EE was hitting a bearing with a sledge hammer and a shard of metal came off and entered right forearm.
Employee was working on bucket teeth and adapters on a shovel bucket. When attempting to remove the G.E.T. lock out the keeper pin fell out and pinched employee's hand between the keeper and a 2 inch plate. The plate was being used to press the wedge up using a ram and hydraulic pump. Injury required stitches to close the wound.
Hydraulic ram broke free from pressing pin out of bores on CAT 011R Dozer and struck employee on the head.
Removing wheel unit from 785C using a pry bar and crane on service truck. Hub came off spindle and struck employee on left hand causing employee to loose balance didn't fall but twisted employee's left hand and knee in the process.
Employee was removing an oil pump from a dozer with a chain hoist. As EE was holding it steady with EE's hand, the pump slipped from the chain hoist, pinching EE's finger. This caused a laceration that required 9 stitches to EE's ring finger

Employee stated while changing a drill bit in a hand held drill, while tightening the drill chuck the employee stated he felt something snap in his upper right arm. an accident report was filed and the employee continue to work. The Employee went to Med Express on 6-27-14 and put on restricted duty. employee Notified his employer of seeking medical attention on 7-1-14
Employee driving company vehicle - engine died, vehicle came to sudden stop and he bumped nose on steering wheel.
On April 7, 2014, this mine was notified that this employee was diagnosed with pneumoconiosis.
Employee was operating equipment, was in the process of adjusting the width of the forks on a JLG forklift. The employee attempted to push the fork from atop the vertical divider by using his foot. The employee placed his right hand in the crossbar support area to steady himself. As force was applied, the crossbar seated, smashing the employees thumb.
Employee was diagnosed with pneumoconiosis on 4/16/14. Company was notified of this event on 6/24/14.

Employee stepped out of the motor without getting the brake set at the #5 switch when the motor rolled over his right foot causing a distal fracture of his 3rd metatarsal. He was excused from work.
Employee was pulling cable when a loop in the cable caught his right foot, pulling him down and injuring his right knee and leg. He was seen in the ER later that evening and diagnosed with a strained muscle and small fracture of the lower right leg. He has been excused from work.
Employee was loading bolts and glue into a scoop bucket when he twisted with materials in hand and heard a "pop" and felt pain in his back. He continued to work his shift but the following day, he went to the ER and was excused from work.
Employee was assisting with the installation of a miner head when he stepped on a rock twisting his right ankle. He continued to work but went to the physician later that day and was excused from work. Diagnosed with a sprained ankle.
Employee was unloading a shuttle car tire from a supply car, while lifting the tire with a slate bar, he slipped and smashed his left index finger between the slate bar and a rock, lacerating his finger. He was transported to the ER and excused from work.
Employee has been diagnosed with a "sufficient degree on pneumoconiosis"(black lung) and is eligible for rights afforded under 30 CFR, Part 90.
Employee was applying rock dust and cleaning the area when he tripped over loose material causing him to fall and injure his left knee. He finished his shift but went to the ER that evening. He was diagnosed with a Left knee injury and calf strain and has been excused from work.

