EMPLOYEE WAS STEPPING DOWN A STAIRS NOT FACING THE STAIRS AND WITHOUT THREE POINTS OF CONTACT WHEN EE LOST EE'S FOOTING AND SLID DOWN THE LAST THREE STEPS LANDING AWKWARDLY ON EE'S RIGHT ANKLE CAUSING A FRACTURE TO EE'S TIBIA ABOVE THE ANKLE.

While walking to EE's dozer employee tripped over EE's feet, hitting the bottom of the manlift with EE's left arm. The date of the incident the employees arm was x-rayed and no fracture was reported. On 8/31 an additional CT scan was performed, which now has reported a fracture.

While closing an access door a maintenance individual fell from a piece of mobile equipment and injured leg.

Employee was repositioning a sign and fell forward hitting/cutting EE's chin.

While an employee was conducting a walk around inspection of a 693 Overburden Truck, the employee slipped on packed snow injuring ankle.

Employee slipped on stairs in office area. Dislocated ring finger on left hand.

While exiting D11 dozer had hand on grab handle and went to step onto ladder to get off, slipped and fell to the ground.

A mechanic was walking around a piece of equipment. Slipped and fell on the ice and injured knee.

An employee was walking around the front of a pickup when he slipped on ice and fell backwards. He landed on his back, left shoulder, and left elbow. Employee was taken to ER for physician evaluation.

Emp tripped while going thru a revolving frame of an 8750 drag line injuring his right bicep muscle. Emp was taken to ER where the doctor diagnosed him with a bicep rupture. Emp did receive a full return to work and was referred by the ER doctor to a specialist on 9/15. The specialist required a MRI on 9/20. On 9/26, emp received results of MRI indicating that surgery was needed.

Security guard had come into the guard shack, tripped and missed a step and fell onto her knees. She extended her arms to break her fall. Her left arm got jammed in between the desk and a blue water cooler crate. Afterwards she felt no mobility in her arm and shoulder.

While Security officer was walking down the coal silo's catwalk his left foot slipped on the frosted grating. He caught himself from falling but twisted his right knee while doing so.

Security officer tripped while descending stairs, loosing her footing and spraining her left ankle.

Employee was assisting with setup of a work platform. When climbing down, he missed the step or slipped backwards approximately 3 feet contacting a retangular guard on an 8750 dragline. This contact resulted in a multiple rib fracture to the employee.

Employee was climbing down the ladder off of 101 dragline when she lost her footing and fell to her left, hitting her head on the floor of the dragline shoe. She received a laceration to her head. She was carrying a bucket of solvent in her left hand as she was going down, however she still contained three points of contact but lost her footing on the ladder.

Minor ankle sprain jumping from 2' bank.

The employee was climbing a ladder so that we could finish a welding project on a coal truck, when the bottom of the ladder slipped. The employee fell to the floor landing on his back and buttocks. The employee was treated at the minesite and taken to the hospital for an evaluation.

THE EE SLIPPED WHILE DISMOUNTING THE SWING CASE ON 101 DRAGLINE. THE EE BRUISED HIS HIP AND RECEIVED A CONTUSION ON HIS LEFT LEG.

EMPLOYEE TRIPPED OVER A GROUNDING CABLE AND FELL TO HIS KNEES.

EE STEPPED OFF DOZER AND FELT PAIN IN HIS BACK.

SHE WAS WALKING DOWN THE STEPS FROM THE CHANGE-OUT TRAILER AND MISSED A STEP.

WHILE RUNNING THROUGH THE SHOP YARD DURING A RAIN STORM, THE EE STEPPED ON UNEVEN GROUND & TURNED HIS ANKLE OVER. RESULTED IN SPRAINING HIS RT. ANKLE.

STANDING AT THE TOP OF A STAIRCASE AND HANDING DOWN A BOX OF RAGS. FOOT SLIPPED AND WAS HOLDING ON RAILING WITH MY RIGHT HEAD. RIGHT SHOULDER WAS JERKED WHEN THE FOOT SLIPPED.

