Removing wheel unit from 785C using a pry bar and crane on service truck. Hub came off spindle and struck employee on left hand causing employee to loose balance didn't fall but twisted employee's left hand and knee in the process.

Employee was applying rock dust and cleaning the area when he tripped over loose material causing him to fall and injure his left knee. He finished his shift but went to the ER that evening. He was diagnosed with a Left knee injury and calf strain and has been excused from work.

Employee was building a stopping when his foot became entangled in the band that secures the blocks causing him to twist his knee and fall. He went to the ER and was diagnosed with a knee sprain and released to return to work. Employee worked until 02/22/13; when he went for an MRI which revealed an ACL tear and he was excused from work. First day away 02/25/2013.

Injured employee was adjusting belt that had been loaded onto a flat car using channel locks. The channel locks slipped causing employee to fall backwards against the rib injuring his right hand & wrist. He continued to work but later went to the ER for X-Rays. Right wrist Hairline Fracture diagnoses by orthopedics on 02/03/12.

Injured employee was walking down the track entry when he stepped in a hole and twisted his right ankle. He was transported to the ER and diagnosed with a right distal tibula fracture. He was excused from work and referred to Orthopedics.

Injured employee slipped into a muddy ditch on the offside of the belt between 3 Head and Drive Motors causing him to sprain his right ankle. He was transported to the ER for evaluation and diagnosed with a sprain. He has been referred to orthopedics.

Injured employee slipped and fell while connecting a water hose causing him to strike a belt roller laying on the ground resulting in a laceration to the right hand requiring 2 sutures. Injured returned to work the same day.

Injured member was stepping off a haulage car when he slipped in the mud causing his legs to split and injuring his left hip. He was diagnosed with a muscle sprain/strain and excused from work.

Injured employee was building forms. Slipped on gravel and fell down bank on to shoulder.

Injured member was carrying a feeder jack and as he was stepping over the feeder cable his knee twisted causing him to fall to the ground. He completed his shift and the following shift. On 03/08 he went to the ER and was told by the physician that he could possibly have a Meniscus tear and scheduled an MRI and excused him from work.

Employee was getting a 6" pipe elbow out of a box outside of the plant. When he bent over to get the pipe elbow out of the box, his foot slipped on ice as he was lifting the elbow out of the box. In regaining his balance, he jerked suddenly and pulled a muscle in his left lower back.

Injured member was walking and conducting his fireboss duties when he slipped and fell. He attempted to break his fall by placing his hands in front of him and injured his right wrist. He has been diagnosed with a wrist fracture and has been excused from work.

Injured employee was ascending the steps from the basement of the plant when he tripped and fell injuring his left hand. X-rays revealed a fractured bone behind his left little finger. Employee was excused from work by treating physician until further notice.

Injured was assisting the scoop operator by placing the scoop on charge when he slipped and fell. Injured struck his left knee against a rock on the mine floor causing pain and swelling. He was transported to the ER. The ER physician excused from work and remains excused from work due to increased swelling and edema in the knee.

EE leaned up against the fan wall. Wall folded and fell with the EE resulting in a bruise to the left knee.

Injured employee was walking back to the roof bolter controls when he twisted his left ankle. Diagnosed with a sprain and placed on light duty.

He states that while he was setting a timber he stepped in a hole and twisted his back.

Injured was conducting an inspection on the 214A and 214B screens, when he tripped on a protective barrier, causing an injury to his right foot and ankle.

EE was standing on step ladder,slipped and fell fracturing 2 ribs on left side he was placed on restricted duty on 7-28-08

Member states as he was getting off dozer he slipped and fell to the ground injurying his right ankle and lower back

employee states that he was walking and there was a stepdown he didnt see and twisted his left knee

While standing on crib blocks working on miner, crescent wench slipped causing him to loose his footing, and he jarred his lower back

Employee was starting up the steps at the RC 4 Tower from the 2nd floor to the 3rd floor. As he reached the 3rd floor he slipped on ice causing him to fall onto his right arm. Wiley was then transported to Boone Memorial Hospital. X-rays were taken and were negative.

Employee was loading drill into #8 man hole, injured and twisted right knee.

EE was pushing on a piece of 3/8" steel plate to straighten it up on the forks of a forklift. Stepped in a rut and twisted his ankle.

Employee was unloading belt out of the storage unit, as he was pulling on the belt, he slipped hitting his leg on the flat car the belt was being loaded onto. Employee went to the hospital on 9/2/06. Xrays were taken and revealed a slight fracture in his left leg. On 9/5/06 employee notified the Safety Dept. on his condition.

While setting timbers, EE stepped back to set wedge and fell backwards over a timber butt. EE fell on his rescuer, which he was wearing on his back, causing injury to his lower back.

EE was starting to wash down rollers using a hose, when the pressure pump kicked on it caused him to slip and fall. He fell on his right thumb fracturing it.

AS HE WAS WALKING THROUGH THE #36 CROSSCUT ON THE LONGWALL BELT, HE SLIPPED IN A TIRE TRACK & FELT PAIN IN HIS RT. KNEE. HE CONTINUED TO WORK UNTIL 3/27/06, AT THIS TIME HE WAS DIAGNOSED WITH A TORN LIGAMENT.

