Miner was called in to repair the splice in a belt. After completing the repair and reassembling the equipment, the miner checked the belt's operation. While inspecting the belt, the miner accidentally stepped on some clinker on the ground, causing them to slip and hyperextend their knee.

A visiting GCC EE had exited a company Pickup truck and was walking to the Maintenance Shop entrance. As EE was stepping up on to the sidewalk apron (approx. 6" high) EE caught the toe of EE's boot on the sidewalk lip and fell forward. The resulting impact of the fall fractured EE's right hip requiring surgery to repair the hip.

Employee was walking across the plant and slipped on ice. EE hit their head, which required 4 staples.

Employee was standing on a 3' high trailer, taping a tarp cover over a motor. EE's glasses fogged up due to wearing a mask, and when EE went to take a step EE fell off of the trailer, hurting EE's shoulder.

Employee was climbing down out of a haul truck, while holding jacket in one hand, and EE missed the grab handle and fell backwards to the ground.

Stepped up on curb and slipped on snow covered ice, breaking left wrist.

Weather was blizzarding with heavy winds. Miner was climbing onto a locomotive with snow on the step and slipped. EE tried to catch self, but fell with all of EE's weight on EE's right arm.

Employee was walking out ot the blend silos and when thee stepped down off of the curb they rolled an ankle. There was a large pile of dirt that they tried to step around so they stepped down wrong.

I was changing bags in Dust Collector and moved to the hand rail to tie rope on handrail and turned around to move back across the steel flooring and caught my foot on old cage that was on the floor and lost my balance and hit the floor with shoulder.

Carrying an extension ladder, my foot got hooked under the rim. I fell forward and landed on my right shoulder and hip.

I was checking on finding some Anti Freeze and I stepped off of a pallet onto the oil pit storage grating. The grating wasn't laying down all the way and it flipped and I fell into the oil pit. Please note:Employee did not report the accident until 11/28/2016

Employee stepped out of truck and when ee put right leg down, knee buckled and ee went down to the ground. EE states that ee cannot move right leg below the knee.

Employee was leaving the building and stepped on 2nd step and slipped on ice and fell backwards and landed on back, buttocks and arms/hands.

Employee was pulled up to fuel pumps with 988. When he was getting down off the machine, his left hand slipped off the hand rail of the ladder. I fell back and caught myself with the right hand causing my shoulder to pop.

Employee did not go to the doctor until 11/13/2014. Employee was at the landfill site unloading cement from a trailer. He climbed up on the truck to unhook the blower hose and fell off the truck and twisted his ankle.

Walking out of the south door of the silo. I saw it was snowy. I was being careful when my foot slipped. I hit the cement with my back and head.

Employee slipped on the ice and his feet went out from under him. He fell backwards hitting head, shoulders, neck and back. Injury is being reported 1/28 because employee brought a prescription from the doctor and has started treatment with a chiropractor.

Employee stepped off the forklift and twisted his ankle when stepping into the snow.

Employee stated back had been hurting. He jumped off the running boards of the bulk truck. Felt a tear in his back which totally immobilized him. Ambulance to emergency room. Emergency room doctor treated him for back pain, sent him home with medications and told him to see his doctor. Employee couldn't get an appointment until 7/1/2013.

Employee was walking to work during a snow storm. States he fell as he walked down the hill on St. Onge Street. He did not report the fall to the supervisor. Worked 1.5 hours. Asked to be taken home. After seeing doctor, he completed the accident form and states he stepped sideways on his ankle and heard a pop while working. Accident is under investigation as to work related.

Employee slipped on wet stairs and slid down 11 stairs on his backside.

Employee was doing a safety inspection and stepped off a step and missed the next step. Caught himself but put pressure on his knee and heard a pop.

Reported accident but did not seek medical attention. Requested medical attention on 11/23/2010. No treatment. 12/1/2010 physical therapy was prescribed by doctor for strained shoulder muscle.

Employee was wearing overshoes over his work boots because of weather conditions. Stated that he wasn't used to the extra weight and tripped and fell on his left side to the ground.

Employee was returning to work area after dropping off tools. He said his feet slipped out from under him and he fell. He tried to catch himself with his right arm and shoulder and fell his shoulder. Employee will be getting an MRI on Friday but at this time pain is his only symptom.

Employee was loading and installing brick retainers in end of kiln shell on the ground. When he climbed into the shell his foot slipped on the ice. He fell and hit his shin on the nose casting.

Employee was stepping up a 12" step (doorway threshold). The toe of his boot caught the top of the step; he lost his balance and fell forward. The fall caused the muscles in the calf of his right leg to contract in an awkward position and caused a strain.

He was walking on the North side of the office by the handicap entrance immediately after the snow had been removed from the parking lot. There was still a thin film of slippery ice/snow and he slipped and hit elbow and shoulder.

Employee slipped which caused his left knee to lock up and give him pain. He was taken to the dr to have his knee examined. The dr prescribed celebrex and tylenol, a splint and crutches, and returned to work with restrictions until follow-up appt on 12-19-05.

EE stepped off of loader ladder and when his leg touched the ground he strained/sprained his knee. Given knee brace on Nov 15, 2005. Note: Surgery on 1-31-06.

TRAINING EXERCISE - MOVING CABLE ON BOOM. EE CLIMBED 6' LADDER TO CHANGE CABLE ON BOOM SHEAVE. FELL OFF LADDER WHILE DESCENDING; HIT FRONT OF HEAD ON LADDER (OR ?), LANDED ON BACK ON GROUND. UNRESPONSIVE, BUT AWAKE WHEN AMBULANCE ARRIVED. REFUSED TREATMENT. WENT TO ER NEXT DAY; ADMITTED (3 DAYS). PRIOR BACK PROBLEMS AGGRAVATED BY FALL.

EMPLOYEE WAS WALKING DOWN THE STAIRS BY THE KILN 6 COOLER. THE SECOND STAIR UP FROM THE BOTTOM WAS COVERED IN ICE. EMPLOYEE SLIPPED ON THE ICE, AND HIS RIGHT LEG GOT CAUGHT IN THE STAIRS WHEN HE FELL. SPLINT ON ANKLE FOR APPROXIMATELY 3 WEEKS. (WALKING)

EMPLOYEE WAS WALKING TOWARDS THE MAILROOM WHEN SHE SIDE STEPPED HER SHOE WHEN ROUNDING THE CORNER FROM HALLWAY. THE SIDE STEP ACTION CAUSED HER TO TWIST HER ANKLE (LEFT) RESULTING IN A JONES F RACTURE. TRAVERSE FRACTURE OF PROXIMAL 5TH METARSAL = JONES FRACTURE.

WHILE WORKING ON BINS A-D IN THE TRUCK LOADOUT AREA, THE EE FELL ON HIS RIGHT SHOULDER WHEN HE MISSED THE BOTTOM STEP WHILE WALKING BETWEEN THE BINS. THE EE DID NOT PURSUE MEDICAL ATTENTION FO R THIS INJURY UNTIL 9/2002. RESULTING IN AN LTA DATING BACK TO THE ORIGINAL DATE OF INJURY. THIS REPORT WAS FILED AFTER EE RECEIVED TREATMENT AND COMPLETED LOST WORKDAY TIME.NOTE:AMENDED PER

EMPLOYEE WAS CLIMBING UP THE LADDER OF THE WA-600 LOADER WHEN HER HAND SLIPPED OFF THE LADDER, SHE FELL OFF THE LADDER LANDING ON HER WRIST.
