wrist appeared swollen. X-rays were negative, but later complaints of soreness/pain resulted in work restrictions.

Employee was getting a Caustic Sample & while approaching vent valve on Press #10 he lost his footing, began falling and broke the fall by grabbing the sample trough. He felt a "pop" in his right knee. Employee was treated conservatively until he saw an orthopedic specialist on 5/10/05 at which time he was placed on restrictions which prevented him from doing his full job.

Employee putting caustic mix tank to circulate, heard loud noise, & began running. Employee was exposed to liquor and while running, he slipped, bruising left knee & shin. Medical provided first aid for minor first-degree burns to face area & contusion to left knee & shin and he returned to work. Reported 4/12/05 to company doctor complaining of soreness in hamstring muscle.

On March 9, 2005, employee reported that he twisted his right knee when he slipped on the steps of Gantry 101 on 3/7/05. This alleged knee was treated using ibuprofen & occassional. The employee continued to have complaints of knee pain, & on 12/13/05, he was provided work restrictions that prevented him from performing his normal job, pending further treatment.
