EE stepped onto a concrete support approximately 6 inches tall, EE's foot shifted, and EE stumbled. Causing EE to fall. EE chose to not go to the Dr that day then the next day worked full day and felt should get it checked. Dr took x-ray and it came back negative. EE was seen on 10/24 and diagnosed with a triangle crack.

He heard a pop, was unable to bear weight and limped to vehicle. He was sent to urgent care center, then to ortho. Currently he is unable to return to work until MRI is performed and read.
EE was tightening roller bolts on under carriage of a d9r dozer. After completing the project he stood and starting walking to his truck when he felt a catch in his left knee. He went to powell clinic and knee was x rayed and he was given an anti-inflammatory medication and placed on light duty until 4/30/08 an at that time he will be re-evaluated.
Two pieces of flooring collapsed at an elevated catwalk at the primary end of plant. The welds that secured the flooring deteriorated and loosened over time. The floor failed to support an employee that was walking the catwalk.
EE was standing on the ground watching loader load a truck. Then turned to walk around and tripped on small stone on the ground and lost balance falling into a truck then to the ground. Broke small bone in lower right leg.
JUMPED OUT OF STOCKPILE TRUCK LANDED ON A CLOD OF DIRT AND TWISTED ANKLE.
EE WAS STANDING ON THE BUMPER OF AN EUCLID REAR DUMP PREPARATORY TO ADDING ANTIFREEZE TO RADIATOR. RIGHT FOOT SLIPPED OFF BUMPER AND EE FELL BACKWARD FROM BUMPER ONTO GROUND STRIKING HIS LEFT SHOULDER, HIP AND SIDE.