He went to the ER and was diagnosed with a knee sprain and released to return to work. Employee worked until 02/22/13; when he went for an MRI which revealed an ACL tear and he was excused from work. An unintentional roof fall was discovered in the Rattlesnake Headings, Entry #4, Spad # 646. The fall measures approximately 20ft L X 20ft W X 6ft H. No injuries resulted from this occurrence.
Employee removed a cable from a rope hanger and when he raised up and turned the hanger hooked him in the nose causing a laceration. He received 3 sutures.
Injured employee was installing a 10ft. cable bolt when he was struck by the drill steel causing a fracture to his left thumb.
Employee was installing a pin in the front spring of a tractor when the wrench/pry bar slipped and struck employees right wrist. Initial injury 01/02/13. On 01/07/13, employee picked up a charger and the discomfort in his wrist became worse, he continued to work until 06/18/13 employee underwent surgery and was excused from work.
Injured employee was using a pick hammer to raise up a cable when he accidentally grabbed the pick side of the hammer causing a puncture wound to his left hand between his thumb and index finger. He received 2 sutures and was released to return to work.
Injured employee was pulling on a beam when it became fouled and when it came loose it struck employees left knee. He was treated at the ER and released to return to work. Employee returned to work the next day but due to continued discomfort, employee went back to the ER on 11/23/12 and was excused from work.
An unintentional roof fall was discovered on the Southeast Mains section in the #6 Entry, inby spad 6975. The fall measured approximately 55ft W x 20ft L x 10ft H. No injuries resulted from this occurrence.
Injured employee was opening a metal storage container when he smashed his left middle finger between the handle and container causing a fracture to the distal tip of his left middle finger. He was seen and treated at the ER.
Injured employee was using a jack to lift a 9Man man-trip when the jack kicked out and struck the employee on the left foot. He was taken to the ER and diagnosed with a contusion of the left foot and ankle. He was excused from work by the ER physician.
Injured employee was using a ratchet to tighten a bolt when the ratchet slipped causing the employee to hit his hand against a flite. He was examined at the ER and diagnosed with a fracture to the tip of his left ring finger.
Injured employee was using a utility knife to trim rubber from a screen panel when he accidentally cut his left Thumb. He was taken to the ER and received 2 sutures. He was released to return to work.
Injured employee was climbing into the coarse ref screen when he began to experience sharp pain in his back and neck. He continued to work but went to the ER later that day. The ER physician excused him from work.
Injured employee and co-worker were carrying long piece of metal. Co-worker dropped his end, causing metal to slip from injured's hands resulting in laceration to right index/middle fingers.

An employee suffered fatal injuries after he fell down the hoist-well from the 3rd floor onto the basement floor of the preparation plant. The employee was on a ladder when it slipped or shifted causing the employee and ladder to fall over the handrails and down the hoist-well. Employee was transported to the ER by EMS.
Employee was pulling on chainfall that came loose pinning his right hand between the chain fall and sump. Employee was wearing metacarpal gloves resulting in only bruise and abrasion, but a prescription medication was given.
An unintentional roof fall was discovered in the # 6 Entry near spad #952. The fall measures approximately 70'L x 18'W x 6'H. No injuries resulted from this occurrence.
Employee was traveling down the slope belt entry on a rubber tire 4-wheeler when the chains broke on the suspended belt structure causing one side of the structure to fall. The structure struck the roll bar of the 4-wheeler and the employees left shoulder and head causing swelling and bruising. He was transported to ER by EMS.
Injured employee was walking when he struck his head against the shield canopy. Employee was seen at the ER and excused from work.
While lifting material at the stage loader, employee began to experience lower back pain. He completed his shift and was seen at the ER later that day. He has been excused from work.
Injured employee tossed a piece of material onto the belt when it struck a slate bar that was on the belt and the slate bar flew up and struck employee over the left eye causing a laceration. He was transported to the ER where he received 5 sutures and was released to return to work.
Injured employee was adjusting belt that had been loaded onto a flat car using channel locks. The channel locks slipped causing employee to fall backwards against the rib injuring his right hand & wrist. He continued to work but later went to the ER for X-Rays. Right wrist Hairline Fracture diagnoses by orthopedics on 02/03/12.
An unintentional roof fall was discovered in the intake, #5 entry, approximately 80 ft. outby spad #950. The fall measures approximately 40ft. long x 20ft. wide x 8ft. high. No injuries resulted from this incident.
Injured employee was setting a beam when he injured his left shoulder. While setting the prop, the bottom of the jack slipped causing the beam to fall striking him on the left shoulder. He refused EMS and finished his shift. He was then transported to the ER by a Safety staff member where he was diagnosed with a Left Shoulder Contusion and excused from work for 7 days.
