Employee was in the process of removing a hydraulic hand pump and jack from the top of the tub base and hung EE's foot on a hose and fell forward and EE's thumb on the left hand was caught between pump and ram which caused a fracture of the first joint of the left thumb.

IE tripped and fell on shoulder, after Orthopedic appointment on 4-21-26 was diagnosed with rotator cuff injury that will require surgery

Employee was walking and rolled EE's ankle. After an ORTHO visit on 10/09/2025, EE was restricted with no work for 4 weeks.

While walking towards crew van employee twisted and began to feel back pain. On 4-17-24 the employee was seen by an Orthopedic and prescribed physical therapy.

IE stated that EE stepped down from a haul truck ladder and felt discomfort in EE's right leg. On 10/18/2024 EE went to the Dr for swelling in EE's right ankle. EE was diagnosed with a strain and given a 1 week restriction with no treatment.

Employee tripped same level and caught self with right arm causing pain. An MRI reading at the ORTHO on October 20th revealed a tear to the bicep requiring surgery.

Employee stated twisted knee and began to feel pain. After an MRI and reading, it was determined that the employee will need surgical repair due to extensive chronic arthritis of the knee.

Employee tripped and fell on the same level while moving a cable crossing causing a laceration to the back of EE's scalp requiring stitches.

IE tripped on own feet while walking toward the shop. IE caught self on the bollard post that had a piece of metal leaned against it causing a laceration requiring stitches.

Standing on the adjustment springs for the lower curtain liner, EE went to step aside to adjust self when EE's foot slipped on the adjustment screw and fell to the deck of the inertia, landing on left leg and at that point went down onto the deck of the inertia holding knee.

Employee was Dismounting the Fuel and Lube truck when they slipped causing a fracture to their left wrist, requiring surgery.

Person slipped in the parking lot when exiting a personal vehicle before shift resulting in a fracture to EE's ankle.

Supervisor lost balance while observing conditions after a weather event resulting in a fractured leg.

Loose ballast exposed rail ties. Light ankle sprain.

Employee lost balance while stepping off of a step and attempted to catch self causing a fracture to the right arm.

EE was dismounting the dozer when they slipped causing lacerations requiring stitches.

Employee slipped while walking on the freshly bladed smooth ground twisting an ankle. An X-ray reviled a fracture to the fibula near the ankle socket.

Employee slipped and bumped head causing a small laceration on forehead requiring some sutures.

IE was walking on flat ground when they tripped and fell causing a fracture to right wrist as they caught themselves.

IE stated that EE was descending ladder, missed the last rung and slipped causing a laceration to EE's left hand requiring stitches.

Employee was walking to haul truck and slipped and fell on the freshly bladed ground causing a fracture to EE's left femur.

Employee was using a high pressure wash bay hose to clean mine equipment. The hose twisted causing the pressure to drop. The hose freed resulting in a surge of pressure causing the employee to be knocked off balance, the hard hat to fall off and EE struck the back of head on a low hanging piece of the equipment that was being cleaned. Resulted in a small head wound.

IE was descending a ladder, slipped and struck a guard on beltline with back, then fell to ground.

Employee was performing cleaning tasks and lost footing. EE caught self with EE's right arm and began to feel pain in EE's wrist. EE was diagnosed with a wrist fracture.

IE twisted EE's ankle when walking down stairs. EE was taken in for evaluation revealing a fracture. The landing area of the stairs was a clean, clear and flat concreted area.

While moving trailing cable, the IE slipped on the freshly bladed ground and felt a pop in the right arm. EE was taken to the ER for evaluation and referred to an Orthopedic Dr. The IE was seen by an ORTHO and scheduled for an MRI. The MRI reading revealed a tricep tear that required surgery to repair.

IE was dismounting a Cat 834 Rubber tired dozer, stumbled or tripped on feet, slipping causing a laceration to right knee and pain in left shoulder.

