Employee was cleaning up Loadout area with a skid steer. The employee contacted concrete footing with the right edge of the skid steer bucket. The sudden impact propelled the employee forward and the top of their head hit the rearview mirror. Incident resulted in a laceration on their head that required 8 staples to close.

Employee was working in the bagging/packaging area. The table for bags dropped and left a 100 pound bag of material suspended. Employee opened the door to catch the 100 pound bag before it fell on the floor. As employee attempted to grab the bag, the 100 pound bag pulled EE's left arm backwards. Employee started physical therapy on 10/15/2019.

EE contacted L-knee with a protruding steel object while servicing a maintenance truck. Contact caused L-knee pain; medical treatment noted no overt body damage, a pre-existing infection due to a pre-existing medical condition noted. Antibiotics and Doctor ordered IV antibiotics and lost days due to being admitted to hospital for 2 days for treatment of cellulitis.

Slip and fall injury to right knee occurred. Employee did not initially want any medical treatment. Later on employee had knee swelling and drained by Dr on 12/6/2017. Doctors diagnosis was possible meniscal tear on right knee now made this a reportable.

Employee was shoveling sand when he felt a pull in his lower back area.

Employee was dumping a portable sand dumpster and his thumb nail on his right hand got mashed between the dumpster and backstop on the forklift.

Employee was palletizing 50 lb plastic bags. He went to turn and throw bag on pallet when he felt a pop in his left shoulder.

Employee saw spray nozzle was loose on airless paint sprayce and went to thighten it. His finger was on the trigger and it discharge hitting him on his left hand palm area. This caused a small cut.

Employees was holding a strap on a super sack to be moved to another location. Employee told the forklift driver to move ahead into the back right corner of strap. When the end of the forklift tine caught the tip of employees L- thumb, between the forklift tine and the strap, causing a cut.

EE was standing on wooden work platform 4" thick. He stepped off to retrieve a "bad bag" and caught the toe of his boot on the edge of the platform. He fell to the ground and struck his right knee on the concrete floor.

EE WAS WALKING TO JOBSITE, SLIPPED ON ICE & FELL.

GREASE FITTING ON FRONT END LOADER WOULD NOT TAKE GREASE. HE HELD GREASE GUN AGAINST CHEST AND PULLED LEVER WITH BOTH HANDS.