Injured was installing a return trailing roller. While welding, bees came out of structure and injured swatted at them. His foot slipped off and he fell. He hit face first striking his left arm & neck & lower back. He had an MRI on 9-28 & was advised to stay away from work until further notice. His first day missed was 9-29.

Employee was walking down mine office steps when his left boot tread caught on steps causing him to twist his left ankle.

Tripped and fell on left side, dislocated rib.

Injured was exiting off the manlift on the 6th floor and reached to open the gate to step through when he slipped and fell. He was treated by FMTS and transported to Memorial Hosp. where x rays revealed a fracture to the right fibula. He visited a specialist on June 23rd where a soft cast was applied above the ankle on the right leg.

Employee was hanging cable and slipped in the mud when he twisted his right knee. His first day away from work was Aug. 28.

Employee slipped and put his hand down to catch himself. His hand contacted a piece of metal, cutting his palm. He received stitches.

Employee was on the third floor stairway platform in A plant when he noticed the heavy media cyclone sump overflowing. When he started to descend the stairs his right foot slipped on the first step causing both feet to fly out. He landed on his lower back and buttocks about four steps below. At the hospital he was diagnosed with compression to L4 & L5 disks.

Injured had just completed servicing scrapers on the #8 belt head and was walking toward the track entry. As he walked behind the belt take-up he stepped on a piece of conveyer belting that was laying on the mine floor and covered with water and rockdust causing him to slip and fall backwards straining his left knee.

Employee was walking through the last open crosscut towards the active longwall face when he slipped in mud and water, causing him to do a split with his right knee buckling under him. He was transported to BMH where x-rays were negative. He continued to work until Friday, Jan. 21st when he went back to hospital due to increased pain. First missed day was Jan. 24.

Mechanic/ welder was putting crib blocks under a D11 dozer, he was on his knees while performing this task. When he went to move his knee popped.

EE was installing tin roof on new security station when he fell approx. 9'. employee was transported to camc general where he was dragnosed with a fractured left wrist fractured mandible and a crack to the left foot. recovery time is expected to be 6 weeks.

EE WAS GETTING OFF OF THE SCOOP WHEN HE SLIPPED AND FELL. HE STRUCK HIS RIGHT KNEE ON A ROCK WHICH CUT IT. HE RECEIVED 8 STITCHES.

EE FINISHED BOLTING CUT AND TURNED THE MACHINE OFF. AS HE TURNED TO WALK OFF THE OPERATORS SIDE PLATFORM HE SLIPPED ON THE PLATFORM STEP STRIKING THE RAILING HE RECEIVED A CUT ABOVE HIS LIP AN D ALSO ON THE INSIDE OF HIS LIP HE WAS TRANSPORTED TO B.M.H. BY AMBULANCE.

EE WAS WALKING DOWN THE TRACK ON 7HG CARRYING HYDRAULIC OIL WHEN HIS FOOT BECAME CAUGHT ON THE TRACK TIE CAUSING HIM TO FALL INJURING HIS LEFT LEG/ ANKLE. X-RAYS REVEALED A FRACTURE TO THE L EFT FIBULA. HE WAS TOLD TO REMAIN AWAY FROM WORKAND SCHEDULED TO RETURN FOR RE-EXAMINATION 8-21-03.

EMPLOYEE WAS CARRYING A CABLEBOLT ALONG THE OFFSIDE OF THE ROOF BOLTER WHEN HE TRIPPED OVER DRAW ROCK ON THE MINE FLOOR. EE INJURED HIS LEFT HAND AND WAS TAKEN TO BOONE MEMORIAL HOSPITAL WHERE X-RAYS REVEALED A SMALL FRACTURE ON THE OUTER BONE OF HIS HAND.

EMPLOYEE WAS SHOVELING THE OVERLOADED SILO BELT WHEN HE TWISTED HIS BACK GETTING OFF OF THE BELT, EE WALKED TO THE TAIL WHERE HE FELL DOWN TO THE GROUND.

SETTING COOKERS. SLIPPED & FELL & HURT BACK.

EE WAS ASSISTING THE REMOVAL OF STRUCTURE TO GAIN PROCCESS TO A FALL THAT OCCURRED OFF OF THE #1 SECTION. WHILE LOADING THE TOP STRUCTURE. HE LOST HIS BALANCE AND FELL AGAINST A PIECE OF STRUC TURE. HE CONTINUED WORKING AND COMPLETED AN ACCIDENT REPORT ON THE END OF HIS SHIFT.

EE WAS THROWING SWITCH WHEN HE STEPPED BACK ON AN OLD PIECE OF RAIL, TRIPPING HIM. EE LANDED ON HIS LEFT FOREARM.

EMPLOYEE LOST FOOTING - FELL ON ROLLER HE WAS INSTALLING BOTTOM BELT ROLLERS.

EMPLOYEE WAS OUTSIDE THE PORTAL SPRAYING OFF TAILPIECE. HE SLIPPED ON ICE THAT HAD FORMED & STRUCK HIS LEFT ELBOW ON AN I-BEAM.
