Employee stated EE was assisting in setting the feeder after a belt move when EE stepped in a hole and felt pain in EE's right knee.
Employee stated was cleaning around #1 Belt take-up, as EE walked inby the take-up EE stepped on a piece of 6" discharge waterline that was partially buried in the mud. This caused EE to fall to the mine floor on right hip. Employee complained of pain in right hip and groin area.
Employee stated EE was loading bolt supplies in the scoop bucket at the supply hole when EE slipped on oil that was in the scoop bucket and fell on EE's right foot. EE was treated at Raleigh General Hospital where EE was treated for a sprained right ankle.
Employee stated that EE was carrying a stopping block from the scoop bucket to where they were building a stopping when EE's feet got caught up in the bands from the block. This caused EE to trip and fall where EE struck EE's right hand on the mine floor which resulted in a fracture to EE's hand.
Employee stated EE was walking back to EE's scoop when EE slipped and fell landing on EE's right hip.
Employee stated EE pulled up to the 993K loader to install steps. EE stated EE got out of EE's truck to begin the task. EE walked to the back end of the loader and decided to reposition EE's truck for better lighting. EE turned to walk back to EE's truck tripped and fell over a rock in the work area injuring EE's left shoulder.
Employee stated that they were stepping out onto the crawlers of the 325 CAT Longarm when the cat pad they stepped on shifted causing them to lose balance and they fell off the crawlers to the ground injuring their head, back, and neck.
Employee stated: was installing a rib bolt on the left side of the entry when employee's foot slipped off the platform. As employee was falling, employee tried to catch self and employee's right arm went between the mast cradle and the mast jack causing a fracture to right forearm.
Employee stated was in the #8 entry. As EE was setting the miner over in the cut, EE was walking backward, slipped on a rock and fell. Employee stated felt pain in left ankle and left knee.
Employee Stated after hanging a back-up curtain in #8 entry, as they turned to walk away they slipped on a fly pad that was laying on the mine floor hurting their right knee. We are submitting this under protest per MSHA recommendation, we feel we don't have enough medical proof until we receive the results of the MRI.
Employee stated was mounting the 980G Front End Loader (C/N#21105), when EE go to the 2nd step and fell. EE fell backwards landing on left arm causing it to break.
Employee stated they were walking from #6 entry to #7 entry to hang fiber cable, when they stepped in a hole that was approx. 10 to 12 inches deep, and felt a pain in left knee, Employee started missing work on 10/6/21 when they went to the doctor, we do know they have had problems in the past, not knowing if this needed to go on a old claim is why we are late filing it.
Employee stated EE was putting EE's rubber tire ride on charge on the surface and was walking around the ride to get EE's tools when EE's right knee gave out and buckled causing EE to drop to the ground.
Employee was moving op shuttle car anchor. When they got out of shuttle car they slipped on a ledge hurting right knee
Employee stated as EE was walking to the ride, EE glanced up to look for a drop cord that EE thought the charger was plugged up too, not noticing the draw rock on the mine floor causing EE to trip and fall.
Employee stated was walking the belts and when getting done with C-Belt, employee stepped down, twisting left knee and fell on the ground. Employee worked until 7/24/2020 at which time employee had surgery and began to miss work.
Employee was dismounting D11 dozer when EE's foot slipped on bottom step. Employee struck the top part of push arm with EE's side resulting with injury to the left rib area.

Employee slipped while descending a slope, falling on the chainsaw EE was carrying, cutting the left upper arm.
Employee was setting a dewatering pump along a beltline. While trying to get the pump into the correct position the latch on hook broke. The pump moved causing the employee to slip and employee's left knee struck the pump.

Employee stated EE was attempting to pull the oil feel hose out to put oil in the MRS when the hose came loose employee fell against the plow of the MRS hurting EE's ribs
Employee stated that EE was in the process of pulling cable off of the #362 Shuttle Car when EE slipped on slick mine floor causing EE to twist EE's right knee. Employee stated that EE's knee started to tighten up immediately.
Footing got stuck in mud carrying a drill reamer. Fell due to feet stuck in mud and injured lower back.
Employee stated they were walking on section when they stepped into a hole and pulled something in their buttocks. Employee sought medical treatment and was placed off work.

Employee stated was rock dusting #8 rt entry when foot struck a rock causing employee to stumble while trying to catch self. Employee states during this motion employee strained lower back and left knee. Employee sought medical treatment on 5/21/2019 and was placed under doctor's care.
Employee stated ee was traveling across the section when ee stepped on a slick spot where the shuttle cars were traveling, causing ee to slip and twist left knee. Employee sought medical treatment and was placed off work due to a strain.
Employee stated EE was working to remove suction line on the bottom floor. Employee stated EE was standing in coal fines when EE turned to reach for a wrench but EE's foot remained stuck causing EE to twist EE's knee and ankle. Employee reported incident on 5/8/2019 and sought medical treatment. Doctor placed injured employee on restrictions due to sprain ankle until 5/13/2019
Dozer operator stepped out of equipment cab onto tracks, slipped and fell on track fracturing 3 ribs.
Employee was dismounting a bolter when their feet became tangled and fell to the mine floor, injuring their back and right shoulder. Employee sought medical treatment on 4/10/2019 and was taken off work under doctors care.

Employee stated ee was climbing up the steps of a 785D rock truck when ee reached to grab the handrail. EE hand slipped, causing ee to fall off the steps and hit the ground, landing on shoulder. Employee did not start missing work until 12/12/2018 due to having surgery on shoulder, resulting in a reportable accident.
Employee was unloading a D11 cutting edge out of a pick up truck when the employee slipped and fell forward striking forehead on the cutting edge. This resulted in several stitches on forehead.
