EE was pulling weeds from the long belt. Lost footing and tripped over a rock. Causing a knee dislocation of the left knee. The foreman on site took EE to the nearest hospital for doctors to recorrect the limb.

Descending ladder and foot missed bottom rung causing employee to fall to ground and sprained ankle. Prescribed rigid boot and immobilization of ankle.
Employee sustained a left foot fracture when ee fell to the ground after cleaning exterior windows of a control room tower. Fracture was diagnosed on 2/1/2016; ee reported the injury as left foot sprain only on 1/22/2016 & did not seek medical attention. EE was terminated after completing a full incident review for willful disregard of safety & violation of fall protection.
Employee fell off of a ladder and fractured his left wrist.
Employee was fueling his equipment from a tank at the mining site. Employee was walking down inside of fuel containment berm when he slipped on the exposed plastic that had frost on it from the cold weather. Employee fell onto back compressing/fracturing his vertebrate.
Employee attempted to access area of washplant which was not protected by hand railing. In violation of company safety policies, training, and work practices, employee failed to use supplied fall protection equipment. Employee lost his footing on framework of plant and fell approx. 8 ft. to ground.
EE slipped on icy ground while draining washing/screening plant at end of shift. Lighting was marginal, but EE was aware of slippery conditions and did not spread available sand to aid traction. EE continued to work and saw physician on 11/25/08. At this time work restrictions were imposed. No lost time to date, diagnosis is pending MRI. Restricted work ongoing.
EMPLOYEE WAS UNLOADING METAL FROM THE BACK OF A PICKUP. HE WAS STANDING ON THE GROUND GETTING THE LAST PIECE OF 1/4 X 12 X 20 METAL. HE TRIPPED AND FELL BACKWARDS LANDING ON HIS BACK IN THE GRAVEL.
EMPLOYEE WAS SWEEPING DIRT OFF CATWALK OF LOADER. AS HE STEPPED FROM CATWALK TO ACCESS LADDER, HIS FOOT SLIPPED, CAUSING HIM TO FALL AND LAND ON THE GROUND. SUDDEN IMPACT CAUSED A SPRIAN OF LE FT KNEE. FIRST AID TREATMENT AND DIAGNOSIS WAS SUFFICIENT UNTIL NOV 21, 2001. A RETURN VISIT TO DOCTOR WAS NECESSARY.
