EE was transporting portable crusher from Columbus to O'Neill Nebraska. EE was unhooking air lines from back of semi at site when EE lost EE's balance and fell about 3 feet to the ground. EE felt fine after getting up having full range of motion. On 2 hour trip back to Columbus EE started feeling sore.

Mostrando los 27Injury was a first aid from 02/17/2020, it became record-able on 03/06/2020. Employee slipped and fell on the same level in a twin sand dewatering screw. Employee has a non displaced fracture of the lower right arm.
Employee was washing out the log washer and felt a pain in calf causing employee to slip. Hyperextended right knee. Did not report injury until 5/22/17 and did not report it as work related until 5/25/17.
Employee stepped off the scale and slipped on ice. Fell to the ground and twisted left ankle. Resulted in left distal fibula fracture.

Pulling the power cable to the electric dredge when the cable was going over a chain that was hooked to the railing the rope slid off causing the railing to come loose and strike the ee knocking him down. He grazed his head on the edge of the dredge air conditioner. He also injured his left knee.
Slipped while going down wet dredge steps, fell on steps bruising low back and side. 3 days off restricted work activity. Full release on 4/8/11
The EE slipped while climbing down the dozer steps bumping his head & right leg on the dozer ladder, resulting in a bruise, received medical attention as a precaution.
Employee was terminated on Monday June 14, 2010. It was after he was terminated that he told his supervisor that he was hosing down a walkway and fell backwards, straining his lower back.
A customer drove his pickup truck onto scale to get an empty weight. He got out of the cab and walked around the pickup instead of walking to the end of the scale as the signs say to do. He tried stepping over the scale side rail, it was snowing and he slipped and fell on the rail.

THE EE WAS MOVING A FRESH WATER PUMP, ONE SIDE OF THE WALKWAY FOR THE PUMP COLLAPSED DUE TO A BOLT BREAKING, CAUSING THE EE TO FALL CATCHING BOTH KNEES BETWEEN THE RAIL AND WALKWAY. THIS CAUSED HIM TO HYPEREXTENT BOTH KNEES.
THE EMPLOYEE WAS CHECKING MATERIAL IN A PUGMILL BIN, WHILE ON AN EXTENSION LADDER ABOUT 12 FEET HIGH, HE STARTED DOWN THE LADDER, TOOK 2 STEPS & THEN LET GO & FELL ABOUT 9 FEET ONTO CONCRETE. HE THOUGHT HE WAS CLOSER TO THE GROUND.
WHILE DOING PLANT REPAIRS THE EE WAS WALKING AROUND THE BACK OF THE MAINTENANCE TRUCK WHEN HESLIPPED ON WET SLUDGE ON THE CONCRETE PAVEMENT. HE TRIED TO CATCH HIMSELF WITH HIS LEFT LEG & STRAINED HIS LEFT KNEE.
A DUMP TRUCK DRIVER WAS WALKING UP THE STEPS TO THE SCALE HOUSE WHEN HE MISSED A STEP & SLIPPED.IN DOING SO HE TWISTED HIS RIGHT KNEE RESULTING IN A STRAIN REQUIRING MEDICAL ATTENTION.
THE EE WAS WORKING ON THE DREDGE, WHEN HE FELL THROUGH A STEEL GRATE THAT WAS LOOSE, CAUSING HIM TO FALL AND BRUISE HIS RIGHT LEG. HE HAD 3 DAYS OF RESTRICTED WORK ACTIVITY.
THE EE WAS CLIMBING DOWN FROM A TANK THAT HE WAS WORKING ON WHEN HE SLIPPED HE TRIED TO GRAB THE SIDE OF THE TANK, HE FELL ABOUT 6' TO THE FLOOR RESULTING IN CONTUSIONS TO HIS LEFT SIDE. 2 DAY S RESTRICTED WORK ACTIVITY.
