Employee was walking up steps to get to 5-1/2' cone crusher when EE tripped on the 5th step and fell hurting EE's left leg above EE's ankle. EE was brought to the emergency room by EE's supervisor and put in a walking boot.

Employee was climbing down off of loader when EE slipped and fell off the loader steps and felt pain in lower back, hip and hamstring.

Employee was lifting an air compressor off a pick up truck when employee slipped and fell. Employee's back hit the side of the truck and the compressor landed on employee's right leg.

The employee was carrying a bucket shroud, climbing over shot rock to get to the excavator on its perch. EE lost balance, fell, and thumb was crushed between a rock and the shroud. EE's thumb split open and required eleven stitches.

Employee was descending a wet plant stairway when the employee slipped and lost footing. Employee leaned back and wrenched shoulders trying to prevent self from falling.

Employee was climbing off a Cat D8 that was in the shop for repair. When ee stepped off the machine, ee rolled ankle and had a hard landing on the shop floor. The following day ee went to the doctor's to have it examined. EE has a broken fibula and torn ligaments.

Employee was on a walkway on the crushing plant. EE ducked down to walk under a conveyor, and when ee stood up ee felt a "pop" in knee. It didn't bother ee until ee got home, when ee felt pain when trying to bend leg.

The employee was pulling on a breaker bar (ratchet) which disconnected from the socket causing the employee to slip and twist employee's left knee.

One employee was on a ladder (wearing fall protection gear) when the ladder slipped out from underneath him, due to ice. The injured employee attempting to help, put the ladder back and proceeded to climb the ladder to get the hanging employee back on. The ladder slipped out a second time, and he was injured when he fell.

Employee was working on the jaw's discharge conveyor when another employee energized the conveyor. The conveyor started, the employee who was on the belt fell, rolled around and hit his head before the conveyor was shutdown.

Employee tripped over concrete footings on scale. He twisted and sprained his ankle.

The employee was pulling on a lever to release the fifth wheel and lost his footing. He slipped and bashed his chest on the fender of the tractor, knocking the wind out of himself and tearing cartilage from a rib.

Moving rock in road, slipped and sprained ankle.

Employee was moving 55 gallon oil drum and slipped on ice.

Climbed down from excavator to help on the portable plant. While descending the 3" x 5" pile where he was working, he twisted his knee and fell to the ground.

PLANT OPERATOR WAS CLEANING WALLS OF HOPPER, HE SLIPPED & FELL INTO THE HOPPER.

WAS CLIMBING OUT OF THE TUB FOR THE MASON SAND SCREWS, SLIPPED AND FELL BACK INTO THE TUB INJURING LEFT KNEE.

CLEANING OFF ROOF OVER 4 FT CRUSHER SLIPPED ON MUD AND FELL ON HEAD ABOUT 8 FT TO GROUND.

STANDING ON LADDER PUTTING UP SIGNS ON BINS LADDER BROKE EE FELL DOWN ONTO CONCRETE FOUNDATION.

WHILE STEAM CLEANING LOADER,LOST BALANCE,GRABBED FOR BUCKET LEVELER ARM,IT BROKE CAUSING FALL TO GROUND.

WALKING ACROSS YARD SLIPPED ON ICE

FELL FROM C RANE HIT FENDER WITH CHEST

SLIPPED AND FELL OFF SIDE OF TRUCK TRAILER.

EMPLOYEE ROLLING CANVAS OFF LOAD, LOST FOOTING ON RUNNING BOARD, HELD ONTO CANVAS ROLLER TO PREVENT FALLING AND ELBOW POPPED.

CLIMBING DOWN LADDER ON TRUCK SLIPPED & TRIED TO HOLD SELF UP WITH RT.HAND TORE MUSCLE IN RT.SHOULDER

CLIMBING DOWN LADDER, SLIPPED AND LANDED ON SIDE OF FOOT AS HE TOUCHED GROUND.

MOVING 55 GALLO3 DRUM OF OIL IN SNOW FOOT SLID AND TWISTED UPPER BODY. NO DISCOMFORT UNTIL 2 DAYS LATER.

WALKING TOWARD 7RUCK TO START WORK DAY- SLIPPED ON SNOW AND FELL ON RIGHT HAND RESULTING IN BROKEN RIGHT THUMB.
