Employee was working on 270 excavator changing bush hog attachment back to bucket. Employee picked up pin to install in back of bucket when he felt pain in his left shoulder. Employee became a lost time accident on 9/12/2014 due to his injury.

Employee was changing out belt roller when he strained his back on 7/26/23, employee was placed on light duty on 8/15/2013.

Employee was lowering a 5 gal bucket of stone patching agent down into belly pan when he pulled his shoulder, employee continue to work, on 9/16/2013 employee was placed on company furlough, on 9/26/2013 while on furlough employee had surgery for partial tear to left shoulder due to this injury.

Employee was moving grate when tripped over grate on to left wrist, employee became a lost time injury due to this accident.

Employee pushed mud off of splash guard with a shovel when he felt pain in left lower back employee has been placed on restricted duty on 10/09/2012.

He said he was sandblasting steel and began to fall from a ladder that he was using. He said he went to grab a piece of the siding that was rusted and not bolted. He said he cut his left wrist when he reached for the steel and fell on his left shoulder on the ground. He said he did not want to see a doctor initially. He said he thought it would work itself out.

Employee was cutting skirt rubber on refuse belt with pocket knife when knife slipped cutting top of left hand, sutures required, employee has returned to regular duty on 02/04/2010 with no restrictions.

Assisting with moving swing-a-way on crane. (packing up a crane to move to another locaton)

While pillaring in #1 entry, 15th row at the left wall, a roof fall has occurred on the Continous Joy Miner. The size of the fall was approx. 19'wide x 8'thick, x 35'long. No ventilation has been effected or miners hurt. Rope bolts, beams, and jacks have been set and additional roof support of 5'and 10' rope bolts will be in installed.

While mining #4 pillar at brk. 24 a roof fall occured on the continous Joy miner, the size of the fall was approx. 11.5'W x 10H x 8' thick. 48" resin bolts and 7' timbers set ,tests holes & straps according to plan. No ventilation affected or miners hurt.

A rock fall was found approx. 12:40 pm in the #1 main return, in #1 entry between brk. 48-49 in the secondary escapeway, the fall was approx. 100' long & 8' thick, the area has been dangered & timbered off, the secondary escapeway has been re-routed to the #3 entry, #4 return, brk. 3. A meeting was held with all employee's on changed with the secondary escapeway.

Pillar #2 into 3rd cut between #11 and #12 crosscut, a piece of rock fell on the Joy continous miner. No ventilation was affected, the fall was approx. 4'x10'x15' to heavy for the miner to back up with, the rock was pulled off the miner with a retreaver jack.

12" waterline broke impeded the escapeway, miners inby were withdrawn until the primary escapeway has been pumped of water.

A roof fall was found in the #3 track entry, the fall extended into #1 and #2 entry. The fall was approx. 6'x140'x60' the affected area in 3 entry has been cleaned up, cribs and arches were built, no equipment involved or ventilation effected.

Employee was changing a floating axle out on shuttle car when the chain he placed on axle tightened catching 3rd digit finger tip on left hand cutting tip off. Employee has returned to work on regular duty.

During maintenance shift a rockfall was found while checking the fresh waterline in the #4 main return, #3entry, @ #5 xcut. Fall is appr. 20'x30'x6-8'thick, no ventilation affected. Affected area will be timbered & dangered off. The fresh waterline will be rerouted away from affected area. By the time the supervisor was contacted & got outside to call MSHA it was 05:20

Drill steel was hung up, employee tried to pull it out by hand and twisted left wrist.

While operating rail buggy, driveshaft broke loose and caused debris to hit right eye, scratching eye. Employee has returned to work with no restrictions.

A roof fall was found in the Perfect Submains, in # 1 entry of the 2nd panel, this fall is approximately 20'x6'x60. Ventilation was not affected no equipment or persons involved.

Employee was using lifting jack on a de-railed mantrip. While letting lifting jack down, bar sprung up hitting employee above left eye, sutures required. Safety talk conducted on safety measures used with lifting jacks. Employee has returned to reg. duty.

Employee complaining of numbness in both hands on 3-7-08. Continued to work. On 5-17-08 he was diagnosed with carpal tunnel in both hands and continued working. On 7-11-08 employee became lost time due to carpal tunnel surgery on both hands.

Employee was helping supply man put 20 ton motor that derailed back on track, slate bar being used with railroad jack slipped and hit 3rd. digit finger on right hand, sutures were required, employee returned to his regular duty.

