While walking along a conveyor catwalk, employee tripped over EE's own feet and fell (same level), spraining EE's left knee.

EE was rebuilding slurry pump at booster EE took rubber guard off of hub seal on pump and laid it on ground next to work area as EE continued to perform task EE tripped on rubber guard and fell on stump and injured lower right side of back and cracked a rib.

Miner was walking from a face drill to a man-lift, distance of about 150ft, when EE lost footing causing discomfort and some pain in the left knee. The miner reported the walkway/work area was illuminated (cap lamp, drill lights & man-lift lights) and was free of slip & trip hazardous.

On 3/15/17 the employee slipped and fell. EE was treated and released to full duty at that time. As days/weeks/months went by EE began having pain in right shoulder. A medical exam in June 2017 revealed a shoulder injury likely caused by the fall and requiring surgery. Surgery occurred July 5th.

Climbing down from the Anfo Loader, lost footing, fell backwards, hurt left wrist.

Haul truck driver sprained their right ankle when EE slipped from the last step of the truck and fell to the ground as EE was dismounting the truck.

Blast Crew member injured EE's right hand when EE tripped over a rock while walking and reached out to catch self from striking the mine floor.

Miner was standing on the drill tire & using a pry bar to move into position for repair work the cylinder of the drill boom. He lost his balance and fell to the mine floor, apx 4ft below.

Crane operator (74 yrs old) fell 5+/-ft from a crane access step to the steel deck below. He was fueling and checking fluids on the Mantiwoc 4600 crane when he fell. He received fractured ribs, fractured collar bone, head & face trauma, temporarily unconscious and bruised lung. Was transported to local area hospital by ambulance.

A Contractor employee was working on top of a silo performing of an abandoned plant. The injured worker fell off the 64' silo to the ground, as a result the injured worker suffered a broken neck

The EE was standing on a 41" high conveyor frame using a torch to remove the walkway and handrails. The frame was positioned on the ground. While the EE was repositioning himself his foot slipped off the structure and went under on of the cross members. EE fell backwards, was upside down by 1 leg. EE sprained ankle and severe contusion to lower leg/sprained hip.

Worker received a sprained right ankle as he was stepping down onto a catwalk at the 5x16 screen. The catwalk was clean & free of debris.

While walking, the injured EE tripped on a small pile of sand, lost his balance and fell to his hands and knees. The injured EE sustained a strained right shoulder and returned to work with restrictions and prescription medication.

Haul truck driver was beginning to climb up truck's access ladder when the rubber hanger for the bottom ladder rung broke. This caused worker to strike right side of his torso against the truck bumper. Worker refused medical treatment but requested then requested treatment 3 days later.

Slipped and fell hitting chin on conveyor while trying to remove a speed sensor.

Worker fell apx 3 to 4 ft from catwalk of mechanical scaler while performing pre-shift exam on the mobile equipment. He stretched up to release a door to engine compartment when he lost balance and fell. He landed back-side first on mine floor against battery pack of his cap lamp.

EE was climbing down from the grizzly belt to the screen below. He then stepped from the screen to the handrai. As he was climbing down his foot slipped and his shin hit the I-Beam.

EE was walking and did not notice he was standing on ice, the ice was covered with sand. The ice broke and he went down to his shins and jarred his back.

Employee had finished his shift and parked his haul truck; when employee reached bottom step which is fastened to the truck by cables it swung in and his foot got caught under fender guard causing employee to fall backwards to the ground. Employee went to doctor who took x-rays and found three broken toes and a sprained ankle. Employee was put out of work until further notice.

Employee slipped and started to fall. Tried to grab a piece of steel which sliced open his left hand. Employee was taken to the hospital where he obtained stitches.

EE stepped in grease before mounting equipment. When EE was climbing ladder to the truck, his foot slipped on step. He held on with one arm and bumped his arm into the trucks fire extingusher holder.

Employee was climbing down from loader at end of shift. He slipped on step and landed on ground. He landed on his feet and hands. There were wintery, snowy conditions. He did not think he had hurt himself at the time, but over a couple months the condition did not improve.

