Employee was fueling his equipment from a tank at the mining site. Employee was walking down inside of fuel containment berm when he slipped on the exposed plastic that had frost on it from the cold weather. Employee fell onto back compressing/fracturing his vertebrate.

THERE WAS A ROCK IN THE TAIL PULLEY ONT HE CONE RETURN. EE AND OTHER TRIED TO BAR THE ROCK OUT. ROCK DID NOT MOVE EE WENT TO THE HEAD PULLEY AND WAS GOING TO TURN THE BELT BY HAND. OTHER DID N OT KNOW THIS AND GAVE THE SIGNED TO ANOTHER TO BUMP THE BELT ON AND OFF. THE BELT TURNED ON AT THE TIME EE GRABED THE BELT.
