Loading a shot, went to step back to the next hole, lost footing, fell to the ground, rolling right ankle
Miner was hurrying across work area with items in both hands. EE had to duck under a conveyor. When EE did so, EE's feet got tangled under EE and EE stumbled without falling.
Employee was exiting service truck at the shop. Employee's right foot slipped from the bottom step of the vehicle. Employee fell on right side injuring hip, ribs and elbow. No witnesses to the event. Employee diagnosed with fractured 8th right rib and contusion to the right hip.
WAS RUNNING LOADER. WHEN FINISHED PARKED MACHINE. EXITED MACHINE AND PROCEEDED DOWN STEPS/LADDER USING THREE POINTS OF CONTACT. ONCE ON THE GROUND, TURNED TO WALK TOWARDS THE PLANT - SLIPPED ON ICE AND FELL TO THE GROUND. LANDED ON BUTT / LEFT SIDE.
On 4/9/18 a third party trucking company driver was exiting cab at the Sweet S&G facility, when the driver's foot slipped on some built up ice on the truck step, causing driver to fall to the ground and injure ribs. It was later determined that the driver had broke a couple ribs from the fall.
Over the road trucking contract driver had been successfully loaded. Made his way to designated tarping area. While engaging manual tarping mechanism the handle malfunctioned causing truck driver to lose balance from bottom rung of ladder. Driver slipped and struck parts of his truck. Suffered lacerations to "rear end and hand". lacerations required sutures(stitches).
Employee was descending the fixed ladder of a Cat 980C end loader. His foot slipped as he was descending and the employee identified he felt a strain to the right knee.
Employee was working on rolling up a belt on ground level. While attempting to track the belt, the hock rod used for assistance in tracking slipped, causing the employee to fall backwards to the same level (ground), injuring left ankle & knee. No initial medical attention given until 11/20/2008, where restrictions were given, identifying a MSHA reportable claim.
Performing maintenance of bushing repair to the bucket of a backhoe, the employee had stepped backward to walk away from the work area. While stepping back and turning, the EE suffered a right knee strain to his foot; his foot had slid between the two teeth of the backhoe. The backhoe was locked/tagged out with the bucket secured at ground level.
Employee identified that stepping from engine level of haul truck, on identified walk area to truck cab, he slipped on step and fell, striking his right knee on the truck. He admitted to not maintaining three point of contact, as he had his coffee cup in hand. Initial medical attention on 5/19 (not reportable from treatment of first aid)
While climbing down from the Komatsu Endloader (company number 9135), the employee fell hitting his ribs on a milk crate used for a step / platform. Fracture to the 6th rib.
WALKING TO START LOADER BEFORE ACTUAL WORKSHIFT BEGAN, HE SLIPPED ON ICE AND TWISTED KNEE. LIGHT IN AREA WAS NOT WORKING (MOTION SENSOR OUT). EE REPORTED INJURY ON 12/8/03. MEDICAL OPINION DET ERMINED THIS A REPORTABLE CLAIM EFFECTIVE 1/5/04. EE CONTINUES TO WORK FULL TIME IN REGULAR JOB (LOADING TRUCKS) UNTIL SURGERY. UPDATED REPORT AS NECESSARY.
PUTTING 55 GALLON DRUM INTO OIL TRAILER, HAND SLIPPED & EMPLOYEE FELL ONTO 5 GALLON BUCKET, CAUSING RIB INJURY.
EE WAS STEPPING DOWN FROM THE CAB OF A WATER TRUCK. HE USED THE HANDRAIL BUT LOST HIS FOOTING, AS HE FELL HIS HAND BECAME WEDGED IN THE HANDRAIL. AS HE CONTINUED TO FALL HIS HAND CAME FREE AND STRUCK THE FUEL TANK.
EMPLOYEE WAS DISMOUNTING A BOB CAT-SKID STEER LOADER, WHEN HIS LT. FOOT SLIPPED ON A WET SURFACE W/SAND. THE EMPLOYEE FELL AND BROKE HIS RT.FOOT
EE WAS LIFTING A DRAIN OIL-PAN, LOST HIS BALANCE & SLIPPED RESULTING IN LOWER BACK PAIN.
EE WAS DESENDING A VERTICAL LADDER ON A FRONT END LOADER, W/A FUEL HOSE AND NOZZEL IN HIS LT. HAND EE FELT A PAIN IN HIS RT SHOULDER, DROPPED THE HOSE, LOST HIS GRIP AND FELL AGAINST THE TOP R UNG OF LADDER.
