A pit loader operator sprained their left ankle while dismounting their equipment. The employee was initially evaluated at the clinic on 7/7 and released to continue full duty with no restrictions. The employee was still experiencing pain on 7/21 and went back to the clinic, where they received restrictions and were prescribed physical therapy.

Electrician was working on deteriorated overhead electrical line from manlift. A section of line ran over the shop. The electrician exited the manlift onto the roof. While walking on roof he stepped on sky light & broke through the ceiling. He fell to the shop floor below. Safety harness was worn but there was nowhere to tie off.
EMPLOYEE WAS DISMOUNTING A HAUL TRUCK AND MISSED HTE LAST STEP. HE LANDED ON HIS RIGHT LEG WITH ALL HIS WEIGHT. HE SPRAINED HIS RIGHT KNEE. THE EMPLOYEE WAS NOT AWARE OF THE SPRAIN UNTILA F EW DAYS LATER. HIS KNEE BEGAN TO REALLY BOTHER HIM AND HE SOUGHT MEDICAL ATTENTION ON THE 28TH.

EMPL WAS WORKING ON CAT WALK HEN IT GAVE WAY & EMPL FELL 15' TO GROUND DISLOCTING RIGHT ELBOW. WHOLE BODY HIT GROUND.
EMP WAS GETTING3DOWN FROM BUCKET TRUCK ON A LOADXX LADDER, WHEN THE LADDER SLIPPED CAUGING HIM TO FALL AND INJURE HIS R WRIST

EMPLOYEE WAS STEPING DOWN OUT OF TRUCK WITH STEAM BHOSE IN HAND ONE FOOT WAS ON THE GROUND WHEN THE PRESSURE OF THE HOSE CAUSED HIM TO TURN AND TWIST HIP JOINT OR HURT DISC IN BACK BONE.
