While walking the employee tripped on the catwalk grating section that connects the two different platforms and fell forward, landing a knee on the grating. The employee got up and noticed they had a laceration just above the kneecap.

Mostrando 50 de 62When using a chain binder to chain pipe to floats, the employee lost their balance and fell off the pontoon, landing on the ground. When falling they knocked the cheater pipe off the float they were using to tighten the binder, causing it to fall and strike the employee, causing a laceration to the forehead.
EE was transporting portable crusher from Columbus to O'Neill Nebraska. EE was unhooking air lines from back of semi at site when EE lost EE's balance and fell about 3 feet to the ground. EE felt fine after getting up having full range of motion. On 2 hour trip back to Columbus EE started feeling sore.
Injury was a first aid from 02/17/2020, it became record-able on 03/06/2020. Employee slipped and fell on the same level in a twin sand dewatering screw. Employee has a non displaced fracture of the lower right arm.
Employee was washing out the log washer and felt a pain in calf causing employee to slip. Hyperextended right knee. Did not report injury until 5/22/17 and did not report it as work related until 5/25/17.
Employee stepped off the scale and slipped on ice. Fell to the ground and twisted left ankle. Resulted in left distal fibula fracture.

Employee was walking up a hill with a tool box. The tool box broke, causing the employee to fall to the ground.
Pulling the power cable to the electric dredge when the cable was going over a chain that was hooked to the railing the rope slid off causing the railing to come loose and strike the ee knocking him down. He grazed his head on the edge of the dredge air conditioner. He also injured his left knee.
Slipped while going down wet dredge steps, fell on steps bruising low back and side. 3 days off restricted work activity. Full release on 4/8/11
The EE slipped while climbing down the dozer steps bumping his head & right leg on the dozer ladder, resulting in a bruise, received medical attention as a precaution.
Employee was terminated on Monday June 14, 2010. It was after he was terminated that he told his supervisor that he was hosing down a walkway and fell backwards, straining his lower back.
A customer drove his pickup truck onto scale to get an empty weight. He got out of the cab and walked around the pickup instead of walking to the end of the scale as the signs say to do. He tried stepping over the scale side rail, it was snowing and he slipped and fell on the rail.

