EE was stepping off the left side of a Caterpillar Dozer D9N as he stepped onto the push arm step twisting his ankle and fell to the ground, injuring his mid to lower back.

Employee was removing forms from the inner wall of the raw coal tunnel. While making a move to the next panel he was relocating his positioning hook at the same time the loose panel fell causing him to loose his balance and fall from a distance of approximately 6 feet.