EE WAS CLIMBING STEPS TO HAUL TRK CAB & LOST HIS FOOTING & FELL TO THE GROUND. THE INJURY TO EE'S LEFT ARM/SHOULDER, AS A RESULT OF THE EE'S HOLDING ON & TWISTING AS HE FELL TO THE GROUND. STEPS WERE WET WITH LIGHT RAIN. EE FELL APPROX 3 FEET.

EE SLIPPED ON ICE IN PARKING LOT.

EMPLOYEE WAS CLEARING MUD OFF OF TRUCK WINDSHIELD; STEPPED DOWN ONTO ROCK AND TWISTED HIS FOOT.

EMPLOYEE WAS GETTING OUT OF HIS HAUL TRUCK WHEN HE SLIPPED ON THE MUD ON THE LAST STEP. MUD IS THROWN ON THE STEPS BY THE FRONT REAR WHEELS. HE FELL BACKWARDS ONTO HIS BACK.

EE WAS CLIMBING INTO LOADER WHEN HE SLIPPED AND STRUCK HIS HEAD AGAINST TOP STEP CAUSING LACERATION TO SCALP & CONTUSION.

WHILE BACKING DRILL INTO A WORKING FACE, EE TRIPPED ON A ROCK WHILE WALKING BACKWARDS. EE FELL AGAINST THE FACE INJURING HIS RT. SHOULDER.

LIFTING A PIECE OF STEEL PLATE FOOT SLIPPED & PLATE FELL ON MIDDLE FINGER OF LEFT HAND

EE WAS STEPPING DOWN OFF OF DREDGE PLATFORM AND TWISTED RIGHT FOOT ON WASH HOSE BREAKING LOWER BONE IN RIGHT FOOT.

EMPLOYEE WAS PLUGGING IN BLOCK HEATER ON LOADER AND SLIPPED ON ICE AND SNOW WHEN HE CLIMBED DOWN FROM THE LOADER.

WALKED BACK TO THE SPUD TO PUT SAND ON IT. STEPPED ON THE MAN HOLE COVER & IT GAVE WAY. EE FELL THROUGH & RIGHT KNEE HIT THE SIDE OF THE MAN HOLE.

WALKING DOWN THE BOAT RAMP TO THE BOAT AND STEPPED IN A HOLE.

EE SLIPPED ON STONE/RUPTURED TENDON IN THE 5TH FINGER OF LEFT HAND.

WHILE PUTTING HOSE IN SMOKE BOX, SLIPPED OFF LADDER AND FELL TO THE FLOOR.

EE WAS WALKING TO JOBSITE, SLIPPED ON ICE & FELL.

EE WAS CLIMBING STEWPS ON T 24 WHEN HE SLIPPED AND FELL TO THE GROUND AND TWISTED BACK.

EE WAS WALKING UNDER THE C-10 TUNNEL BELT WHEN HE SLIPPED AND FELL LANDING ON HIS LEFT HAND AND WRIST. IT WAS HEAVILY SNOWING AND SLEETING.

EE WAS WALKING DOWN STEPS OF CRUSHER, HIS HEAD AND NECK JERKED BACKWARD CAUSING PAIN AND STIFFNESS IN NECK AND BACK.

WALKING OUT OF THE DRYER SCREEN ROOM, ABOUT TO TAKE A STEP TO GO UP 3 STEPS WHEN FOOTING WENT OUT FROM UNDER HIM AND FELL ONTO THE STEPS INJURING KNEE.

WHILE DESCENDING FROM EUCLIC, EE FOOT SLIPPED OFF LADDER RUN & EE FELL TO THE GROUND. THIS RESULTED IN A HYPEREXTENSION OF THE LEFT KNEE

EMPLOYEE WAS LEAVING SHOP TO GO HOME, HE STEPPED ON GRATING. AT THE END OF THE STACK OF GRATING WAS A SHORT PIECE WHICH SLIPPED OUT FROM UNDERNEATH OF HIS FEET. HE FELL BACKWARDS AND LANDED ON HIS HAND.

EE WAS BACKING OFF THE BOTTOM STEP OF A STAIRWAY ONTO THE FLOOR. HE WAS WEARING RUBBER BOOTS. HIS FOOT SLIPPED OFF BOTTOM STEP, PULLING MUSCLE IN LOWER BACK WHEN FOOT CONTACTED FLOOR.
