An employee fell while walking. While attempting to catch themselves, they sustained lacerations to their left hand and chin.

Employee arrived to work for the start of day shift. On their way into the truck shop from their personal vehicle, the employee slipped and fell on ice in the parking lot and broke their hip. Employee was transported to the hospital for medical treatment.

An employee was ascending a stairwell prior to the start of their shift when the employee experienced a personal medical issue. The employee subsequently tripped and experienced a stair fall impact sustaining injuries to their left upper extremities.

Operator was cleaning tripper for maintenance. Operator was traveling on tripper walkway when the operator slipped and fell on water/ice formation. Operator sustained dislocation of their right fifth finger.

Loader operator tripped and fell while entering the cab of their loader, straining their neck and back.

Employee was deboarding water truck 5168 from the operator's cab when their foot slipped off the top step causing pain to their right hip and knee. Employee continued to work full duty until imaging conducted on 11/19/2024 revealed injury to their right knee. Employee was prescribed restrictions.

A maintenance mechanic was deboarding a CAT 24M motor grader (GR22) when they fell from the operator's cab platform to the tandem platform landing on their right knee. The employee was evaluated by a site EMT and transported to the local area hospital for further evaluation. The employee sustained right hip sprain and right knee contusion.

An employee was transferring from a fixed ladder to 28B conveyor platform when the employee slipped, contacting their right shin on the platform grating causing a laceration to the shin. The employee was evaluated at the ER and received 3 stitches for the laceration.

Pit electrician was picking up a 13.8kV cable head when force released from the cable and pushed EE to the ground, injuring EE's shoulder. EE is currently in PT.

An employee slipped and fell on ice that accumulated in the lower tunnel entrance to the plant building while arriving to work. The employee experienced left knee pain and was sent to the hospital for further evaluation.

Employee was walking in the contractor parking lot from their car to the company van. As employee was walking, employee slipped and fell onto knee.

Worker fell from scaffolding ladder and sustained an injury to EE's left leg

While employee was delivering parts to the mine warehouse, the employee slipped on the snow when returning to EE's vehicle and fell on EE's right side.

While walking on a grate to return a torch EE was using, EE's feet slipped out from under and EE fell and landed on their buttocks.

Descending stairs to retrieve tools. Both hands were on the handrails and EE slipped, caught self but EE's left hand got caught on the handrail upright. EE's left ring finger was fractured right at the top of the hand.

Employee was exiting the cab of a Skid-steer loader, facing the loader, when left foot caught on a cutting edge bolt of the loader bucket. This caused the employee to trip, twisting left ankle and falling to same level.

Prior to shift, EE entered the walkway/tunnel that spans from the parking lot to Tilden plant. EE was descending stairway, holding on to hand-rail with left arm, carrying lunch bucket in right arm, left foot slipped on 3rd or 4th stair from bottom causing left leg to move forward resulting in fall onto stairway steps. Evaluated by onsite EMS then by UPHS Bell ER.

Operator was performing pre-use inspection walk around 992 loader at the Rail Tiedown. Slipped on slick area on south side of the bucket and fell on shoulder causing discomfort. Refused to see an EMT and returned to work. Later complained of discomfort and taken to ER for evaluation. Taken off work.

A quality tech was walking outside the west end of the de-slime basement when slipped and fell to ground, striking left elbow on ground. Employee was seen by on-site EMT's, taken to UP Health System Bell for evaluation. X-rays taken, returned to work to follow-up with nurse practitioner. Had follow up with specialist and given restrictions.

EE's boot caught on L63's boarding step while boarding the loader causing EE to lose balance. EE felt discomfort in right forearm that was holding the handrail. Evaluated in the ER and taken off work.

Employee slipped and fell in the night shift parking lot when reporting for night shift. Employee didn't want to be seen by an EMT, just wanted it reported. Later the employee was brought to the hospital for evaluation. Employee was removed from work.

Employee reported walking on Drill Pattern 6168 after snow cleaning was performed by a loader. The employee stated that EE lost footing and fell landing onto EE's right side. Employee was evaluated by onsite medical responder, and then later brought to hospital for further evaluation. Employee was removed from work.

Loader operator reported slipped on stairs, struck knee on grating while going to make oil checks on L48. Operator seen by on-site EMT, and then brought to hospital for further evaluation.

EE was removing item from shelf when EE tripped and fell backwards onto the floor. EE hit head causing a small laceration. Evaluated in ED, mild concussion.

EE was pulling a hose near header pipe when EE lost balance and fell injuring left hand. Evaluated at ER and diagnosed with a hand fracture.

Snow covered ice in parking lot. Employee slipped while exiting vehicle and hyper-extended right shoulder when ee grabbed the vehicle door and caught self during fall. Employee finished out shift and returned to work for next shift the following day. No ER treatment

Employee finished shift and was walking to vehicle to leave. Slipped in parking lot and fell. Complained of pain to knee. Evaluated by MD and taken off work.

