Mostrando los 17He said he was sandblasting steel and began to fall from a ladder that he was using. He said he went to grab a piece of the siding that was rusted and not bolted. He said he cut his left wrist when he reached for the steel and fell on his left shoulder on the ground. He said he did not want to see a doctor initially. He said he thought it would work itself out.
Employee was unloading supplies with a battery scoop. As he was getting out of scoop he fell backwards. Employee has been placed on light duty, he has a strain to the lower back.
Employee was primining pond pump on 06-10-07, when finished employee stepped back and foot slipped in mud causing employee to fall spraining left knee. Employee had continued to work until 3-24-08, when employee had surgery on left knee. This is an injury that was filed on 6-10-07, it became lost time on 03-24-08.
Employee was making belt examinations when she stepped in some muck and twisted her left ankle. She went to the doctor and was placed on restricted duty, due to a sprained ankle.
EE WAS CLEANING UP AROUND THE #1 DRIVE WHEN HE STEPPED IN A HOLE TWISTING HIS RIGHT KNEE. HE WENT TO THE DOCTOR AND RETURNED TO WORK THE NEXT SCHEDULED SHIFT BUT WENT FOR A FOLLOW UP VISIT ON 01/13/2003 DUE TO SWELLING AND PAIN THE DOCTOR DID NOT ALLOW HIM TO RETURN TO WORK. BECAME REPORTABLE 01/13/2003.
EE WAS WORKING ON THE TUBING IN THE RAW COAL VIBRATOR WHEN HE SLIPPED AND HIS RIB AREA STRUCK A PIECE OF METAL, HE RECIEVED SIMPLE FRACTURE TO 3 RIBS. HE RETURNED TO WORK HIS NEXT SCHEDULED SH IFT.
EE WAS WALKING BY THE BELT TAIL PIECE, HE FELL HIS LEFT KNEE STRUCK THE TAIL PIECE. THIS OCCURRED ON 10-24-01 AND NEVER GOT WELL. HE TWISTED IT AGAIN ON 11-1-02 AND WENT BACK TO DR TWICE AND T HE DR TOOK HIM OFF WORK AS OF 11-11-02. THAT IS WHEN IT BECAME REPORTABLE.
THE EMPLOYEE WAS HELPING MAKE A BELT SET UP. HE WAS CARRYING ROLLERS WHEN HE SLIPPED IN MUD AND TWISTED HIS RIGHT KNEE AND ANKLE.
EE WAS GETTING OFF HIS 20 TON DIESEL MOTR, HE TRIPPED ON A STRAP AND FELL, BREAKING HIS RIGHT ARM NEAR THE ELBOW.
THE EMPLOYEE TRIPPED ON A DA RAMJACK. HE FELL AND HIS RIGHT HAND STRUCK A SHIELD PONTOON AND BROKE BONE IN HIS RIGHT HAND.
EE WAS WALKING IN THE PLANT, HE STEPPED IN A WET AREA AND FELL. HE DID NOT GO TO THE DOCTOR UNTIL 11/27/01. HAS BEEN OFF WORK SINCE. BECAME REPORTABLE 11/27/01.
WHILE HOISTING SHEETING MATERIAL, THE CLAMP SECURED TO THE MATERIAL FAILED CAUSING THE INJURED TO SLIP AND FALL TO THE GROUND.
EE STEPPED OFF OF A SHIELD PONTOON INTO A HOLE. TWISTED HIS LEFT ANKLE. X-RAYS WERE TAKEN. DID NOT SHOW ANY BROKEN BONES. SPRAINED ANKLE.
WHILE STEPPING THROUGH A MANDOOR HE TRIPPED. THETOP OF HIS HARD HAT STRUCK A BRATTICE CAUSING THE HAT TO HIT THE BRIDGE OF HIS NOSE. X-RAYS SHOWTHE NOSE TO BE BOKEN. NO LOST TIME.
EE STEPPED ON UNEVEN MINE FLOOR CAUSING HIS FOOT TO HURT MORE THUR USUAL EE HAS HAD A PROBLEM WITH TENDONS IN HIS FOOT FOR MORE THAN 2 YEARS. HE MISSED WORK ON 3-9-01 RETURNED TO FULL DUTY 3-1 0-01. THEN WENT BACK TO THE GO TO BACK OF SHEET
EE WAS GETTING OUT OF A 20 TON BROOKVILLE LOCOMOTIVE, HE STEPPED ON SOME LOOSE MATERIAL AND TWISTED HIS KNEE.