Employee was working on putting broken belt back together, when he picked up end of belt and pushed forward he felt pain in shoulders. Didn't become lost time till 4/14/08.

A roof fall was found during a weekly exam. Located in the neutral, in #2 entry Brk. 16 on #10 belt, the size of the fall was approx. 20' x 30' x 5' thick. No equip. was involved and ventilation has not been effected. The area has been cribbed and dangered off.

Employee was building a stopping when a piece of drawrock fell hitting him on his head and his left shoulder. Employee has a swollen clavicle and has been placed on restricted duty.

Employee was drilling hole in top with drill steel when a piece of rock fell cutting his left forearm, 6 sutures were required, employee has returned to work with no restrictions.

A roof fall was found in the old 019 section, behind #1 bleeder, the last open break at the face. The size of the fall was 125" long, 8'high,and 17' thick. The area has been danger off and additional roof support will be placed.

Employee was pulling miner cable slack, when he strained his lower back, this accident became reportable on 10-15-07, employee was put on restricted duty from 10-15-07 to 10-22-07, employee has return to regular duty with no restriction.

Employee was unloading supplies with a battery scoop. As he was getting out of scoop he fell backwards. Employee has been placed on light duty, he has a strain to the lower back.

Several pieces of 4' waterline coupled together were being moved when a piece broke apart at the coupling hitting employee in the left leg; he received a fracture to the left fibula.

Employee was moving waterline with scoop, caught waterline with scoop, causing waterline to come up on top of the scoop. Water line hit the top of his head on the left side , sutures were required.

Employee strained his back on his lower right side, employee continue to work. On 9-6-07 employee became restricted duty due to injury.

A roof fall was found in the #5 entry from crosscut 13-15, the fall is approx. 120' long, the fall is approx. 3' to 7' thick, the fall area was cribbed and dangered off. Additional roof support has been put up in entry 5 & 6 out-by the fall, 8 ft. tension bolts were installed.

Employee was leaned over in a strain bolting top in #2 heading, when back started hurting.

Employee was pulling up cable slack from roof bolter, bottom was muddy at the time, employee strained his lower back.

Employee was primining pond pump on 06-10-07, when finished employee stepped back and foot slipped in mud causing employee to fall spraining left knee. Employee had continued to work until 3-24-08, when employee had surgery on left knee. This is an injury that was filed on 6-10-07, it became lost time on 03-24-08.

Employee strained his back on his lower right side, employee continue to work. On 8-01-07 employee ceased work complaining of pain in lower back on right side. Injury became reportable on 8-2-07.

He was pulling a belt pin that was hung, he struck a hammer with another hammer and a piece of metal broke off and lodged in his right thumb. It was imbedded and was removed at a hospital. He is on restricted duty.

The injured employee was struck by a piece of drawrock. He was reaching for a roof bolt when he was stuck on his right arm. His arm was bruised and he was placed on restricted duty for seven days.

The employee was pulling a belt pin out when a piece of metal became lodged in his right thumb. He went to the doctor and had it removed. He retuned to regular job his next scheduled shift.

Employee was working at the power center, he dragged his hand across a piece of metal that had a sharp piece of metal on it. His left ring finger was cut on it. Sutures were required and he returned to his regular job his next scheduled shift.

While putting his starter barrel up a piece of drawrock fell striking him on his left arm. Sutures were required he returned to his regular job.

Employee brought in a statement from NIOSH stating that he has pneumoconiosis. This statement was brought to the company on 08/30/2006.

The employee was working on the dozer and struck the muffler with his arm. He had sutures placed in the cut. He returned to work his next scheduled shift to his regular job.

A roof fall was found on the #2 belt. The fall was located in break 22 on #2. Two rows of bolts were pulled out. The fall was cleaned up and the roof supported. Additional support was added inby and out by the fall area.

Employee was making a belt splice, knife slipped and cut his leg.

Employee was turning a right break when a rib rolled off striking him on the left foot. He continued to work and on 6/20 he went back to the doctor and a simple fracture was found in a bone in his left foot. At that time he was taken off work and the accident became reportable.

Employee was operating a bolter when his glove got caught by the drill steel; his little finger on his left hand was dislocated and also had sutures placed in it. He returned to work on light duty.

A roof fall was found in the #6 return break #9 intersection. The fall was found during a weekly examination. The fall is approx. 20' wide X 20' long X 3' thick.

He was removing a belt pin. He struck it with a hammer and a piece of metal struck him on his right thumb. He went to the Doctor and had it removed. He returned to his regular job.
