While performing morning routine housekeeping, employee went to exit the catwalk going down steps, EE slips on the first step. Employee was holding onto the handrail and hit knee on the steps.

While climbing a ground ladder, employee lost balance fell to the ground injuring right foot(heel)

An employee fell approximately 8'6" off of a concrete landing, striking the ground. The employee was transported by EMS to the hospital. EE injuries include a left wrist fracture and a fractured pelvis. Notification was made to the call center when the accident occurred.

A cow was loose in the pit and several mine employees were trying to herd the animal out of the road. EE aproached the cow to move it and the cow ran at him. When EE ran out of the way he twisted his ankle.

ON LADDER TO ADJUST CONTROLS ON DRILL LADDER SLIPPED EMPLOYEE FELL ON ROCKS.

EMPLOYEE WAS CLIMBING INSIDE BED OF TRUCK WHEN HE TURNED HIS ANKLE

EMPLOYEE WAS GETTING INTO HIS TRUCK WHEN HIS FOOT SLIPPED CAUSING HIM TO FALL ON HIS LEFT ARM.

I NAME & ANOTHER EMPLOYEE WERE HORSEPLAYING ON THE JOB WHEN I NAME FELL ON HIS LEFT LEG & FRACTURED BONE BOTH EMPLOYEES HAVE BEEN DISCHARGED FROMTHEIR JOBS

MEPL FOOT SLIPPED OFF TRUCK AND AS HE TRIED TO CATCH HIMSELF HE HIT HIS ELBOW

EMPLOYEE WAS ST5NDING ON HOOD OF TRUCK WATCHING A LOAD OF ROCK BEING LOADED WHEN A ROCK BOUNDED TOWARD HIM, HE JUMPED BACK AND GOT OFF BALANCE AND FELL TO THE GROUND. FALLING ABOUT 8 FEET.

EMPLOYEE WAS ST1EPPING UP ON THE STEP OF THE LOADER WHEN HIS FOOT SLIPPED CAUSING HIM TO FALL ON THE STEP & BREAKING HIS RIBS

EMP STUMBLED AN8 FELL HITTING KNEE ON CATWALK IN PUGMILL TUNNELL

EE WAS GETTING OUT OF WATER TRUCK WHEN HE LOST HIS BALANCE & FELL INJURING HIS BACK.
