Mostrando los 16EE jumped down from a storage trailer that was approximately 4 feet in height. When EE landed, left knee gave out. After visiting the doctor following the incident, EE was returned to work with restrictions that still allowed EE to perform EE's job duties. On August 11th, EE had an MRI conducted and it was discovered that EE had a torn ACL and meniscus in left knee.

WHEN STEPPING DOWN FROM FAN LEVEL TO BAGHOUSE DAMPER LEVEL HEEL OF EE'S BOOT CAUGHT CAUSING EE TO FALL FORWARD ONTO PISTON HOUSING FRAME. EE LANDED ON FLANK (LEFT RIBS).
EE WAS CARRYING CHAIN INTO KILN - TRIPPED FALLING ON CHAINS.
EE WAS TIGHTENING HOSE FITTING WITH WRENCH. TOOLSLIPPED OFF FITTING CAUSING EE TO FALL TO GROUNDLANDING ON ROCKY AREA.
LADDER SLID-EE SUPPORTED SELF BY ARM AND HAND WHILE STRAIGHTENING LADDER.
EMPLOYEE SLIPPED ON LOOSE MATERIAL AS HE ATTEMPTED TO CATCH HIMSELF FROM FALLING, HE STRAINE DHIS SHOULDER.