IE was dismounting the ladder on a D10N. When the IE stepped to the ground, foot slipped causing IE to pull on the hand rail. IE felt a pop in shoulder. On 6.25.18 the IE was seen by an ORTHO and it was determined that IE pulled the biceps tendon off the shoulder. IE was given a work as tolerated release until surgery on 7.19.18 at which point it became an LTA.

while preparing to unload diesel fuel i was pulling fuel hoses into place and fell in a pot hole causing me to fall and hit mouth on a piece pipe coming out of the tank trailer and broke one tooth in half. The area does not have lighting for after dark site.

Employee was power washing a piece of the mine owners equipment in an open air wash bay. The overspray was accumulating on the equipment and glazed the equipment almost like black ice and was very difficult to see. As the employee stepped down off the equipment employee stepped on the dozer tracks, slipped and fell approximately 1 foot on left elbow.

The IE twisted ankle while working on a haul truck. The individual felt pain in ankle but did not go in for medical treatment. The IE reported still having pain on the 14th at which point ee was scheduled with an orthopedic doctor. IE was seen on the 16th and an MRI was scheduled. On the 30th the employee's MRI was read and ee was diagnosed with neurapraxia.

Employee was walking on flat ground when employee slipped on some ice rolling an ankle resulting in fracture to the right ankle.

The IE slipped and fell to the ground when stepping out of the vehicle. The IE experienced pain in the right knee and was treated at first aid, then transported to the hospital for further evaluation. The IE was referred to an orthopedic doctor. An MRI was ordered and was read on 12-19. The IE was diagnosed with a meniscus tear and will undergo orthoscopic surgery.

IE felt a "pop" while entering the crew van at the end of shift. IE was examined at the ER, referred to an Orthopedic doctor, MRI was ordered which revealed a small meniscus tear and was prescribed physical therapy. The IE had surgery on the same knee for a meniscus tear in the past. Given this along with the ee denying any slip or trip the event will be classified as an illness

EE was walking down a stairway when EE slipped and fell resulting in a deep bruise to EE's back. EE was not released to return to work due to pain management.

Employee was walking across a frozen rutted area when he tripped and fell on his right shoulder resulting in a simple fracture to the upper humerus bone.

Employee was walking down steps of shovel ladder, falling causing a hairline fracture to his left ankle.

Employee stepped into a depression hyperextending his knee. He was referred to a specialist and on 7/23/2015 was diagnosed with a torn meniscus.

Contract employee rolled his right ankle causing his right knee to pop. The employee was issued a restricted duty release making this a reportable injury.

An employee rolled his ankle resulting in 2 broken bones in his ankle and a fractured bone in his little finger when he tried to catch himself.

The employees foot slipped off the first step while boarding a haul truck.

EE climbing in engine bay on left side going back by the drive alternator. EE slipped and caught himself with right hand spraining right wrist.

Employee parked Rail Link truck on East end of Rail Link office and exited truck from drivers side. Walked around back of truck, slipped on ice covered with snow, striking her left elbow. Contributing factor - ice covered by snow. Employee was not wearing ice cleats. Injury - contusion to left elbow.

While exiting a crew van, the employee misstepped and twisted his left ankle. The employee was diagnosed with an ankle sprain and was issued a restricted duty release.

The IE's hand slipped off the iron door handle while attempting to close the crane doors at C3 Transfer Building, causing him to fall backwards. The IE struck the right ear on a guard covering the drive motor and was taken to Campbell County Memorial Hospital Emergency Room where sutures were applied to close the lacerations.

Employee slipped off of a loader bucket resulting in a stable simple fracture to his pelvis.

Employee stumbled and caught himself with his left leg feeling a pop and pain in his left knee resulting in a lateral meniscus tear.

After dismounting a service truck, the employee turned in motion with his feet on the cement floor rolling the right ankle.

IE was rotating a tire manually on a 854K rubber tired dozed while standing on the push arm block when he slipped and caught himself. IE began to feel pain in his shoulder. 2-13-2014. IE was seen at the orthopedic specialist where it was determined that there was a severe rotator cuff muscle tear that will require surgery to repair making this a lost time injury.

Employee was dismounting a water truck when he stepped on a rock twisting his ankle.
