While walking bleeder entry with MSHA inspector on 12/10/13 Glade Taylors back gave out causing him to fall in some rocks. He felt it was caused by an accident he had earlier in the year,at this time he felt pain in neck and back his first day off was 12/11/13.
EMPLOYEE IN OFFICE STATES GOT UP WALKING FELT DIZZY M, REACHED FOR THE CHAIR , BUT FELL DUE TO DIZZINESS AND HIT MOUTH ON CHAIR, KNOCKED OUT TOOTH.
EE was getting out of a mantrip and placed his left foot under the frame of the mantrip on the mine floor. When he placed his right foot on the mine floor, he slipped and fell backwards, spraining his left foot that was under the mantrip frame.
EE was loading rock dust into a rock duster and fell and struck his right shoulder on the rock duster. X-rays were negative.
EE was setting a belt head and stepped backwards and stepped on a crib block, causing him to fall and fracturing (hairline) his left ankle.
EE was working on the 3rd floor of the Screening Plant. He was at the sizing grid and stepped on a piece of floor grating. The piece of grating gave way and EE fell approximately 12 ft. to the floor below.
EE was walking around the roof bolter and tripped and fell, striking his knee on a rock that was lying on the mine floor.
EE was greasing the tailroller at the section tailpiece. As he was starting to stand up, his foot slipped causing him to twist his right knee. He continued to work regular duties until 6-3-08, when the MD restricted his activities.

EE was walking in the #6 entry of the 001-0/002-0 MMU outby the section power center and stepped in a hole and twisted his left knee. EE continued to work at his regular duties until 5/2/06 when MD placed him on restricted duty.
CONVEYOR STRUCTURE TURNED (ROLLED) ONTO IT'S SIDE DUE TO MATERIAL OVERLOAD ON WALKWAY (SLURRY PIPE). IT STRUCK VICTIM ON ARM. HE FELL BACK AND STRUCK METAL TRUCK BED. INJURED ARM & BACK.
EE was servicing a continous miner in the #2 entry of the 002-0 mmu. He was walking around the front of the continuous miner and slipped on loose coal, fell, causing injury to left knee.
EE WAS STEPPING TOWARDS THE CONTINUOUS MINER TO REMOVE A BUCKET OF MINER BITS & STEPPED ON A ROCK, SLIPPED & FELL, CAUSING A LACERATION TO THE RT. KNEE. ACCIDENT OCCURRED IN THE #5 ENTRY OF THE 003-0 MMU.
