Employee slipped while walking down steps. Employee fractured right ankle.

Walked on the Scale to talk to operator and fell walking off of the scale.

EE was walking and tripped, EE fell on EE's face and broke EE's nose and rt. cheek bone.

Employee tripped on steel stacked in shop area. NOTE: Employee did not seek treatment (to which restrictions were placed) until 05/11/2016. This report was filed within the 10 working day requirement.

Employee was climbing up into an articulated haul truck. The employee slipped and started to fall. EE's L. Leg caught between the rungs of the access ladder and the employee fell and EE's left leg stayed in between the rungs of the access ladder.

Contract Welder ee was walking in the area adjacent to the primary jaw crusher and tripped on concrete from "clean out" of previous concrete pour.

While climbing down the ladder of a haul truck,employee's foot got stuck between ladder rungs causing the employee to fall. As a result, employee sprained/strained her left knee.

Employee was cleaning crusher when his foot got caught between the motor and guard.

Employee is an over the road delivery driver and slipped while walking across scale to get into his truck. The left knee had torn ligaments and required surgery.

Employee was exiting a articulated Volvo haul truck and mis-stepped on the ladder. The employee suffered contusions to the L. elbow & shoulder. A potential fracture to the L. elbow is possible, pending further examinations.

Employee was exiting a front end loader and slipped on the last step next to ground level.

The injured employee was standing on a pile of R-6 rocks at the North hole sump. The rocks slid and the employee fell down on the pile of rocks and into the water.

While performing mechanical work on a John Deere 800 hoe, the employee fell landing on his tail bone, resulting in a sprain.

Going up step ladder with cutting torch. Was on the 6th step of a 12 ft step ladder when ladder twisted & went to the side and fell down to the ground. I fell down and hit the concrete floor and hit on my right shoulder & bruised the muscle in my right arm. Had checked ladder stability each time used.

Lost footing and fell face first into a backhoe bucket.

Employee slipped on fuel tank and fell to ground. The tank was wet, with fuel residue and the tank was round.

Employee fell thru a elevated walkway when the steel grating approx. 22" wide and 36" long he stepped on dislodged. the walkway was supported on both side with 3"x3" angle iron and was adjacent to a crusher. It appears the weld broke as a result of vibration.

Injured employee was standing on top of a tail pulley guard servicing an adjacent conveyor when he slipped. He attempted to catch himself with his right hand, when his hand wedged between a guard and a speed reducer belt/pulley.

He claims he jumped off #2 stacker & twisted his back- No witness- He admits he hurt his back after work while working on a friend's auto- Did not report "off" for work until we called him & ordered him to go to listed medical provider as state law requires for all w/c claims.

DISMOUNTING FROM TRUCK CAB & FOOT SLIPPED ON BOTTOM STEP CAUSING HIM TO TWIST HIS BODY.

EMPLOYEE WAS WALKING DOWN FLIGHT OF STAIRS AND LOOK TO THE SIDE AFTER HEARING NOISE FROM PLANT.EMPLOYEE STUMBLED AND FELL DOWN APPROX 7 STEPS.

WHILE WALKING ON EMBANKMENT, MATERIAL GAVE WAY & HE TWISTED HIS KNEE WHILE REGAINING HIS BALANCE.

LOST HAND GRIP ON HANDHOLD BAR WHILE ENTERING WATER TRUCK AND FELL.

WHILE SHOVELING UNDER CONVEYOR, HE STEPPED BACKWARD ONTO UNEVEN SURFACE & TWISTED KNEE.

CLIMBING OUT OF HOPPER BIN AND TWISTED KNEE.

WAS WALKING ON STOPPED CONVEYER BELT AND SLIPPED ON SNOW ON BELT- HIT SHIN ON BELT CAUSING ABRASION, FAILED TO REPORT IT AND GET MEDICAL TREATMENT UNTIL INFECTION BEGAN, WAS SENT TO MEDICAL PR OVIDER TODAY.

WAS WALKING ACROSS FLAT GROUND AREA; STEPPED ON SIDE OF FOOT, TWISTING ANKLE.

EE PULLED WATER TRUCK UNDER FILL PIPE. HE DID NOT APPLY PARK BRAKE OR HAVE TRUCK IN GEAR. THE TRUCK STARTED TO DRIFE AND EE RAN AFTER TRUCK, SLIPPED AND FELL, INJURING HIS RIGHT SHOULDER AND A RM.
