Mostrando los 13Employee was preforming maintenance tasks on the cone while plant was shut down when ee lost footing on the cone discharge. EE fell from approximately 4 feet high onto shoulder. EE has had shoulder problems in the past and it does become easily dislocated. EE shoulder was in fact dislocated and ee was taken to Medical Center for treatment.

ON A LADDER INSTALLING BOLTS ON A CATWALK. CATWALK WAS BEING FABRICATED FOR A NEW CRUSHER LADDER WAS NOT STABLE, AND EE FELL, INJURING SIDE.
