Injured was attempting to dismount Volvo rock truck with wet muddy boots and EE's foot slipped while climbing down steps causing an unexpected load on EE's left shoulder resulting in a sprain/strain type injury. Injured started missing work on 3/13/26 and was put off work pending an MRI. No problems with equipment or compliance issues, training or PPE.

Injured standing in first crosscut outby 8 west equipment doors near the end of loose take up rails while a scoop was unloading a double stack of take up sections on top of the loose rails. Duckbill drug across a loose rail, kicking it up when it got to the end of the rail striking injured, knocking EE down and landing on EE resulting in a fractured vertebrate.
Having trouble fixing an air tugger, EE was on the platform and coworker just looked at them and had eye contact. Coworker turned to keep working with third person and they heard a noise and EE was on the ground. No one knows exactly what happened. The assume that the safety catch for the drill steel came out.
While making preshift examination on 6 West belt, just outby the middrive, the examiner miss stepped on left foot, stepping on its side and rolling the ankle, resulting in a fracture to the left fibula. While falling they struck left elbow on belt structure resulting in a chipped bone. No problems with equipment or compliance issues, training, or PPE.
Injured was on 3rd rung of ladder in the 13 block intersection #2 entry of 7 west, leaning over trying to hook the end of a chain tape measure onto a surveying spad when the employee fell off the ladder and fractured right wrist upon landing. No problems with equipment or compliance issues, training or PPE.
Injured walking down 6 belt at 135 block using a leaf blower to turn over the rock dust and rolled right ankle (same ankle EE twisted a month ago) resulting in a strain/sprain type injury. No problems with equipment or compliance issues, training or PPE.

The injured was hanging belt structure using a step ladder in 7W Belt Trench. A scoop in an adjacent entry caught a cable running through the trench. The cable pulled contacting the ladder knocking the EE to the ground. EE began missing 5/1/24.

Employee slipped on stairs in the prep plant going down from the 10th floor to the 9th floor. When the employee slipped they grabbed the handrail and strained left shoulder and also over extended and strained right hip. Employee was wearing the proper PPE. Employee did not start missing work until 8/2/2021.