EE WAS WALKING ON MUDDY GROUND CONDITIONS, HE LOST HIS FOOTING AND FELL ON HIS BACK. STRAINED MUSCLES IN HIS LOWER BACK.

EE WAS DISMOUNTING FORM A CABLE TRACTOR AND WHEN HE PLACED HIS RIGHT FOOT ON UNEVEN GROUND HE TWISTED HIS ANKLE. RESULTED IN TORN LIGAMENTS IN HIS RIGHT ANKLE.

EMPLOYEE WAS DIMOUNTING MOTOR GRADER TO CLEAN WINDOWS AND SLIPPED ON STEPS AND FELL STRIKING HISLEFT ELBOW ON THE BOTTOM OF THE DOOR JAM. RESULTED IN A LACERATION THE REQUIRED SUTRUING. THE GR OUND AND STEPS WERE WET AND MUDDY DUE TO RAIN.

EMPLOYEE'S CAP BLEW OFF IN EMPLOYEE'S PARKING LOT AND AS THE EMPLOYEE WAS RUNNING AFTER HIS CAP HE SLIPPED AND FELL ON SNOW COVERED ICE. RESULTED INS TRAINING LIGAMENTS IN HIS RIGHT WRIST.

EMPLOYEES WERE MOVING A WALK BEHIND TRENCHING AMCHINE, EMPLOYEES FEET BECAME TANGLED WITH ANOTEHR EMPLOYEE'S FEET AND AS THE EMPLOYEE STUMBLED HIS FOOT TWISTE CAUSING THE TIBIA AND FIBUAL BONE S IN HIS LEFT LEG TO BREAK ABOVE THE ANKLE.

INJURED STATED WHILE HE WAS MAKING THE LONG STEP DOWN FROM THE DRAGLINE HE SLIPPED ON SOME UNSTABLE GROUND AND HYPEREXTENDED HIS RIGHT KNEE.

WHILE STEPPING FROM WORK PLATFORM TO A STEP STOOL THE STEP STOOL SLID & THE EMPLOYEE FELL TO THE GROUND RESULTED IN BRUISING HIS TAILBONE

WHILE MOVING ACROSS THE FRONT OF A TRACTOR TO CHECK THE OIL, HE SLIPPED ON THE BATTERY BOX AND STRAINED MUSCLES IN HIS LOWER BACK.

WHILE AN EMPLOYEE WAS DISMOUNTING A PIECE OF EQUIPMENT, HIS FEET WERE MUDDY, & AS HE STEPPED DOWN HIS LEFT FOOT SLIPPED OFF THE STEP & HIS OTHER FOOT LANDED ON THE FLOOR & TWISTED. TESULTED IN A SPRAIN TO HIS ANKLE ON HIS RIGHT FOOT.

SLIPPED & FELL WHILE CONDUCTING A PRE-SHIFT INSPECITON ON A 195-M MARION DRAGLINE. EMPLOYEE RECEIVED A LACERATION TO HIS LEFT ELBOW THAT REQUIRED SUTURING.

EMPL WELDING OVERHEAD WHEN LEGS SLIPPED AND EMPL FELL INTO A "SPLITS"POSITION.

STEPPING DOWN OFF FROM THE BACK OF A SCRAPER, WHEN HE PLANTED HIS RIGHT LEG ON THE GROUND, HIS KNEE OVER EXTENDED TO THE SIDE.

SLIPPED AND FELL FROM THE BED OF A LOW-BOY.

EMP SLIPPED ON 3CE AND FELL WHILE GETTING ONTO D10 DOZER. MEDICAL TREATMENT WAS GIVEN FOR BRUISE TO RIBS WHEN EMP SLIPPED AND FELL WHILE CLIMBING UP TEH FOOT HAND HOLD ARA OF DOZER ARM. HE FE ONTO THE DOZER 5RM CYLINDER WHILE FACING THE DOZER
