When the cable was pulled down it caused the employee to fall to the ground injuring his left foot.

EMPLOYEE WAS POSITIONING A PUMP IN THE WATER AND LOST HIS FOOTING DUE TO SLIPPERY CONDITIONS. HE REACHED TO GRAB FOR THE RIB TO KEEP FROM FALLING AND WHILE DOING SO, HE TWISTED HIS LOWER BACK.
EE JUMPED OFF ROOF BOLTER INJURING LOWER BACK.
EE TRIPPED OVER A ROCK ON MINE FLOOR CAUSING HIM TO FALL AND STRIKE HIS KNEE ON ANOTHER ROCK.