EE states EE was changing out an LP cylinder on a portable welder at the entrance of 16c feeder(outdoors on road base gravel). The EE states EE unexpectedly stepped on some uneven ground resulting in pain to left lower back and leg. EE has on-going, non-work related back issues EE is treated for. Aggravation of a non-work related illness resulting in restricted duty work.

Employee was stepping out of a bus, lost EE's footing, fell hard on the steps of the bus and rolled to the ground.

EE was injured while preparing S31 for moving from bottom of the Hematite Pit using the motivator. A cable crew was pulling slack and loops out of trailing cable. The trailing cable became taut contacting the ee causing a fall that resulted in injuries. The ee was transported to a hospital for evaluation and treatment of injuries, and later hospitalized resulting in Lost Time.

IP was in process of completing a delivery to customer site. Foot became tangled in rope between wheel chocks causing IP to fall. IP place arms out to break fall and when landed caused fracture of the bone.

While conducting industrial hygiene duties at the Tilden Concentrator, employee reported stumbling and falling forward landing on right hand. A minor injury was sustained, but employee did not seek medical attention. Employee continued to have a problem with the finger and was later evaluated. On 08/24/17, employee underwent surgery and was back to work the next day.

EE reports exiting cab of cable truck when experienced right knee pain. EE claims traction soles on protective footwear may have caught on step tread causing stress to knee while turning. After conservative treatment, follow up evaluation was performed where restrictions were ordered on 2-2-17.

EE was working on #3 Poly pump in the deslime basement on midnight shift and when going to retrieve a tool, he slipped and fell onto his back. He was evaluated by a Physician, given restrictions that the site could not accomodate. He was given a full duty release at his evaluation on 10-26-15.

A repairperson was operating a pneumatic impact inside a balling drum when material fell from the drum striking the ee causing a fall from the staging platform to lower level, about 2 ft below. EE sustained abrasions to back reporting pain in shoulder, back, & neck.EE was assessed by site MED PERS,& taken to HOSP for further eval.EE was removed from work, resulting in LOST TIME.

EE was stepping onto a metal stair system when right foot slid over the edge of a tread causing a slip down several steps results in right ankle getting caught on stair tread. He was evaluated by a physician, diagnosed with a fracture and taken off of work.

EE was turning on a water valve on the fifth floor of the Tilden crusher when the ladder being stood on slipped out from underneath. EE was evaluated by a Physician and taken off of work.

Alleged - Employees was standing next to a cyclone feed pump. When he stumbled backwards striking left elbow on a machine guard or concrete floor. The employee was evaluated by a Physician and taken off of work.

EE was lining in a primary mill. When he attempted to step off a ladder, he landed on uneven ground losing his balance. He reached back to catch himself, straining his R shoulder. He was evaluated by a Physician, given restrictions and returned to work.

EE was stretching a hose between 2 balling lines. He was backing up when his right heel caught a rolled up belt causing him to fall with his left shoulder striking the grating. He was evaluated by a Physician and returned to work with restrictions until his full duty release on 1-2-15

EE rolled left ankle as he was stepping down 11" onto the grating from line 7 flot feed pump concrete base. EE was evaluated by a Physician and ee was given restrictions.

EE was walking on the grating outside of the ER shop when his boot got caught in the grating resulting in the ee reporting pain to his left calf. He was evaluated by a Physician, and given an air splint for his left ankle. He returned to work full duty.

Employee alleges right knee pain after getting out of a pickup truck. He was evaulated by a physican and surgery was preformed on 8/18/14.

Employee was cleaning the drill towards the end of the shift. As he attempted to exit the drill, the employee slipped on the top step of the exit steps; (stairwell had a total of 4 steps) and fell, landing on his back. Ice had accumulated on the top step from the wash water the employee was using to clean the drill.

The employee was stepping over shovel tracks when he lost his balance and fell. His right foot was twisted in the tracks during the fall, resulting in injury. He was evaluated by a physician and given a splint for a dislocated right toe. He was released back to work.

EE was walking down a stairway and ee slipped on the bottom stair tread which was bent resulting in back discomfort. The ee was evaluated by a Physician on 11-6-13 and rtw with restrictions.

While deboarding a cable truck employee missed a step and fell onto his back. On 10/13/13 employee was evaluated in an ER and was taken off of work. He missed his scheduled shifts on 10/13 & 10/14. He returned to work full duty on 10/17/13.

Employee reported left knee pain after tripping on a chain. He was evaluated on 7/11 and taken off of work.

On 4/24/13 the employee tripped on grating and twisted his left ankle. On 5/17/13 he was evaluated by a physician and diagnosed with a fractured left ankle. He was given restrictions that prevented him from working. First day missed was 5/18/13. At a follow-up evaluation with a physician on 5/20/13, employee was taken off of work.

EE was walking and tripped over a grease line landing on his right knee. He has been followed by a Physician and on 7-18-13 underwent surgery resulting in lost time.