THE EMPLOYEE WAS CHECKING MATERIAL IN A PUGMILL BIN, WHILE ON AN EXTENSION LADDER ABOUT 12 FEET HIGH, HE STARTED DOWN THE LADDER, TOOK 2 STEPS & THEN LET GO & FELL ABOUT 9 FEET ONTO CONCRETE. HE THOUGHT HE WAS CLOSER TO THE GROUND.
WHILE DOING PLANT REPAIRS THE EE WAS WALKING AROUND THE BACK OF THE MAINTENANCE TRUCK WHEN HESLIPPED ON WET SLUDGE ON THE CONCRETE PAVEMENT. HE TRIED TO CATCH HIMSELF WITH HIS LEFT LEG & STRAINED HIS LEFT KNEE.
A DUMP TRUCK DRIVER WAS WALKING UP THE STEPS TO THE SCALE HOUSE WHEN HE MISSED A STEP & SLIPPED.IN DOING SO HE TWISTED HIS RIGHT KNEE RESULTING IN A STRAIN REQUIRING MEDICAL ATTENTION.
THE EE WAS WORKING ON THE DREDGE, WHEN HE FELL THROUGH A STEEL GRATE THAT WAS LOOSE, CAUSING HIM TO FALL AND BRUISE HIS RIGHT LEG. HE HAD 3 DAYS OF RESTRICTED WORK ACTIVITY.
THE EE WAS CLIMBING DOWN FROM A TANK THAT HE WAS WORKING ON WHEN HE SLIPPED HE TRIED TO GRAB THE SIDE OF THE TANK, HE FELL ABOUT 6' TO THE FLOOR RESULTING IN CONTUSIONS TO HIS LEFT SIDE. 2 DAY S RESTRICTED WORK ACTIVITY.
THE EE WAS CLIMBING UP INTO THE CAB OF THE FRONT-END LOADER, WHEN HE SLIPPED AND TWISTED HIS LEFT ARM ON THE LADDER. DID NOT INFORM ANYONE AND RECEIVED MEDICAL TREATMENT. STRAINED MUSCLES IN H IS LEFT ARM.
EE WAS IN PROCESS OF WASHING OUT THE PLANT, SO NO MATERIAL WOULD FREEZE IN THE EQUIPMENT OVERNIGHT. THE PLANT WAS SHUT DOWN FOR THIS SHIFT UNTIL A PUMP BEARING COULD BE REPLACED. HE DROVE THE FRONT END LOADER OVER & RAISED THE BUCKET ABOUT 6' OFF THE GROUND. HE TURNED IT OFF. HE WAS CLIMBING UP INTO THE LOADER BUCKET WHEN SLIPPED & FELL TO THE GROUND FROM 6' UP, BREAKING HIS RT ARM
THE EE WAS INSTALLING SOME DREDGE PIPELINE WHILEIT WAS RAINING THIS CAUSED THE FLAT METEL SURFACE ON THE WORK BOAT TO BECOME SLIPPERY THE EE SLIPPED ON THE WET SURFACE & FELL ON THE DREDGE PIP ELINE RESULTING IN 1 FRACTURED RIB ON THE LEFT SIDE OF ABDOMEN
THE EE WAS WALKING BACK TO THE DREDGE OPERATOR'S CABIN, WHEN HE SLIPPEED ON SOME WET STEPS AND FELL FORWARD HITTING CHIN ON ANOTHER METAL STEP. THIS CAUSED A LACERATION JUST UNDER HIS CHIN THA T REQUIRED STITCHES.
THE EE WAS REMOVING A PIECE OF EXPANDED METAL FROM THE DREDGE WITH ANOTHER EE WHEN HE STUMBLED AND LOST HIS BALANCE AS HE FELL HE GRABBED THE EXPANDED METAL TO BREAK HIS FALL. WHEN DOING SO HI S LEFT MIDDLE FINGER WAS CAUGHT IN THE EXPANDED METAL CAUSING A DEEP LACERATION AND CUTTING A TENDON. THE EE RECEIVED MEDICAL ATTENTION TO REPAIR THE TENDON AND SUTURES. RETURNED NEXT SHIFT.
THE EMPLOYEE WAS CLIMBING DOWN THE LADDER ON THEFRONT END LOADER WHEN HIS FOOT SLIPPED. HE HUNG ONTO THE LADDER WITH HIS LEFT ARM SO AS NOT TO FALL TO THE GROUND.L THE WEIGHT OF HIS BODY PULLE D ON HIS LEFT ARM STAINING RIB MUSCLES.
THE EMPLOYEE WAS STANDING UP PUNCHING OUT TICKETS FOR THE TRUCK DRIVERS. WHEN HE WENT TO SIT DOWN IN THE CHAIR, HE CAUGHT THE EDGE OF THE CHAIR CAUSING THE CHAIR TO ROLL BACK. AS HE FELL HIS R IGHT ARM STRUCK THE DESK TWISTING HIS BACK.
THE EE WAS ASSISTING TWO OTHER EE S IN SETTING CROSS BRACES TO BE WELDED FOR SAND CHUTES, WHEN A SUPPORT WHICH THE LADDER WAS LEANING AGAISNT TOPPLED CAUSING THE LADDER AND EE TO FALL FROM HEI GHT OF ABOUT 12 FEET. THE EE S RIGHT HEEL LANDEDON THE LADDER WHEN HE FELL.
THE EE WAS PERFORMING REPAIRS ON A CONVEYOR, WHEN A THUNDERSTORM CAME UP. DURING THE HEAVY RAINS AND HAIL. THE EE WAS MOVING TO COVER, WHEN HE STEPPED OVER A 15" WHITE PIPE AND SLIPPED AND FEL L. U
THE EE WAS WORKING ON THE SCALE DECK, THEN WHILE STEPPING DOWN FROM A STEEL BEAM, HIS FOOT SLIPPED, CAUSING HIM TO FALL BACKWARDS FROM ABOUT 4 FEET ONTO A CONCRETE WALL.
THE EE WAS PULLING ON A WATER HOSE, WHILE WASHING OFF THE CONRETE AREA UNDER THE PLANT, WHEN THE HOSE CAME LOOSE AND THE EE FELLO OVER A PIPE, LANDING ON ANOTHER PIPE WITH HIS RIGHT SHOULDER.
THE EMPLOYEE WAS HELPING CHANGE A BLOW OUT PIPE ON THE DREDGE PIPE LIEN AND WAS TRYING TO GET THE POWER CABLE OUT OF THE WATER FROM THE WORK BOAT. HE LOST HIS BALANCE AND FELL ON THE 12" PIPE , HITTING HIS CHEST AND FALLING INTO THE LAKE.
EMP WAS WALKING UP SNOW COVERED STEPS TO ELECTRICAL TRAILER CARRYING A 40 LB BATTERY CHARGER WHEN HE SLIPPED ON SNOW COVERED STEPS AND FROM ABOUT 3' LANDED HARD ON BOTTOM OF LEFT FOOT ON GROUN D.
EE WAS PUTTING BLOCKING UNDER A GRAVEL WASHER TO UNLOAD IT, WHEN DOING SO, THE EE SLIPPED ON TEH ICE AND LANDED ON HIS BACK, CAUSING SOME PAIN & DISCOMFORT IN HIS BACK. WHEN THIS HAPPENED THE EE DECLINED ANY MEDICAL ATTENTION UNTIL THE WEEK OF 4-7-. HE HAD GONE TO THE DR. PRIOR TO THIS INJURY BECAUSED OF SOME BACK PROBLEMS.
THE EMPLOYEE WAS WALKING OVER THE SUCTION FRAME TO DO SOME REPAIRS. THE EMPLOYEE'S FEET WERE WET AND SNADY CAUSING HIM TO SLIP AND FALL. HE RECEIVED SCRAPES & BRUISES TO HIS RIGHT ARM AND SH OULDER THAT STRUCK AN ANKLE IRON BRACE.
THE EE WAS WALKING DOWN THE STEPS FROM THE DISPATCH OFFICE TO HIS TRUCK, WHEN THE EE S RIGHT FOOT CAUGHT THE WROUGHT IRON RAILING, CAUISNG HIM TO LOSE HIS BALANCE AND FALL ON HIS LEFT LEG AND LEFT KNEE.
THE EE SLIPPED ON FUEL OIL ON THE STEPS TO THE DREDGE ENGINE AREA AND TWISTED HIS RIGHT KNEE. MEDICAL EVALUATION REVEALED A STRAINED RIGHT KNEE.

THE EMPLOYEE WAS TREATED AT A HOSPITAL WHICH RESULTED IN LOST TIME.

THE EE WAS SECURING THE DREDGE ANCHOR ON THE STRIPPING BANK WHICH WAS MUDDY. THE EE TWISTED HIS RIGHT KNEE MEDICAL ATTENTION REVEALED A STRAINED RIGHT KNEE. THE EE HAD 6 DAYS OF RESTRICTED WOR K ACTIVITY BEFORE RELEASED TO NORMAL DUTIES.
