The employee was beginning to climb down a set of steps from a work platform when the platform tilted causing a loss of balance. As the individual attempted to secure himself, he fell on his back which ultimately resulted in pain to the back and neck.

He then proceeded to jump from the tail roller guard as opposed to falling from a distance of 4'7" to the concrete pad. The impact ultimately resulted in a fracture to the right heel.
