EE working on installing forms. EE went to sit down on form and form broke. Tried to catch self but cut hand on anchor while doing so. EE received stitches.

EE was going backwards down the staircase EE missed two steps and hit EE's left knee on the staircase. On the next trip down EE noticed another EE coming down the stairs and IE tried to backup to provide room for EE to pass and again IE missed two steps and hit EE's right knee.
Employee was walking and fell on a flight being stored cutting side of face and hand.

EXITED PLATFORM FROM ROLLER AREA INSTEAD OF USING STAIRS. TRIPPED ON IRREGULARITY ON FLOOR SURFACE & HURT ANKLE.
PULLING OUT A PALLET WITH A HOOK, HOOK CAME LOOSE AND EMPLOYEE FELL ONTO SHOULDER.
OPERATOR SLIPPED WHILE ATTEMPTING TO JUMP OFF OF A PALLET, RESULTING IN OPERATOR'S FEET BURYING IN HOT CLAY. SOME CLAY, ENTERED THE BOOT AND BURNT THE TOP OF HIS FEET.
OPERATOR SPRAINED HIS WRIST WHEN HE SLIPPED, STRIKING HIS HAND AGAINST A ROLL IN THE PRIMARY CRUSHER.
OPERATOR BECAME OVER BALANCED WHILE TRYING TO DESCEND A STAIRWAY WITH A BOXIN HIS ARMS. OPERATOR FELL STRIKING HIS KNEE AGAINST THE PLATFORM.
