On 2-3-15 employee was descending a ladder when he inadvertently missed the last rung. As he stepped past the last rung and made contact with the ground he twisted his left knee. Employee continued to work while medical diagnostics and reviews were being done. He was taken off of work 4-30-15 for surgery to repair his left knee.

Mostrando los 31Employee was walking to the tailgate to reset a faulted shield during the mining process. When he reached 125 shield a bounce occurred. The bounce threw material toward the shield employee was standing on. The material hit the bounce guard which resulted in employee being knocked over striking his ribs against the shield. This caused a small laceration to his spleen.
On 4/8/13 ee fell straining his shoulder, he continued to work until 5-9-13 at which time he was seen by a Dr. and taken off work. It was undetermined at this time if his condition was work related, on 8-29-13 it was accepted as an industrial accident, he is schedule for surgery 0n 9-12-13. The difference of the accident date and reporting date was pending acceptance.
Employee was placing a section of wire mesh on the atrs of a roof bolter and as he went to step onto the driller's platform, he missed the platform and his leg dropped approx. 2 feet below the platform. This caused him to strain his right knee. He was scheduled to see an orthopedic Dr. for an evaluation. The Dr. has now taken him off work.
On 4-23-2012 employee was traveling down the longwall face when he tripped and fell injuring his shoulder. He continued to work until March 28th 2013 at which time he had surgery to repair his shoulder. First day of work missed due to this injury was March 28th.
EE was working at the mines tipple and as he was walking to a parked truck, he slipped and as he fell he landed on his left elbow which caused a separation of his left shoulder.
On January 12, 2012 EE was walking and his feet got tangled up in a plastic band which caused him to fall. As he went to catch himself he injured his left shoulder. EE continued to work. On February 3rd, he went to the Dr. and was taken off work. The first day of work missed was 2-6-12.
On 7-18-11 employee was stepping down off a gob pile and the material shifted causing him to twist his right knee. He continued to work but his knee never healed. On 1-30-12 he had to have surgery to repair his knee.
On 4-4-2011 employee was walking in the mine and slipped and strained his knee. He was seen by a Dr. and released for work. His knee continued to bother him. He was seen by a Dr. again and it was determined that surgery was needed. Employee's first day off work because of the surgery was July 25, 2011.
Employee slipped while tightening a bolt with a ratchet and strained his shoulder. This employee worked for several weeks then went off work and had surgery. Date noted above was first report of injury. Difference of date of injury and date of report was pending a medical report from physician and acceptance as in industrial injury.
Employee claimed he was stepping backwards and fell and as he reached back to catch himself he strained his shoulder. This employee had a previous condition with this shoulder. The difference between the date of injury and reporting date was pending a medical report from the Dr. and acceptance of the claim.
On 3-26-07 EE was getting out of a truck and his foot got hung up, he fell to the ground injuring his back. He continued to work with back pain through this time period with no other incidents. On July 18, 2011 he had surgery which has been accepted as an industrial injury. The difference of the first day off and report date was pending a medical report.
Employee was walking through an area that had been re-graded and slipped on some loose rock causing him to fall. As he reached to catch himself with his hand, he jammed his wrist, which caused a fracture to his wrist.
Employee was walking to his car in the parking lot and slipped on some ice and fell jamming his shoulder. The employee continued to work. After a followup visit with a physican it was determined that surgery was required. On 4-2-2007 surgery was performed. This was the first day away from work.
Employee stepped in a mud hole and twisted his knee as he turned around. The difference between the accident day and report day is because employee continued to work. First day away from work due to injury was 4-30-2007.
EE stated he was rock dusting and stepped on an uneven surface and strained his knee. He finished his shift without incident. The following day he seen a doctor and was scheduled for exploratory surgery on 10-23-06. The medical report indicated a tear. The report was received 11-29-06. The difference from the accident and reporting date was due to receiving a medical report.
Employee was stepping down from a belt cross over and twisted his knee causing him to fall. The fall hyper extended his knee. The initial doctor visit released the employee to work. On 9-26-06 he return to his doctor and has been taken off work. This is why there is a difference from date of injury and date report was submitted.
As the employee was walking along the side of a bundle of wire mesh he stumbled and as swung his arms to catch his balance he struck a piece of metal banding which caused a laceration to his thumb.
The employee was cleaning a floor in a service pit area and became dizzy and fell straining his back, leg and shoulder. The employee had been suffering dizzy spells for several days prior to this incident. The employee is also seeking medical treatment for the dizziness.
Employee was bending a piece of wire mesh and fell on the mesh causing a cut to his chin.
Employee was walking in a intake entry and stepped in a hole twisting his knee. The employee work several days after this incident before seaking medical treatment. The employee recieved medical treatment for this injury April 14th 2005.
EE WAS STANDING ON A STEP LADDER, LADDER TWISTED AND EE FELL TO THE MINE FLOOR LANDING ON HIS ELBOW. EE REPORTED ACCIDENT LATE, REPORTED ON 9-4-02, DR. VISIT WAS ON 9-5-02.
EMPLOYEE WAS REMOVING A BELT RAIL FROM A SET OF BELT STRUCTURE. HE SLIPPED AND FELL, WHEN HE FELL THE BELT RAIL STRUCK HIM IN THE CHIN.
EMPLOYEE WAS PULLING ON A SHUTTLE CAR CABLE AND SLIPPED AND FELL. EMPLOYEE REACHED BACK TO CATCH HIMSELF AND STRAINED HIS SHOULDER.
EMPLOYEE WAS WALKING DOWN AN ENTRY, TRIPPED ON A PIECE OF COAL CAUSING HER TO FALL.
EE STEPPED OUT OF A PIECE OF EQUIPMENT, STEPPED ON A ROCK AND ROLLED HIS ANKLE.
EE WAS SITTING IN A CHAIR EATING HIS LUNCH WHEN SOMEONE HIT THE FLOOR UNDER HIM CAUSING HIM TO JUMP OUTOF THE CHAIR & FALL TO THE FLOOR
EMPLOYEE WAS CROSSING AN OVERCAST GOING DOWN THESTEPS WHEN HE TRIPPED & FELL CAUSING A FRACTURE TO HIS LEG.
THE EE WAS INSTALLING HOSES ON THE SHIELDS OF A LONGWALL, HE SLIPPED ON THE TOES OF THE SHIELDS AND TWISTED HIS KNEE.
EE STATED THAT HE GOT OUT OF A PIECE OF EQUIPMENT AND AS HE WAS WALKING AROUND THE EQUIPMENT HE STEPPED IN A HOLE AND TWISTED HIS KNEE.
EMPLOYEE WAS CLEANING TRASH OUT OF FEEDER BREAKER WHEN HE SLIPPED AND FELL HURTING HIS ARM AND SHOULDER.
