Injured was attempting to dismount Volvo rock truck with wet muddy boots and EE's foot slipped while climbing down steps causing an unexpected load on EE's left shoulder resulting in a sprain/strain type injury. Injured started missing work on 3/13/26 and was put off work pending an MRI. No problems with equipment or compliance issues, training or PPE.

Injured was carrying tools past 2 parked mantrips in 8W belt spur. While walking between mantrips EE stepped on an uneven surface causing EE to fall. Injured started missing work on 2-18-26. No problems with equipment or compliance issues, training or PPE.

Injured was walking away after throwing switch at Sugar Run slope back switch when EE slipped on the rail, falling onto the point rail. Injured started missing work on 2-19-26. No problems with equipment or compliance issues, training or PPE.

Injured was rib pinning in sump at St Leo Mains at 129 block, while walking around the bolter, EE stepped in a hole and twisted EE's left knee. Injured started missing work on 1/27/2026. no problems with equipment or compliance issues, training or PPE.

Injured was hanging a run through curtain up in #2 entry of St Leo Mains 131-132 block, standing on the third rung of a 4' ladder leaning off the ladder to install a nail through the curtain when EE fell off the ladder and bruised EE's back when landing on the mine floor. No problems with equipment or compliance issues, training or PPE.

Injured standing in first crosscut outby 8 west equipment doors near the end of loose take up rails while a scoop was unloading a double stack of take up sections on top of the loose rails. Duckbill drug across a loose rail, kicking it up when it got to the end of the rail striking injured, knocking EE down and landing on EE resulting in a fractured vertebrate.

Having trouble fixing an air tugger, EE was on the platform and coworker just looked at them and had eye contact. Coworker turned to keep working with third person and they heard a noise and EE was on the ground. No one knows exactly what happened. The assume that the safety catch for the drill steel came out.

While making preshift examination on 6 West belt, just outby the middrive, the examiner miss stepped on left foot, stepping on its side and rolling the ankle, resulting in a fracture to the left fibula. While falling they struck left elbow on belt structure resulting in a chipped bone. No problems with equipment or compliance issues, training, or PPE.

Injured was on 3rd rung of ladder in the 13 block intersection #2 entry of 7 west, leaning over trying to hook the end of a chain tape measure onto a surveying spad when the employee fell off the ladder and fractured right wrist upon landing. No problems with equipment or compliance issues, training or PPE.

Injured walking down 6 belt at 135 block using a leaf blower to turn over the rock dust and rolled right ankle (same ankle EE twisted a month ago) resulting in a strain/sprain type injury. No problems with equipment or compliance issues, training or PPE.

While descending ladder in 6W #2 entry after hanging high line during a power move the injured stepped off the ladder into a minor depression in the mine floor, rolling right ankle and resulting in a sprain/strain. No problems with equipment or compliance issues, training or PPE.

While walking in the Miracle Run Loaded Track injured tripped and fell, striking left side of ribs against mine rib, causing a contusion. Examination of area revealed no tripping hazard, and EE does not know what EE tripped over (maybe track rail). No problems with equipment or compliance issues, training or PPE.

The injured was hanging belt structure using a step ladder in 7W Belt Trench. A scoop in an adjacent entry caught a cable running through the trench. The cable pulled contacting the ladder knocking the EE to the ground. EE began missing 5/1/24.

The EE got up from being seated and tripped over a piece of wood striking left knee on the ground. EE worked until 1/29/24 where EE began missing.

The EE was climbing down off of D-7 dozer. The injured slipped on the step at the blade arm and fell. EE's left ear struck the dozer cat, causing a laceration. EE received 4 stitches, and did not miss any scheduled shifts.

The injured EE was climbing up on #2 D-10 dozer at the stockpile. As EE pulled self up onto the tracks, EE's right foot slipped and EE fell from the dozer. EE began missing work on 9/25/23.

The injured EE was walking down the steps between the 3rd and 2nd floor of the Prep Plant, when EE twisted their left knee. EE worked until 7/28/23, and after the shift, EE began missing.

Walking down 8 Belt and tripped, landing on left shoulder resulting in a contusion. No problems with equipment or compliance issues, training or PPE.

Employee was exiting mantrip at end of shift and twisted/sprained right knee. Employee was not able to put weight on right knee.

The EE was carrying Cement blocks, when EE tripped over the plastic banding that secures the bundle of blocks. The EE caught self before falling to the ground and felt a pain in lower back. The EE was wearing PPE. Started missing work 5-18-23, Returned 5-23-23.

The injured was assisting in loading DMU parts on a duck bill scoop. The EE took a step backwards, and tripped. At this time EE felt a pop in left knee. EE finished the shift and began missing 4/18/23.

The injured employee tripped over the cable handler of a loading machine, twisting a knee and striking lower leg against a shuttle car waiting to be loaded. This resulted in a torn ligament and a fracture to the lower leg. It did not involve any compliance issues, job skills, proficiency, training, PPE or protective devices.

While tightening nut on a gear box at the Prep Plant with a pipe wrench the wrench slipped causing injured to fall forward and hit EE's upper groin/lower abdominal area on a support brace causing a contusion. No problems with equipment or compliance issues, training or PPE.

While walking to a date board near the 60# track switch EE tripped over a crib block that was partially obscured by curtain and experienced pain in previously injured right knee. This is not a new injury, and probably did not really occur. No problems with equipment or compliance issues, training or PPE.

