ICY STEPS, SLIPPED AND FELL LANDED ON BACK. BRUISED BACK AND LUMBAR STRAIN

EE was walking down the slope beltline and slipped and fell, causing pain to his lower back. He was seen by his family DR. on 9/3/2013 who took him off work until his next Dr. visit.

PRE SHIFT START TIME EMPLOYEE GETTING STUFF OUT OF HIS VEHICLE AND WAS STANDING TO CLOSE TO DITCH BERM AND DITCH GAVE WAY AND EMPLOYEE TUMBLED INTO DITCH. FIRST AIDE ADMINISTERED AT HOSPITAL. EMPLOYEE STATED DR. DID NOT PUT HIM OFF FROM WORK, BUT HE CAN'T COME BACK YET

While walking bleeder entry with MSHA inspector on 12/10/13 Glade Taylors back gave out causing him to fall in some rocks. He felt it was caused by an accident he had earlier in the year,at this time he felt pain in neck and back his first day off was 12/11/13.

EMPLOYEE IN OFFICE STATES GOT UP WALKING FELT DIZZY M, REACHED FOR THE CHAIR , BUT FELL DUE TO DIZZINESS AND HIT MOUTH ON CHAIR, KNOCKED OUT TOOTH.

EE was getting out of a mantrip and placed his left foot under the frame of the mantrip on the mine floor. When he placed his right foot on the mine floor, he slipped and fell backwards, spraining his left foot that was under the mantrip frame.

EE was loading rock dust into a rock duster and fell and struck his right shoulder on the rock duster. X-rays were negative.

EE was setting a belt head and stepped backwards and stepped on a crib block, causing him to fall and fracturing (hairline) his left ankle.

EE was working on the 3rd floor of the Screening Plant. He was at the sizing grid and stepped on a piece of floor grating. The piece of grating gave way and EE fell approximately 12 ft. to the floor below.

EE was walking around the roof bolter and tripped and fell, striking his knee on a rock that was lying on the mine floor.

EE was greasing the tailroller at the section tailpiece. As he was starting to stand up, his foot slipped causing him to twist his right knee. He continued to work regular duties until 6-3-08, when the MD restricted his activities.

EE WAS HELPING MAKE A BELT MOVE. HE WAS CARRYING A STRUCTURE RAIL AND SLIPPED AND FELL ON A HAMMER THAT HE WAS CARRING ON HIS SIDE, INJURING HIS HIP.

EE was walking in the #4 entry of the 001-0 mmu when he slipped and fell, resulting in pain in his back.

EE was walking in the #3 entry of the 003 mmu and stepped in a hole in the haul road and strained his back. He worked regular job on 11/3/06. Did not seek medical attention until 11/6/06.

EE was walking in the #6 entry of the 001-0/002-0 MMU outby the section power center and stepped in a hole and twisted his left knee. EE continued to work at his regular duties until 5/2/06 when MD placed him on restricted duty.

CONVEYOR STRUCTURE TURNED (ROLLED) ONTO IT'S SIDE DUE TO MATERIAL OVERLOAD ON WALKWAY (SLURRY PIPE). IT STRUCK VICTIM ON ARM. HE FELL BACK AND STRUCK METAL TRUCK BED. INJURED ARM & BACK.

EE was servicing a continous miner in the #2 entry of the 002-0 mmu. He was walking around the front of the continuous miner and slipped on loose coal, fell, causing injury to left knee.

EE WAS STEPPING TOWARDS THE CONTINUOUS MINER TO REMOVE A BUCKET OF MINER BITS & STEPPED ON A ROCK, SLIPPED & FELL, CAUSING A LACERATION TO THE RT. KNEE. ACCIDENT OCCURRED IN THE #5 ENTRY OF THE 003-0 MMU.

EMPLOYEE TRIPPED AND FELL OVER CABLE AND EXPERIENCED KNEE PAIN.

EE STEPPED IN A HOLE & TWISTED RIGHT ANKLE

WHILE CHANGING PLACES WITH MINER EE TRIPPED OVERMINER CABLE AND FELL COMPLAINED OF PAIN IN BACK AND HIP

EE EXITED THE BUILDING, STEPPED DOWN TO CHECK A FAN, THE HOPPER HORN SOUNDED, HE TURNED TO STOP AND SLIPPED-FELL ON STEP. IT HAD RAINED AND STEPHAD MUD AND GRAVEL OVER ON IT.

EMPLOYEE WAS DOING MAINTENENCE ON MINER. HE WAS STANDING ON TOP OF MINER, LOST HIS FOOTING, SLIPPED AND FELL TO THE FLOOR CAUSING PAIN TO LEFT KNEE, LEG AND FOOT AND TO HIP AND LOWER BACK.

EMPLOYEE WAS PULLING A PIECE OF CURTAIN, SLIPPED AND FELL CAUSING PAIN IN BACK. EMPLOYEE DID NOT MISS WORK OR RETRIEVE MEDICAL TREATMENT UNTIL 10-21-96 AT WHICH TIME ACCIDENT BECAME REPORTABLE TO MSHA.

EE WAS WALKING BACKWARDS AND TRIPPED AND FELL, CAUSING BACK PAIN AND TINGLING IN HIS LEGS.

EE WAS WALKING TO THE MINER TO CHECK A PROBLEM WITH THE MACHINE, WHILE ON THE WAY HE TURNED & TWISTED HIS KNEE. INCIDENT HAPPENED ON 2/9/96. EE DID NOT MISS WORK OR RECIEVE MEDICAL TREATMENT U NTIL 4/1/96 AT WHICH TIME INCIDENT BECAME REPORTABLE TO MSHA.

EE WAS WALKING TO BATH HOUSE TO GET SOME HAND TOOLS HE SLIPPED ON SOME ICE & FELL STRIKING HIS RIGHT SHOULDER & HEAD.

EE STEPPED OVER A PILE OF PINNER CABLE CAME DOWN ON LEG WRONG AND TWISTED HIS KNEE.

WASHING DOWN FIRST FLOOR WITH 1 1/2" WATER HOSE,I.NAME BACKING UP AND TRIPPED.FELL ONTO CONCRETE FLOOR STRIKING BACK AND RIGHT ELBOW.

EMPLOYEE BECAME ILL AT THE BEGINNING AND INFORMED HIS FOREMAN THAT HE NEEDED TO GO HOME.THE FOREMAN TOLD HIS MECHANIC TO GO WITH HIM A ND DRIVE HIM HOME.WHEN THEY WENT OUT OF THE PLANT OFFICE, EMPLOYEE FAINTED STRIKING LOWER JAW ON PAVEMENT.
