Employee was greasing when EE stepped down from elevated platform (2 step) EE's left ankle rolled fracturing EE's fifth metatarsal. Employee was initially being treated with conservative care however went for X-rays on 06/11/2025 where it was determined to be fractured.

Employee jumped from loader ladder into soft berm and rolled EE's left ankle. Requested to see doctor on 3/26/2024 and was given restrictions.

EE WAS TRANFERRING FROM THE GANGWAY TO TRAILER WHEN LEFT HEEL OF BOOT CAUGHT ON THE DOME LID CAUSING THE EE TO STUMBLE. THIS RESULTED IN A STRAIN TO THE LEFT KNEE WHILE REGAINING THEIR BALANCE.

While walking on a flat surface, miner twisted left ankle causing a potential foot fracture.

EE had walked just outside the hydrate building North of blend loadout when EE collapsed falling to the ground. Medical opinion is that this occurred due to EE becoming overheated. However there is nothing to confirm the diagnosis. EE had a laceration requiring 5 sutures to close just over the left temple.

EE slipped and fell on snow and ice and was place on restricted duty following the incident. EE landed on their RT side including hip and elbow, however when medical treatment was sought it involved EE's right foot.

Employee was coming down the stairs of DCL#5 at the 75 Loadout. As EE stepped down on to the asphalt off the stairs EE's left foot gave out and rolled as EE planted it. The ankle swelled up immediately. The employee sought medical attention the following day and received restrictions to not be on feet for more than 4 hours at a time.

Miner was descending from a ladder, lost footing and used right arm to hold the ladder to prevent a fall. Miner sustained a sprain of the right shoulder rotator cuff. Physician put the miner on restricted work activities, and was released back to work immediately.

Employee was adjusting a wire rope sling, which was connected to a bar that assists in unrolling conveyor belt. The employee lost EE's balance and fell backwards. When EE fell backwards EE braced EE's fall on a pallet positioned behind self fracturing left wrist.

An Employee was walking down the stairs when they slipped on the last step and struck their arm on the midrail.

EE was walking from the breakroom to the West warehouse. When EE walked across an area that had been leveled with 2" rock EE lost balance. When EE fell and caught self with hands and suffered a contusion to left palm.

Employee was dismounting a forklift, slipped and strained right leg.

An employee was climbing down from the loader when right foot slipped off the top step at the rear left side of the loader. Medical attention was not immediately sought, but was later diagnosed with a strained knee.

EE was cleaning grease on the floor of Ball Mill 1, slipped and struck guard

Employee was accessing the top of a customer trailer to close lids. EE bent over to close the lid. With left foot on the rail, the rail broke, caused employee's left knee to come into contact with a rail bracket sustaining a sprained knee.

Employee was walking down stairs and caught boot on stair; employee was diagnosed with a sprained knee.

EE had sample bags in left hand and right hand was on the handrail while walking down stairs. Right hand slipped off the handrail and EE fell down six or seven stairs.

EE was walking on a wet concrete slope, lost footing, slipped & fell striking EE's rt wrist.

I pulled on the handle of a sand blasting pot to check the sand volume. The pot was completely empty and tipped back. I attempted to step back and tripped, pulling the pot down with me. The handle of the sand pot came down across my left ankle and caused a fracture.

Employee engaged in the process of demolition of #2 scrubber was injured when he fell while wearing fall protection harness attached to retractable lifeline. At some point employee fell through opening that was not protected by handrail. The employee fractured the humerus bone of his right arm near the elbow.

Employee was on top of open top railcar observing a maintenance task, when employee lost balance and fell over the edge of the railcar. Employee fell to the ground and sustained significant injuries.

EE stepped off of forklift onto muddy surface and hurt left knee.

Employee rolled ankle while walking. Sprain ankle/foot

Trucking company would not release personal information. A Non employee / contractor fell when descending from stairs at scalehouse. Injuries that occurred from fall were ankle sprain, Hip / thigh strain, Leg / knee sprain.

At around 7:00pm control room operator was replacing them belt on a screw. She was positioned on top of the cover for another screw. While opening the guard she lost her footing. When she made impact with the surface below approximately 2 feet she struck her right knee on an old pipe that is no longer in service. Currently at home waiting to return to the dr. for an MRI.

Employee slipped through bar grating while closing bottom hopper doors of rail car. Injury includes significant laceration to the left leg just below the knee. Seven sutures were required to close.

Miner was walking on some uneven ground and sprained his ankle.

Employee was descending from rear of VOLVO L350F Front End Loader and slipped and fell to the ground landing on his feet causing trauma to his upper back.

Miner using a-frame ladder to make repairs, stepped off ladder and missed bottom step. Miner' s foot landed awkwardly and caused strain to left groin muscle.

Miner had finished cleaning up a wet concrete floor with the skid steer when he stepped (already out of equipment) on concrete floor and slipped and fell.

Closing doors on coal railcar hopper, miner lost balance and twisted left knee.

Miner was walking down steps, at last step he twisted knee, tearing ligaments.

Employee was dismounting Komatsu Haul Trk in wet muddy conditions when foot slipped off access ladder. As a result a fall insued, and the right arm was lacerated on the toe plate of access platform. Injury required sutures to close the wound.

EE was coming down from loading hydrate when he stepped down from the sidewalk to the ground. He did not land on level ground and rolled over his right ankle.

Employee slipped and fell on wet surface straining left shoulder. This was initially classified as a first aid treatment that has been reclassified as a medical treatment. The reclassification occurred on 09/02/2010 as a result of medical intervention.

Employee making routine maintenance rounds stepping down from ladder and felt knee "pop". Worker reported and was monitored as he finished his shift. He returned the next day with stiffness in knee and requested evaluation by medical doctor.

Employee was standing on ladder when ladder slipped out from under him and his tools hit him in the lip. The ladder possibly slipped from dust material under legs of ladder.

Employee stepped through an uncovered 12" x 16" grated opening in an elevated catwalk resulting in a contusion to his right leg and hip

Lowered gangway to access fall protection harness when end section of the extended walkway gave way. Result was fall onto tanker top. He temporarily held onto the handrail but could not support himself. Fell to ground below.

Employee stepped over a small vertical wall from one elevation to another, in doing so he stepped onto a 4 inch symmetrical hose laying on the ground causing his foot to roll out and in turn shift his weight onto his left knee. His leg is immobilized to a point that he is restricted and unable to return to his normal job function.

EMPLOYEE WAS WASHING LOADER WHEN HE SLIPPED ON WET SURFACE STRAINING LOWER BACK.

Employee was utilizing three points of contact as he was accessing his 769 haul unit. He felt a strain in his right shoulder and back.

Employee was walking on paved surface within plant site when he rolled up on his left ankle. As a result employee fractured fibula at the left ankle.

Employee slipped and twisted right knee while crossing a warning barrier! Contributing factors include water flowing through area due to recent heavy rains! Injuries as a result of the activity include a sprain of a unspecified site of the right knee! Restrictions as a result include no squatting or kneeling, use of crutches 90% of the time and no climbing stairs or ladders!

EE WAS GUIDING AUGER ON BULK TRK TOWARD THE HOLEHE WAS FILLING WITH BLASTING AGENT WHEN HE FELT A STRAIN IN HIS BACK HE WENT TO DR ON 1/14/00 HE WAS NOT PUT ON MODIFIED DUTY OR LIGHT DUTY BY T HE DR HE WENT TO DIFFERENT DR 2/3/00 AFTER PAIN KEPT GETTING WORSE. DR PUT HIM OFF WORK 2/28/00 AFTER HE RETD TO DR ON 2/25/00
