Employee was installing a wear shield in the transfer box. While picking up the shield to bolt it in, the employee slipped bumping his elbow against the transfer box.
The employee was pressure washing the caterpillar 325CL backhoe. The employee was standing on a step ladder to reach the boom area. The employee was approximately three feet off the ground. The employee leaned over the side of the step ladder, it then tipped over, causing the employee to fall to the ground.
I HAD TAKEN THE CRANE & BASKER TO THE #10 CONVEYOR TO REPLACE SOME BAD IDLERS. I POSITIONED AND SET UP TO MAKE A LIFT. THE REPAIR CREW WAS ALREADY WORKING ON THE ROLLERS TO BE REPLACED. AS I W AS STARTING TO LIFT AN EE IN THE BASKET, I SAW SOMETHING FALL INTO THE PILE OF SAND BELOW THE BELT. I IMMEDIATELY LOWERED THE BASKET TO THE GROUND & THE EE RAN TO CALL FOR HELP. I WENT TO SEE
CLEANING FLOOR WHERE CHEMICAL HAD SPILLED ON FLOOR. PUTTING DOWN STAY DRY ABSORBENT. SLIPPED AND FELL ON RIGHT KNEE.
