EE was using cable spooler to reclaim matrix coaxial cable. He was leveling the cable as it was being spooled on the reel. When the cable reached the coupling it pulled the cable out of his hand and through the reel striking him on the right hand, fracturing his small finger.

The hoist was inoperable from 12:00 am to 12:35 am with and electrical problem. The "roll-back" sensor would not function.

Unplanned roof fall - seal set #45 1st west main involving intersection of the #4 seal/ No ventilation was affected. The are of the fall has been dangered off. Supplemental support has been added to the area to assure safe walk way to the #4 seal. The #4 seal has been examined and no conditions are present that would compromise the integrity of the seal.

Employee stated that he parked his ride in front of the power box in the #7 entry. When he stepped off his ride, his foot hung under a metal band. This caused him to fall to the ground, injuring his ribs

Employee stated he was installing a roof bolt in the face area. He did not know that the rotation hose on the bolter boom had hung on the canopy post. When he pushed the bolt up the hose dislodged and struck him in the chin.

Employee was operating a 2 man diesel, His vision became blurred, before he could stop he bumped into a supply trailer in the roadway.

Employee was operating a Fletcher roofbolter and back into an idle miner sitting in the belt entry, pinching his right forearm. He received stitches to close the wound.

MSHA alleges that on 10/22/14 a fire occurred at the coal stockpile that was not reported. Highland disputes that a fire occurred, & that any reportable accident occurred that would invoke a reporting. The report is being submitted under protest solely to abate citation 8517781.

Employee and others were disassembling K&K structure. The bottom roller had been removed from one side of the stand by another employee, who flipped the roller striking him on the right cheek causing an inch laceration that required 5 sutures to close.

Employee was trying to free a drill steel when he caught his left hand and suffered an injury. Stitches were required to close the laceration.

Employee stated that he was loading belt rollers onto the forks of a forklift. As he was loading one of the rollers he failed to lift it high enough to clear the end of the forks and injured his finger. He went to the doctor and was treated for a bruise.

Employee stated that he was helping the roof bolter operators load supplies on the bolting machine. When he threw a bundle of glue across the scoop he injured his shoulder.

Employee stated he was walking from the Main North to the slope bottom. As he was getting ready to walk through the 2nd airlock door located at crosscut # 3 on the Main North he tripped, and hit his shoulder on the door. Employee worked until 10/02/2014. The first day he missed was 10/03/2014.

Employee stated that he was prying down loose rock. His feet became entangled in some draw rock. Causing him to fall forward injuring his shoulder.

Employee fell on a conveyor belt and suffered injuries to his neck & right shoulder.

Employee states that he was installing water line and as he stepped onto a 3 ft step ladder his LT knee hurt.

The Hydraulic braking system on the hoist was inoperable.

Employee states was handling a bundle of glue while loading a roof bolting machine. then stated that his back hurt.

Employee states that he was repairing L brackets on an FCT with an 8lb sledge hammer and after completing the job his lower back hurt.

The hoist was inoperable due to drum brakes not releasing.

Employee stated that he was walking through a man door located at cross cut 3 on the Main North. He did not bend over enough and struck his head on the top of the door frame. Employee started missing on 09/14/2014

Employee stated that he was operating a roof bolter on his right knee and when went to stand up his left foot slipped on a piece of draw slate. This caused him to fall downward striking his forehead on the roof bolter. EE received 9 sutures.

Employee stated that he was stepping down off of the FCT low framing onto the mine floor and he felt and heard a popping noise in his left knee.

Employee stated that while walking into mine, down the slope catwalk, he slipped on a wet spot and hyper-extended his LT knee.

The hoist was inoperable due to electrical problems.

This mine has experienced an unplanned roof fall in the Main North Return. Location was at cross cut #-7 in the #2 Entry. (202+70) The fall was approx. 20ft long X 20ft wide and approx 15ft high. there were no injuries, witnesses or disruption to ventilation.

Employee stated that while operating a diesel lube ride the brakes locked up and caused him to fall off ride onto the mine floor.

Employee states that he was operating a continuous miner when a piece of rock fell from between a roof bolt and the coal rib. The rock struck employees RT foot causing a fracture.

Employee stated that he was walking in the #8 entry on the #4 section when a piece of rock fell from the mine roof and struck his hard hat.

Employee states that he was operating a battery scoop on supply when a rock fell from between roof bolts in the mine roof. The rock hit the scoop canopy and struck the inside of his RT knee.

Employee stated that he was installing a cable stand off on a continuous miner. While installing a bolt the ratchet, 1/2 inch drive with a 30 mm socket, malfunctioned and caused him to stumble forward. His lower back was sore the next day.

Unloading a piece of cold roll steel 4" by 7' when it shifted catching the left middle finger between the steel and a rail on a pick up truck.

Employee was cleaning on a beltline with a shovel when his back started hurting.

Employee was hanging curtain in #9 entry. The continuous miner was loading gob out of the intersection. A plate picked up by the continuous miner head struck the employee in the left thigh causing a incision that required 3 sutures to close.

Employee states that the roof bolting machine's power cable was pulled down from mine roof and struck employee on shoulder and neck.

EE was operating a walk-through truss bolter on #3 unit. He was standing on the catwalk of the bolter. He had right arm resting on the tram lever guard when a piece of rock, 14" x 14" x 2", fell and struck him on his right forearm.

EE was operating a continuous miner. He bent over to move a loop of cable that was in the travelway and felt a pain in his lower back.

Employee was hanging a miner cable at the face. He had hung at 3 locations when the cable fell striking him on the right shoulder.

Employee was shoveling the section tailpiece when he felt a pain in his left elbow.

Employee was picking up trash on supply road. As he was exiting the Joy ram car he hit his knee on the frame of the ram car.

Employee stated that he was picking up two buckets of miner bits, turned to go to scoop, and stepped in hole. Twisted back and hip.

Bending roof bolt in hole to straighten, bolt spun in hole causing him to fall. Pain in left shoulder, arm and hand.

EE was operating operator side of #3 bolter on #1 MMU. He had left hand on handle under canopy and right hand operating swing lever. There was an overhang on rib . As canopy swung, it went over the top of the overhang and mashed his left ring finger between the overhang and canopy handle. EE received 3 stitches to finger.

EE was installing last bolt in the face of #3 entry of #3 MMU. He put the top section of his finish steel in the roof and the bottom section in the chuck of the bolter. He coupled the two sections together. As he removed his hand , he hit the rotation and feed lever. The pressure from the boom caused the steel to bend and as it rotated, it struck EE in the arm.

Employee was washing concrete bottom on the slope with a wash down hose. His feet became entangled in the hose and he tripped and fell to his knees ,jarring his lower back.

Employee was on his knees cleaning under a belt tail piece, when the pipe that anchors the tail piece fell striking him in the lower back.

Employee was cutting a small notch out of a roll of belt to determine the carrying side of the belt, which was to be added into the existing belt-line. He was using a Lutz, Model 82 utility knife to cut the notch. As he made the last cut, the knife slipped and the blade struck his left forearm. He sustained a 2" laceration to his arm which took 7 stitches to close.

The Employee bent to pick up a concrete block and felt a pain in his lower back.

Employee was picking up a cross over pad while his partner was moving roof bolter. As he attempted to put the pad onto the miner, he felt a pain in his lower back.
