Employee stated that he parked his ride in front of the power box in the #7 entry. When he stepped off his ride, his foot hung under a metal band. This caused him to fall to the ground, injuring his ribs

Employee stated he was walking from the Main North to the slope bottom. As he was getting ready to walk through the 2nd airlock door located at crosscut # 3 on the Main North he tripped, and hit his shoulder on the door. Employee worked until 10/02/2014. The first day he missed was 10/03/2014.

Employee fell on a conveyor belt and suffered injuries to his neck & right shoulder.

Employee stated that while walking into mine, down the slope catwalk, he slipped on a wet spot and hyper-extended his LT knee.

Employee stated that while operating a diesel lube ride the brakes locked up and caused him to fall off ride onto the mine floor.

Bending roof bolt in hole to straighten, bolt spun in hole causing him to fall. Pain in left shoulder, arm and hand.

Employee stated that he was picking up two buckets of miner bits, turned to go to scoop, and stepped in hole. Twisted back and hip.

Employee was washing concrete bottom on the slope with a wash down hose. His feet became entangled in the hose and he tripped and fell to his knees ,jarring his lower back.

EE was cleaning bath house. He attempted to step over a bench while pulling a water hose in the change room. He hung his toe on the bench and felt a pain in his knee. He was diagnosed with a twisted knee. This accident became reportable on 02/21/2014 when the employee started missing work to have surgery.

Employee stated that he stepped into a hole in mine floor and his knee hurt.

Employee was walking through cross-cut when he fell down and fractured his left ankle.

Employee was on top of a continuous miner adjusting the water sprays. As he dismounted, he slipped and landed on his left foot, twisting his knee. The accident became reportable on 3/12/14 when the employee started missing work.

Employee was dismounting the Manitou fork lift. As he descended the steps, his right foot slipped causing him to pull his lower back.

Employee stated he tripped on mound of gob and injured his right foot.

Stated: Employee was helping move miner. He was standing on a pile of gob when his footing gave way. He felt a sharp pain in his right calf. He finished the shift and sought medical treatment the next day. He was diagnosed with a strained calf and did not report to work his next shift.

Stated: Employee was walking from shower to locker at the end of his shift. He slipped on the wet floor and twisted his left knee. Sought med. treatment and released to RTW full duty. Results from MRI received on 04/09/2013 indicated degenerative changes and a torn meniscus.

Employee was walking in the supply road when he stepped on a mound of fire clay twisting his left ankle.

Employee stated that he tripped over a metal band that is used to bundle concrete blocks together. This caused him to fall down and land on a pile of concrete blocks.

Stated: Employee was walking toward rear of bolter when he tripped over a rock. He caught himself with his hand against the bolter. He was taken to the ER where x-rays revealed two broken metacarpal bones in his right hand.

Stated: Employee was driving scoop to charging station. He hit the panic bar and had to exit the scoop to reset breaker. He reset breaker and fell from the scoop injuring his foot and back. He did not go to the doctor until 10/8/2012. He was placed off work pending further tests.

Stated: Employee was loading pin plates in scoop and got foot tangled in plastic. He tripped and fell on the pin plates, cutting his arm. He received stitches and remains off work.

Stated: Employee was attempting to climb onto FCT when his foot slipped. He came back down and landed on his left leg, twisting his ankle. He continued to work until 3/20/2012 when he sought medical treatment. He was diagnosed with a fracture of the left lateral malleolus and chronic degenerative changes. He was placed in a walking boot. First day missed: 03/21/2012.

Stated: Employee was walking between road grader and rib to access cab of grader. Waterline was hung at 5' off mine floor at this location. Employee hit head on waterline coupling, slipped and rolled her ankle. Later diagnosed with fracture.

Stated: employee had just tightened a jack pipe on a tail piece. As he stepped down, he twisted his left knee. Due to waiting on MRI results, 14 days, and movement of safety personnel, this report was inadvertently delayed.

Stated: Employee was swinging boom in to install next pin when he slipped and fell. He caught himself with his right arm and overextended his wrist. Initially diagnosed with fracture. Cast removed on 10/06 by orthopaedic doctor, diagnosed with no fracture. Still off work with pulled tendon/ligament.

Sub. under protest to abate cit. #8504175. EE alleges that he was ent. though mandoor on out-by airlock doors when he caught his foot on top of mandoor frame causing him to fall striking his shin and falling on his torso pushing tape measure into his ribs. The op. is curr. invest.the EE's claim and reserves the right to amend and/or retract this report as info. becomes known to op.

Stated: Employee was hanging cable across the 3rd left XC when he slipped and twisted his left ankle. The mine floor had a 4" displacement with loose material at this location. The bolter cable was stretched tight with no slack as he was trying to push the cable up and over to the roof bolt plates to hang it.

Employee stated: while putting belt dogs on the belt, his left toe caught on a rock lying on the bottom. This caused him to fall on his right arm. He was working in the splicing hole at the 3B drive. His initial Dr visit indicated a possible fracture but further diagnosis is contusion/sprain.

Stated: After falling on ice felt pain in his back.

EE stated: He was shoveling in the area near the 3B header/3A tailpiece. He attempted to use his shovel to pick up 1/2 of a kettlebottom that he estimated at a weight of 10 lbs. He said that the floor area was wet and he slipped and fell onto one knee. This caused pain in his lower back on the right side. He stated that he picked the rock up by hand afterwards.

