Mostrando los 13On 1/15/2020 a mine mechanic slipped on the ice as the mechanic stepped out of a pickup. Initial medical diagnosis was a slight knee strain and the mechanic was allowed to return to work without restrictions. On 1/22/2020 the mechanic was seen by another doctor and it was determined that an MRI was needed for possible surgery and the mechanic was not allowed to return to work.
While working to replace roof structure a rafter failed causing an employee to fall resulting in a broken arm.
Employee had just climbed off of the bulk truck and slipped and tripped over some round grinding balls and fell to the ground injuring EE's right shoulder.
Employee tripped on pallet and forklift forks falling to the ground and landing on right arm/wrist, fracturing right wrist.
Employee had finished lowering boosters down a blast hole and turned to walk to the next blast hole when he tripped over a rock and fell. The workers compensation insurance has been investigating this case to determine if the injury is work related. On 02/09/2015 we received notice that this incident is determined to be work related.
Employee stepped back off of a 2 foot step ladder and lost his balance causing him to fall backwards. The employee instinctively stuck out his right hand to break the fall and struck a tool cart that was placed in the area. The impact caused a laceration to the palm of the right hand. Employee did not fall all the way to the floor.
Employee was helping perform maintenance on a conveyor belt. He put some tools in a service truck and was returning to get some more tools to return them to the tool room. As he was returning to the conveyor, he slipped and fell. Employee did not report the incident until the next day 05/20/2010.
Employee was changing picks on the Feeder Breaker; after he climbed out of the hopper (using the ladder) as he stepped from the ladder to the platform he felt a pain in his left leg just behind the knee. ER doctor released employee to work no restrictions 10/08/2008. Follow-up by another Dr. on 10/13/2008 he was placed on restricted duty.
Employee was pulling on forks of a forklift when he stepped backwards onto an uneven surface & twisted his right knee. He did not report any problems until 11/6/05. At that time he had an infection that required surgery. The surgeon would not comment on whether the infection was associated with the sprain. Workers Comp had another doctor review the case.
EE WAS USING A TORCH TO CUT PART OF A BLDG. STRUCTURE FOR REMOVAL PURPOSES. AS HE CUT THE FIRST END OF A BEAM, THE BEAM MOVED, WHICH CAUSED EE TO LOSE HIS BALANCE. THIS CAUSED HIM TO FALL & STRIKE HIS KNEE INTO A SECTION OF HANDRAIL. EE HAD HIS SAFETY HARNESS ON WHICH PREVENTED HIM FROM FALLING FURTHER.
EE WAS HELPING THE ELECTRICAL DEPT. PULL WIRES THRU A CONDUIT ALONG THE SIDE OF #4894 CONVEYOR BELT. THE WIRES HUNG UP THEN CAME LOOSE. AS THE WIRES CAME LOOSE EE STEPPED BACK TO MAINTAIN HIS BALANCE. EE S FOOT HIT A BELT SUPPORT BEAM ON THE GROUND CAUSING HIM TO FALL BACKWASRD. THE SUDDEN SLACK INT HE ROPE THAT WAS ATTACHED TO THE WIRES THAT WERE BEING PULLED THRU THE CONDUIT CAUS
EE WAS HELPING START EQUIPMENT FOR THE MINE OPERATION. HE HAD STARTED UNIT AND AS HE WAS EXITING THE CAB HE HAD HIS RIGHT HAND ON THE HOOD HANDRAIL AND STEPPING DOWN FROM FENDER ONTO THE TRACK S. THE TRACKS HAD SOME ICE BUILDUP FROM THE RAIN THE NIGHT BEFORE. WHEN EE STEPPED ONTO THE TRACKS HIS FEET SLIPPED AND HE FELL. AS HE WAS FALLING HE TRIED TO CATCH HIMSELF BY PUTTING HIS HAN
