EE reports walking to open valve to fill water tank, when right foot slipped on slippery ground causing right knee to twist while trying to maintain balance and not fall. EE describes right knee pain that prevents normal movement and prevents ability to work.

EE seen at ER w/no restrictions. On 12/15/25 EE had lingering complications. On 12/17/25 diagnosed w/fracture of L shoulder, not able to RTW until reevaluation.
Fell into a washout pipe while connecting it, with a dislocation of left shoulder

EE reportedly stepped into a small hole of missing grating causing a twisting fall to the decking floor. EE landed flat on back. EE found by ERT standing holding onto desk for support in control room. EE stated back pain that prevents normal walking. EE refused ambulance transport but did request transport to ER for evaluation.

Employee (EE) reported to the Clinic on 03/21/2024, states while stepping down backwards they missed the step and fell back on to their left hand and reported left wrist pain. EE was working washer area to tear down a pump. As moving EE stepped in a drainage ditch and experienced pain in left lower back. EE was referred to company care and given restrictions which could not be accommodated.

While walking dragline, EE was utilizing tractor to pull cable. Employee was descending off tractor while still on ladder, EE's left knee buckled. EE noticed slight discomfort but had good mobility. When EE awoke next morning EE's knee was swollen & had moderate pain. EE was sent for evaluation on 9/27 & placed off work. EE had previous injury to knee
EE was walking from pit car and stepped into a hole. EE stated EE did not hurt just wanted to report the incident. The following day EE did not report to work. EE was sent to Company Care for an evaluation. EE is off work due to restrictions which cannot be accommodated. Similar incident occurred 10/18/21.
Employee was walking in the area of the traveling water screens and the grating fell through the support causing employee's right foot to go into the resulting hole. Employee states that employee's left leg kept employee from falling all the way into the hole. Right ankle discomfort and placed out of work.
EE reports EE was walking in tunnel & stepped on uneven area & noticed lower back pain, later noticed right foot pain. EE went to emergency room, while there EE contacted EE's supervisor to report injury from previous shift who instructed EE to call clinic and report injury. Employee was clearing area and got a tree on the track of bulldozer, when EE got out of cab to get tree off EE slipped off cleat on the track and experienced pain in right knee. EE's pain increased throughout the night. EE able to ambulate, but states that it is sore. EE reported to clinic 2/3/20, EE had surgery on 8/21/20.
Employee broke for lunch and deviated from the JSA compliant travel path. The path chosen by the employee had slip, trip fall hazards. EE as a result slipped and fell and struck the back of head on the concrete surface. EE blacked out temporarily and as a result was transported for medical evaluation which determined that employee had a mild concussion.

EE was standing on top step of lift pump, stepped backwards, foot slipped EE fell on right buttocks. No complaints, over time had discomfort, on 9/5 EE was seen in clinic & later seen by co. dr. EE had prior back problem, referred to specialist no restrictions that would limit duties. F/up visit w/specialist on 1/26/18 condition had not improved, EE was placed on modified duty.

On 8/5(0700)employee presented to site clinic with unresolved leg pain & swelling from incident. The injured employee was evaluated & released to work with sedentary restrictions.
Employee was ascending stairs when left knee gave way causing employee to collapse onto the stairs.
Employee was dismounting D8 bulldozer when his foot slipped from bulldozer blade rail causing him to spin around hyper extending his right elbow under his body weight. He reported the incident and was able to return to work. He reported to work on 7/2 complaining of pain with restricted range of motion.
Emp. was changing out transmission filters on D-10. Emp. had to mount rear of tractor to complete job. As he was dismounting, he stated that the underlying rock/buildup shifted. At this time he experienced pain in his R knee. Emp was evaluated by an ortho surgeon on 10/29/09 and placed on restricted duty pending surgery.
General Foreman went to the #5 lift pump at the #4 matrix line to perform a key procedure audit. Three mechanics were performing a PM on the pump. As GF walked around the front of pump sled, he stepped into a hole that was covered with mud. The sudden step into hole caused his left ankle to turn, resulting in torn ligaments.

Employee, Heavy Equipment Trainee with 7 months experience, stepped over a piece of 20" matrix pipe and the ground gave way twisting his right leg. He reported discomfort in his right knee to his supervisor and saw the company doctor on 1/25/07. He worked without restriction until he was placed on restricted duty by the company doctor on 3/21/07 for subjective pain.
Employee was walking up a sand incline leaving a pumping station when his right foot slipped and he fell forward as his right leg shot outward. He experienced tenderness in his right knee and thigh. The company physician referred the employee to an orthopedic physician. Outpatient surgery was performed to repair the injured knee.
Injured employee was preparing to fuel an excavator. He placed the fuel nozzle on the catwalk by the excavator and then climbed up onto the catwalk. Once on the catwalk he lost his balance and jumped/fell 4-5 feet to the ground. An x-ray taken on 8/18 revealed a cracked bone. Employee was then placed on light duty status.