Went in early on this day. Walking across the section to see what areas needed cleaning. Walked in the crosscut between #2 & #3 entries, he stepped across a rut in the floor made by a scoop tire and hyper-extended right knee. Continued working and did not seek medical attention until 3/26/10 on scheduled day off. Saw dr. due to pain, 1st day missed was 3/29/10.

EE stated that he was roof bolting in #9 entry where some loose shale was on the ground. He was installing the second row of bolts in this working place. He was walking towards the rear of the bolter when he stepped into a hole in the ground. EE cont. working until 3/5/10. EE stated he went to Dr. on 3/5/10 and was diag. with knee strain. He began missing 3/5/10.

Stated: He had walked to his personal vehicle in the parking lot to get in after the end of his shift. As he started to get into his vehicle, his foot slipped on snow/ice and he landed on his right leg/hip. He has been diagnosed with lumbar and right hip contusions. He started missing scheduled work days on 2/17/10.

EE stated that he was walking down the slope belt deck in order to examine the slope belt while carrying his lunch bucket. He said he stepped on some black ice in the walkway at about 7:00 down from the top of the slope causing his feet to slide out from beneath him, and he landed on his hips and lower back, Missing scheduled days on 2/9/10.

EE stated that he had been shoveling coal around 4 C belt drive.EE said he walked towards the backside of the header when his foot slipped. EE said he fell forward and felt pain in his right shoulder after he caught himself on the ground. EE completed his shift and called the mine the next day to report the incident. EE stated that he had a shoulder sprain.

EE reported: Stopped in the battery charge station to change batteries. As he was walking around in the station, tripped over the battery charger cable, alleges that he twisted his right knee.

EE reported: While attempting to move up next row of bolt location,he stepped in a hole twisting his right knee.Contacted the mine following morning reporting he would not be at work due to his knee being sore and was having trouble lifting it up.

EE reported: Attempting to pull slack cable off a Flecther roof bolter machine's reel, ee lost her balance and fell backwards landing on left foot. The foot began to swell a couple days later, ee sought medical treatment and was determined to have a hairline fracture to a bone in the foot.

EE reported after removing nylon straps form low boy, walked over to slope shaft wall and hung on storage rail, turned to walk away, foot become entangled and fell on left elbow.

EE reported, at start of shift helping coworker remove supply trailer from lowboy. When walking along the mine bottom area stumbled over a piece of rock lying on the mine floor, twisted his left ankle

EE was walking along the #4 unit FCT low framing when he stepped on a piece of slick draw slate. He slipped and twisted his left knee. He continued working, notified the mine on 1/5 he would see his doc on about 1/8 to have his knee checked out to continued knee pain. He had surgery on 3/5 for a menisci tear of his knee. His 1st scheduled work day missed was 3/5/10.

Reported: As he was attempting to install a nail in the mine roof using a hiiti gun, the roof popped, he jumped sideways resulting in pain to his left ankle.

Employee reported: Walking along mine entry, dragging piece of ventilation curtain. As he was traveling thru a muddy area, slipped on wet mine floor twisting his left ankle. Reported event at end of shift. Later during the night ankle began to swell, sought medical treatment the following day.

EE stated that he stepped on the edge of a hole in the mine floor (approx 12" diam. by 3" to 4" deep) while walking through a curtain. This caused his right ankle to twist outward, resulting in a sprain. He will be off work for a few days and his first scheduled workday missed is 5/8/09.

Employee reported he tripped over miner cable, somewhat stumbled and caught himself on ground. Investigation revealed that due to pre-existing medical conditions (4 maintenance prescriptions) he has pain in left wrist, left 3rd finger, L upper scapula, low back & left hip with numbess in groin. He missed his first schdeuledwork day on 4/27/09. The mine is charging this as Illness.

Employee reported: Operating continuous miner in face area, feet became entangled in the machine's cable. He stumbled and fell across a pile of slack resulting in a twist to right knee. Worked the entire shift, called the mine the following morning, reported his knee was swollen and painful & was to seek medical treatment. Diagnosis was a sprain to his knee.

Employee reported: When walking across section, stumbled and fell onto his left shoulder. Pain and discomfort continued, 6 weeks after event received medical treatment. In late June, MRI was taken, determined he a torn rotator cuff in the left shoulder. On 7-23-2009 surgery was performed to repair the tear. Became a lost time reportable injury on that date.

Employee reported: Prior to the start of his shift, walking in the bathouse and his knee began to hurt. Pain continued, reported the pain to his supervisor at the end of his shift the following morning. Due to no event occurring, the mine is charging this event as a work related illness.

Stated: He had his knee (w/knee pads) on top of a Ram Car towards the tram motors in order to check permissibility on the tram motors. His knees slipped forward slightly and he put his left arm down to prevent falling forward. He experienced pain in his left arm and shoulder. His regular doctor has him off work to determine source of pain.

Walking across the section. He heard a ram car traveling down the entry, he attempted to hurry into a crosscut. He stepped on a "wind-roll" of fireclay. This caused his foot to turn twisting his right ankle resulting in a simple fracture to a small bone in the ankle.
