While exiting the read door on the engineer side of locomotive #1200 to set the hand brake the employee stumbled down the two steps to the cat walk. The employee was carrying a lunch bucket therefore had no way to catch self resulting in a broken right leg and fractured right ankle. The area was dry and had zero moisture accumulation. Area was well lit. Approximately 47F out.

Mostrando los 15Employee tripped and stumbled over a pallet in the maintenance shop yard and struck the front bucket of a parked backhoe causing the fracture of 3 ribs.
Employee attempted to align a railroad switch. When employee rotated EE's torso and exerted the appropriate amount of force to move the switch handle to the opposite position, EE's right foot slipped off of the cross tie into the adjacent ballast gravel used to stabilize the switch. This caused the employee's lower body to do the splits, resulting in a strain to the lower back.
Driver fell off scales 5 Ft onto ground. Bruising to left side of body.
Employee arose from a sitting position while on break and temporarily blacked out causing him to fall forward out of the conductor's shanty and onto the track ballast rock resulting in lacerations to the forehead and chin requiring 1 staple and 3 stitches.
Employee stepped down to ground level from the auger house stairs and as he went to turn to the right to leave the area, he twisted his left ankle resulting in a fracture.
Employee was unloading empty pallet from back of mechanic's truck and when he stepped to the ground his knee gave out and felt pain. This is the same knee that was injured in 7/2011 requiring surgery where the employee was off until he was released to return to work on 4/9/2012.
Employee was mounting a parked locomotive on CSX Yard 1 West and slipped bruising his left side ribs and back. Employee was placed on restricted duty by doctor in that the employee couldn't climb or lift greater than 10 lbs for 48 hours. Case turned into lost time due to out company not having light duty positions.
Employee was dismounting a railcar and when his left foot touched the ground he felt a "pop" in his left knee resulting in pain.
Tech walked into sample building to remove bag and stepped on loose floor grating and fell through with left leg up to shin below knee. The grating returned to position trapping leg. No skin damage; ankle sprain resulted. Grating may have been loosed due to vibration from sampling system or not replaced during recent construction. Grating in walkway path.
Employee was walking across the storage yard and twisted his right ankle, which caused a sprain. Employee's doctor put him off for 3 days, therefore resulting in a lost time accident.
EE was cleaning in the draw off tunnel by the feeder #2 walkway above tunnel floor. After cleaning was complete, EE was preparing to climb down the fixed ladder access. Before climbing down the ladder, the EE needed to reconnect the safety chain and while doing this, slipped and fell against the tunnel wall, injuring his right shoulder.
EMPLOYEE WAS STANDING ON REAR BUMPER OF PICKUP PUTTING COAL SAMPLES IN BED OF TRUCK. HIS BOOTS WERE MUDDY WHEN HIS FEET SLIPPED OFF BUMPER FALLING ON TOP OF TAILGATE, STRIKING HIS CHEST.
EE WAS WALKING TO HIS TRUCK WHEN HE STEPPED ON A BOARD WHICH CAUSED HIM TO TRIP. HE FELL AGAINST HIS TRUCK AND HIS LEFT ARM WENT BETWEEN THE RUNNING BOARD AND THE TRUCK BODY FRACTURING HIS SHO ULDER.
EMPLOYEE WAS EXITING THE BINDER PUMP CONTAINMENTAREA WHEN HE SLIPPED & FELL ON A PATCH OF ICE. ACCIDENT NOT REPORTED BY EMPLOYEE UNTIL 1/8/01.
