While on ground level, the EE reached into an excavator for a hoodie, lost footing, and grabbed the handrail, injuring their left elbow. Sought treatment the same day and returned to work with restrictions that did not affect regular job duties. An 08/15/25 MRI showed a left triceps rupture requiring surgery. No equipment, compliance, training, or PPE issues were involved.

WHILE REPLACING BOLTS ON VIBRATORY UNIT UNDER SCALPER. FELL APPROX 5' TO THE GROUND FROM TWO 2 X 8 BOARDS THAT WERE BEING USED AS SCAFFOLDING, STRIKING THE ANGLE IRON SUPPORTS ACROSS THE BACK OF SCALPER.
TWISTED LEFT KNEE WHILE WALKING ON ROCK AND ICE MOVING THE WATER PUMP.
INJURED WAS WORKING ON MAINTENANCE TO THE PRIMARY CRUSHER. HE MOVED TO THE OTHER SIDE, NOT NORMALLY WHERE WE WORK FROM, TO GAIN BETTER ACCESS TO THE BEARING HE WAS REMOIVNG. GOOD ACCESS WAS NOT PROVIDED AS WE DON'T NEED TO GO TO THE OTHER SIDE. HE WAS STEPPING OVER THE MOTOR WHEN HE SLIPPED AND FELL 2 FT AND HIT I-BEAM WITH KNEE.
I NAME WAS STARTING TO CLIMB UP THE LADDER INTO TRUCK WHEN HE WENT TO PULL HIMSELF UP OFF THE GROUND ONE FOOT ON LADDER HIS FOOT SLIPPED AND HE FELL JERKING HIS BACK
