Surface employee tripped/fell near rail yard and suffered minor abrasions/contusions to left hand, right knee/thigh & right chest. Medical attention was offered and declined. 3 days later (3/28/25), employee requested to be evaluated and was diagnosed with abrasions/contusion to right knee, left hand & right chest wall. *Became reportable on 3/28/25**

EE USING AN OIL DRUM CART/DOLLEY TO MOVE A FULL 55 GALLON DRUM IN SURFACE LUBE ROOM. EE PICKED UP THE DRUM WITH THE CART, SLIPPED AND FELL. THE DRUM AND CART LANDED ON TOP OF EE, PINNING EE. EE WAS ABLE TO REACH EE'S CELL PHONE AND CALLED FOR HELP. EE WAS LOCATED IN A MATTER OF MINUTES; THE DRUM WAS LIFTED OFF OF EE AND EE WAS TRANPORTED VIA EMS TO THE HOSPITAL
Employee was performing visual inspection of equipment while standing on step (attached to machine) elevated approximately 18 inches from ground level. Step failed/broke at the weld point causing the employee to fall on their left arm which led to a dislocation of the employees shoulder.
Employee was exiting the cab of a pay loader during a snowstorm. While turning around in order to descend the ladder, they slipped and fell to the ground below. The employee fell on their feet but fractured their right fibula.
A surface maintenance employee was working on an e-stop PM for the OCS gallery. Upon completion at 1 belt tail, the employee walked back to golf cart that was parked at 14 belt head. During the employee's walk back to golf cart, they slipped on ice that they did not see. The slip and fall resulted in an injured lower right leg.
IE was assisting with lowering an apparatus into a reclaim tunnel. The IE pushed on the lowering cable when the load shifted causing the IE to fall toward the hole. The IE jumped across the corner of the hole and landed on ankle. IE's right ankle rolled backward causing a dislocation and a fracture of the lateral malleolus.
The employee slid down a small slope and hit an obstruction forcing EE to stumble. EE tried to catch self with left arm, resulting in a dislocated left shoulder.
An employee working at the UG stacker slipped when stepping down from the loader EE was operating. The employee had one hand on the handrail when EE slipped, and as EE fell off balance, EE swung sideways into the machine and contacted EE's left shoulder on the frame of the equipment. Several months of diagnostics were performed until medical treatment was administered on 11-1-19
Upon completion of repair work an employee rolled ankle while stepping down from the cab of a face drill. Upon follow up visit to a physician on 10/11/2018 the employee received restrictions and medical treatment that made the incident reportable.
January 26th employee was descending from Tractor the heel of his boot caught the bottom step. The employee lost balance and attempted to catch himself from falling; sudden movement caused a pop in the employees lower back. The employee declined going to the doctors. January 28th the employee asked to see a doctor due to pain. The employee was prescribed medication for pain.
The employee was running the hammer. He was going down steps when his right heel got hooked and he fell forward to the cement and stuck his left hand out to break his fall. His left middle finger was jammed and he received what is referred to as mallet finger that needed to be surgically repaired through outpatient surgery.
While descending the LHD ladder way, the maintenance employee lost his balance as he stepped to the ground. He stumbled backwards, fell to his backside and contacted his head on the structure of a spare tail piece near where the LHD was parked. The employee received staples to close the wound to the back of his head.
An employee was sitting in his gator taking a break. He stood up to go get a drink and due to an underlying medical issue blacked out falling forward and contacting the lower half of his face with the floor. He was transported to Lutheran hospital and treated for a cut on his chin and a broken jaw. The employee is still under evaluation by personal physicians for medical condition.
While employees were hoisting a piece of steel which had been removed from the S1 Screen, the weld broke on the pick point allowing the piece of steel to fall, ricochet off the screen and strike the EE knocking him to the floor. This impact caused bruising to the upper thigh, minor laceration to the back of head, and minor fracture to the EE's foot.
Employee was working on a water heater. His right foot got caught on a copper tubing and he tripped. He fell, landing on his right knee, followed by his face and then his hands. He received surface abrasions on his face and right knee and had a stiff neck and back. He was released to full duty. But on January 31,2012 he was taken out of work pending MRI and treatment.
Employee fell when poorly designed steps failed. Employee ended up with a strained AC joint that needed prescription medication to repair.
While making a right hand turn the door of operator's pickup opened and his balance was thrown off. Grasping the wheel, as his body shifted, the truck was turned to the left and impacted a pillar. Upon impact the operator fell to the ground.
Was at the back of the truck checking load and fell of the back--he apparently caught his leg between the back doors and crushed some tissues--x-rays show NO broken bones--He will return to work on 8-06-2010.
The employee was seated on a park bench smoking when he leaned back on the back legs and the supports failed causing him to fall backwards. The employee hit his head on a concrete post knocking his hard hat off and then made contact with the pavement. He was put on restrictions for one shift and given prescription medication for the pain.

