Employee was servicing a front end loader and was walking away from loader tripped and fell to the ground catching themselves with right hand to break fall.

Employee was putting antifreeze in rock truck. When getting down, feet got caught. To keep from falling, employee grabbed railing and pulled right shoulder.

Employee tripped over steel cable in the office parking lot injuring EE's right back and hip.

Employee was performing equipment pre-shift when lost his footing and fell from grader striking his head on the back of blade.

Employee was working on a Front end Loader and twisted right knee when EE dismounted the machine.

Employee was using a pry bar to loosen coal that bridged in the crusher. He slipped and fell twisting his Right Knee.

Employee was walking up berm to get on bulldozer and slipped bending his hand back causing pain.

Tripped coming across rock pile. Tried to catch self from falling and dislocated middle finger and scratched arm.

Employee was loading wethole product when he tripped on a box and landed on his knee.

EE was cleaning off side mirrors when he slipped and grabbed the handrail of the truck causing pain in his left shoulder and arm.

EE climbed upon the push arm of D-11 Dozer then onto track while reaching for the handrail on hood he slipped and fell to the ground.

Employee was dismounting Caterpillar 785 haul truck & missed step on ladder. Employee grabbed handrail with right hand causing pain in right shoulder & down right side & back.

Walking to water pump. The area was muddy, EE slipped and twisted right knee.

Employee was climbing down ladder of rock truck when he slipped and fell to ground.

Employee was getting a 1 gal bottle of windshield washer fluid from foreman when he slipped and fell landing on the track of the d-9 dozer he was operating. He started loosing time on 6/8/09

Operator was dismounting rock truck and twisted his right foot(ankle).

Employee was climbing down steps of Cat 777 rcck truck, he had tools in his hand. He missed the last step and fell to the ground landing on his right hip and back.

Employee was climbing onto D10N dozer. His foot slipped on the track causing him to fall-back striking pusharm of dozer. He complained of tenderness and swelling in lower back area.

Opr jumped off excavater truck instead of using hand rails to get down. EE sprained his right ankle.

Driver was sweeping out cab of truck, not pay any attention on what he was doing and walked backward off the catwalk of the truck.

EE said that fe fell from step ladder.

Employee was exiting 992G loader. Foot slipped on top step of ladder and he fell on his back down the steps and onto the counterweight.

Operator was standing on machine checking coolant. He lost hand grip; to avoid falling he jumped causing injury.

Employee was getting out of his truck. When he turned to step out on fuel tank he had a bad pain in his lower back.

Employee was getting off the back of fuel truck & his foot slipped, pulling his left knee & twisting it.

Employee was getting off coal loader in coal pit to check coal. Ground was uneven. He twisted his left ankle thus breaking a small bone in the top of his left foot (thought he just sprained his left ankle when he saw the doctor later in the day).

EE WAS CLIMBING THE LADDER OF A DM45 IR DRILL TOPREPARE DRILL FOR START UP. HE WAS IN A HURRY & LOST HIS FOOTING, FALLING BACKWARDS TO THE GROUND LADING ON RIGHT ARM.RESULTED IN A KNOT SWELLIN G UP JUST BELOW RIGHT ELBOW

EE WAS OPERATING A 777C ROCK TRUCK. HE GOT OUT OF TRUCK & WALKED TO THE BACK OF TRUCK TO CHECK SUSPENSION CYLINDER. THE GROUND WAS SNOW COVERED & FROZEN, HIS FEET SLIPPED & HE FELL ON HIS RIGH T SHOULDER.

CLIMBING DOWN FROM BACK OF ANFO TRUCK. HIS FOOT SLIPPED ON LADDER AND GOT CAUGHT ON RUNG. HE CONTINUED FALLING STRIKING BACK OF HEAD AND TWISTING KNEE.

STEPING OFF OF DOZER LOST FOOTING AND LANDED ON KNEE ONT HE PUSH ARM OF DOZER.

THE 135 DEMAG WAS LOADING 777C CAT ROCK TRUCK WHEN BOLTS SECURING CAB OF MACHINE BROKE LETTING CAB TIP FORWARD OFF THE FRAME OF MACHINE.

CLIMBING OUT OF D9 DOZER STEPPED ON PUSH BEAM FOOT SLIPPED, FELL & HIT BEAM ON RIBS, FELL TO GROUND ON BACK

EE WAS STANDING ON THE BACK OF THE SERVICE TRUCKTAILGATE & CLOSING THE TRUCK CARGO DOORS. HIS FEET FLEW OUT FROM UNDER HIM & HE FELL ONTO THE FROZEN GROUND & HIT HIS LEFT ANKLE.