The employee was taking inventory in Metz Warehouse. The injured stood on a box to gain height. The box gave way, and the EE fell back striking their head on the forklift parked behind them. EE received 8 staples in their head.

The employee was going from the 2nd floor in the plant to the first floor. The employee tripped and caught self with hand injuring the ring finger. The employee continued to work until 1/6/23, where employee began missing.

EE was walking along the 9 North Track when EE tripped and went down on right foot twisting right ankle.

The EE had just climbed over the GOB plate on the LW face. As EE was stepping out into the TG entry, EE's left foot went in a hole in the pavement. This caused EE's ankle to roll. EE suffered a fracture above the left ankle.

Injured was walking down an overcast ramp on the 9 North Mains section and got EE's feet tangled in wire screen that was located in the rubble of the ramp, causing EE to trip and fall, injuring EE's previously surgically repaired right knee. No problems with equipment or compliance issues, training or PPE.

While carrying belt structure along the 4 West belt line to advance the belt line the injured stepped in a hole which resulted in a muscle strain to EE's lower left leg. No problems with equipment or compliance issues, training or PPE.

The EE was shoveling the Longwall Transfer area. EE was walking backwards, when EE tripped on some coal fines, and fell injuring EE's back. EE began missing on 8/15/22 when EE went to seek medical treatment.

While employee was working in a belly pan replacing discharge rubber someone started a water pump which started filling belly pan with water. As EE was evacuating the belly pan EE slipped and aggravated right wrist that had underwent carpal tunnel surgery on 7/11/22. No problems with equipment or compliance issues, training or PPE.

EE was walking past #4 shield on the LW face. EE's toe caught a D-ring on the relay bar, and EE tripped. As EE was falling, EE reached with right hand and was able to grab a chain. This caused the EE to strain shoulder as EE felt a pop.

EE was walking 9 North #1 Belt and slipped in a muddy area at 10-11 block. After slipping the EE felt pain in EE's left knee. EE began missing work 7/30/22.

The EE was in the process of changing a hose on the head of the miner in the 9 North Mains Section. The cob was already removed, and as the EE began to get up, EE slipped and fell against the head. This caused a laceration to EE's left wrist. EE received 6 stitches, and began missing work on 7/28/22.

EE was working at 9 ML, pulling new belt on. The shive rope was pulled tight. The injured tripped over the rope and fell against a belt cradle laying against the rib. The EE received 5 stitches, and returned to work the next scheduled shift.

EE was hanging life line on 9 North Track, connecting 3 West's secondary escapeway. EE stepped on the track rail, and EE's knee buckled causing the injured to fall. EE began missing on 5/31/22.

EE was walking down the slope to work on the track rails. The EE slipped on the pavement, and rolled left ankle. The EE began missing work on 5/10/22.

The injured was walking down the Slope track pre-shifting the area. The EE slipped on some loose material and fell. EE injured lower back, and began missing work on 4/11/2022.

Injured employee was working in the 9 North Mains as a general inside laborer. When EE stepped out of a scoop EE felt EE's left knee give out, and this occurred several more times during the shift, resulting in a strain/sprain type injury to the left knee. No problems with equipment or compliance issues, training or PPE.

The injured EE was dragging a rock dust hose. While dragging the hose, the EE tripped over the hose. When EE fell to the ground EE struck EE's back on the track rail. Incident occurred in the chutes area of Sugar Run. EE began missing work 3-21-22.

The EE was pulling a whip line attached to a breast drill. The injured stepped down on a piece of coal and fell, injuring right knee. The EE worked until was scheduled for surgery on 5/18/22. EE began missing on that day.

EE was the utility man on the 4W mining section. EE had climbed on the miner to hang up a dog chain for a ventilation tube. The EE slipped off the miner, catching left arm on the rib drill, injuring the left shoulder. EE began missing work 2/16/22.

The EE was walking 5 belt doing a pre-shift examination. At 54 block, the EE tripped and fell against the belt. There was no obstruction noticed by mine management that would create a tripping hazard. EE began missing work 2/16/22.

The EE was exiting vehicle in the parking at Miracle Run. As EE stepped out, EE slipped twisting right knee. The EE sought medical attention on 2/15/22, and began missing 2/16/22.

The injured EE fell in the shower after tripping on their sandal. The EE lost balance, fell forward, and struck their face on the shower nozzle. The EE received stitches above eye and a fracture to their face. EE began missing work 11/1/21.

The injured EE got off a mantrip. As EE was walking to tool box located in the crosscut, they felt a pop in knee. EE was unable to put much pressure on it, and was taken outside. Began missing work 10-18-21.

EE was knelt down putting together a dust hose, when EE lost balance. As the injured fell, EE put hand up to catch self. The EE's right ring finger came into contact with a bottom roller. EE's finger was pulled and caught between the belt and bottom roller.

The employee was examining 6 ML Belt. EE ducked under a waterline, as EE did, EE stepped in a ditch twisting right knee. The employee was scheduled off and then saw a specialist on 7/23/21. At this time EE began missing work.

Employee slipped on stairs in the prep plant going down from the 10th floor to the 9th floor. When the employee slipped they grabbed the handrail and strained left shoulder and also over extended and strained right hip. Employee was wearing the proper PPE. Employee did not start missing work until 8/2/2021.
