Closing the lid of the crusher, EE was moving a rock that was in the way with a metal rod. When the rock moved, EE's weight shifted as well, and EE fell into the lid retainer wedge holder with EE's upper right chest.

Miner states EE was jogging from the VSI to the control house stairs when EE twisted EE's ankle. Injury sustained was diagnosed as an ankle sprain.

Offloading a 40' auger from a flatbed, EE placed chocks against the auger and removed ties securing it in place, then moved to secure it to the forklift with a come-along winch. When the auger started to roll the EE jumped clear of the auger and upon landing on the ground EE's right knee was twisted and EE's left wrist was broken.

Employee was pressure washing a 631-G Scraper, employee stepped off the fender and slipped on a spot of grease twisting employee's left ankle.

Operator was descending the steps on the front of a 775 haul truck. When EE placed right foot on the ground, it slid back on some loose material. EE fell forward onto the steps/bumper of the truck hitting left elbow/forearm and chin.

Customer truck driver hauling material for CKJ Trucking LP. Climbed into bed of truck on tar plane of the haul road as he was climbing down from the top of the truck he slipped and fell approx. 5 feet to the pavement below, landing on his right side.

REPORTED ON 3/4/10 - tapping holes, walking from hole to hole on the shot, hurt right foot.

EE slipped and fell going down stairs to crusher control house, severely spraining right ankle.

While desending from a 775 quarry truck, the employee blacked out and fell to the ground landing on his left side injuring his eye, ribs and knee.

While repairing troughing idler pulleys on the over land conveyor, the employee twisted his right knee when he climbed down off of the conveyor belt.

WHILE ATTEMPTING TO DISMOUNT HIS LOADER, THE OPERATOR SLIPPED ON THE LAST STEP OF THE LADDER AND FELL BACKWARDS LANDING ON HIS BACK AND RIGHT WRIST.

EE WAS CONDUCTING A PRESHIFT INSPECTION OF THE SECONDARY PLANT. WHILE CLIMBING UP A FIXED LADDER THE BOTTOM PORTION CAME LOOSE CAUSING THE EE TO FALL ABOUT 3 FT TWISTING HIS KNEE.

EE STATED THAT HE ENTERED THE OFFICE BLDG AND A FEW MINUTES LATER AS HE WAS LEAVING THE BLDG SLIPPED AND FELL ON THE LANDING AREA. THE AREA CONSISTS OF A LEVEL CONCRETE FLOOR COVERED BY A ROOF . THE EE WORKED UNTIL 1-28-04 AND LEFT ON FAMILYMEDICAL LEAVE FOR AN UNRELATED ILLNESS RX ONLY.

EMPLOYEE WAS REPAIRING OVERHEAD AIR CONDITIONER INSIDE NON-OPRATING ROTARY DIRLL (WITH DOOR ON DRILL OPEN) AND STANDING ON WATER CAN. THE CAN SHIFTED AND THE EMPLOYEE FELL THROUGH OPEN DOOR, STRIKING RIGHT UPPER ARM ON DOOR LATCH.

EMPLOYEE WAS WORKING IN ELEVATED AREA INSTRUCTING A WELDER WHEN FLOOR GRATING COLLAPSED AND EMPLOYEE FELL TO GROUND BELOW.

EE ATTEMPTED TO WALK ACROSS FRONT CATWALK, LOCATED ABOVE ENGINE ON 775 HAUL TRUCK W/O HOLDING ON, LOST HIS BALANCE AND FELL TO GROUND BELOW, LANDING ON HIS FEET.

EMPLOYEE WAS DESCENDING LADDER AND MISSED A RUNG NEAR THE BOTTOM CAUSING A JARRING SENSATION & BACK STRAIN WHEN HE STEPPED ON GROUND.

NIGHT OPERATION- SEMI-DARKNESS - INJ HAD JUST CLEARED CONVEYOR BELT OF OBSTRUCTIONS,DESCENDED LADDERA AND STEPPED OFF LADDER ONTO CONCRETE SLAB WHILE TURNING.RIGHT FOOT WAS PARTIALLY OFF SLAB WHEN WEIGHT WAS APPLIED.TURNED ANKLE CAUSED FRACTURE.

INJURED WORKED AT ROCK CRUSHER UNIT. A BOULDER BECAME LODGED IN JAWS OF CRUSHER AND STOPPED THE PLANT. INJURED ENTERED THE HOPPER ACCESS TO THE JAWS WHERE HE WAS TO PLACE A CHARGE TO DISLODGE THE BOULDER. INJURED LOST FOOTING ON SLANTED SIDES OF HOPPER SLID INTO BOULDER.

INJURED WAS BREAKING LOSOE A PIECE OF DRIED MUD WITH A SHOVEL USING HIS RIGHT FOOT. THE MUD UNEXPECTEDLY BROKE LOOSE, HE SLIPPED AND SPRAINED HIS LEFT KNEE. INJURED REQUESTED MEDICAL ATTENTI ON ON 5-22-90.

WHILE GETTING O0F OF THE BED OF A FLAT BED TRUCK, HE SLIPPED AND FELL OFF OF BED SUCH THAT HE LOST HIS FOOTING HIS LOWER LEG HIT THE SHAPR BUMPER EDGE CAUSING A SCRAPE AND BRUISE.