Employee was exiting powder truck, stepped down back first. Landed on left foot first, but rolled/twisted ankle. Employee was examined/x-rayed, diagnosed as sprain. Restricted duty 5 days. Return to full duty 12/19/05.
AN ADMINISRATIVE EMPLOYEE MISSED THE BOTTOM STEP TRAVELING DOWN A FLIGHT OF STAIRS. EE LANDED ON THE OUTSIDE OF THE FOOT & ROLLED THE RIGHT ANKLE. THE EE WAS TRANSPORTED TO THE OCCUPATIONAL HEALTH CLINIC AT LUTHERAN HOSPITAL WHERE X-RAYS WERE NEGATIVE. THE PHYSYCIAN DIAGNOSED A MINOR SPRAIN & RELEASED THE EMPLOYEE TO RETURN TO WORK WITH NO RESTRICTIONS.
employee was re-palletizing 80# bags of salt. She crossed the warehouse carrying a bag and slipped on a moist floor that had some salt spillage on it. This caused her to strain her lower back.
EE WAS CARRYING A CANNISTER OF AFEX POWDER USED FOR FIRE SUPPRESSION ON THE SURFACE FRONT END LOADERS. HE HAD THE CANISTER ON HIS LEFT SHOULDER CARRYING IT AROUND THE LOADER TO ACCESS SIDE. EM PLOYEE TRIPPED OVER A PARTS BOX ON THE FLOOR, AS HE CAME AROUND THE LOADER. THE CANISTER LANDED ON HIS LEFT HAND.
AT END OF SHIFT, EE WAS WAITING AT SHAFT BOTTOM FOR A RIDE OUT. HE NOTICED A PAIN & DISCOMFORT IN HIS RIGHT FOOT. THE EE CONTINUED TO WOPRK HIS REGULAR SCHEDULE. ON 3-1-02, HE SOUGHT MEDICAL A TTENTION WHICH DETERMINED HE HAD A SMALL BONE FRACTURE BEHIND THE BIG TOE, RIGHT FOOT.
EE WAS WALKING TO POWER BOX. STEPPING OVER GREASE HOSE EXTENDING FROM LUBE TRUCK, LUBE OPERATOR PULLED HOSE, CATCHING REAR FOOT OF VICTIM. CAUSING VICTIM TO TRIP AND FALL. LANDED ON RIGHT SIDE AND BACK.
EMPLOYEE WAS SITTING ON SUPPLY TRACTOR WHILE FILLING TRAILER. HE WAS PAGED ON THE MINE PHONE, AND TRIED TO EXIT THE TRACTOR ON THE RIGHT SIDE, GOING FACE FIRST DOWN THE STEPS. HE LOST HIS FOOT ING AND FELL APPROX 30" TO THE GROUND. HE LANDEDON HIS LEFT FRONT SIDE. HE FRACTURED HIS LEFT SCAPULA.
EE WAS WALKING FROM OFFICE TO LOCKER ROOM, SLIPPED ON ICY PATCH IN DRIVEWAY, FELL DOWN ONTO BACK. DID NOT SEEK MEDICAL ATTENTION UNTIL 12-19-00, 4:00 PM EE TAKEN TO CAYUGA MEDICAL CENTER, EXAM INED AND RELEASED TOLD NOT TO LIFT MORE THAN 10 LBS. UNTIL 12-28-00.
EE WAS CLOSING TOPS OF RR CARS. HE WAS CONNECTED TO THE FALL PROTECTION SYSTEM. AFTER STEPPING ONTO THE CROSSOVER DECK OF THE LAST CAR, HE TURNED TO UNHOOK HIS LANYARD AND STEPPED OFF THE CROS SOVER DECK. LACERATION 7 STITCHES-NO RESTRICTIONS.
EE WAS INSTALLING A ROOF MOUNTED HEAD FRAME STRUCTURE WITH M37 LIFT TRK, THE PLATFORM WAS RAISEDTO AID INSTALLATION. THE PLATFORM WAS RAISED TO HOLD HEADFRAME IN POSITION EE NEEDED TO CLIMB DO WN OFF PLATFORM TO ASSIST IN PUTTING 6' BOLT UP THRU MOUNTING PLATES. WHILE CLIMBING DOWN EE SLIPED & FELL BANGING LEFT KNEE ON STABILIZER JACK.EE RETD TO WORK 1/24/99 TO REGULAR JOB ON 3/11/

EE FELL AND STRUCK RIGHT WRIST.

EMPLOYEE WAS STANDING ON HIS DRILL RIG ATTEMPTING TO REMOVE THE DRILL STEEL WHICH WAS STUCK IN THE FACE. WHEN DISMOUNTING THE RIG, HE STEPPED ON A SALT CHUNK, TWISTING HIS ANKLE AND FELL.
EMPLOYEE WAS COMING DOWN STAIRS THAT LED FROM THE PARKING LOT TO THE LOCKER ROOM AND HE TRIPPED ON A STEP AND FELL.
EMPL WAS STEPPING OUT OF OPERATORS COMPARTMENT OF UNDERCUTTER MACHINE. LEFT FOOT SLIPPED ON SOME LOOSE SALT ON FLOOR & HE FELL.
TWO EMPLOYEES WERE CARRYING A BARRELL OF TURNBUCKLES. THE INJURED PARTY TRIPPED ON A PALLET AND THE BARREL WAS DROPPED, PINCHING HIS HAND AGAINST THE FLOOR.
EMPLOYEE RETURNED TO WORK ON 3/14 AFTER BEING OUT 40 DAYS DUE TO HIS AGGREVATING A DEGENERATED DISC. HE IS OUT AGAIN WITH THE SAME PROBLEM. NO NOTICABLE INCIDENT OCCURRED.
EMPLOYEE WAS MOUNTING SCOOPTRAM, LOST FOOTING, SLIPPED OFF SEAT, BUMPED KNEE ON SHUT-OFF LEVER.
