Employee was kneeled down plugging in an air hose. Employee then raised up and turned to leave, running headlong into the frame of a conveyor. Hard hat took the brunt of the impact, but it is suspected that their reaction of jerking their head backwards to try to avoid or lessen the impact caused whiplash to their neck.

Mostrando los 3Employee states employee bumped hard hat on a down pipe at approximately 7:00 pm. It did not knock employee down, knock off employee's helmet or cause employee any pain. Approximately 5 hrs later, employee was complaining of a headache & nausea & then briefly passed out so an ambulance was called. Employee was treated for concussion like symptoms and released from hospital.
Employee was walking backwards talking to another employee in the equipment parking area. As EE turned around, EE turned into a fill pipe on the side of the parked water truck. The pipe struck EE in the right upper rib cage shoulder area causing EE to fall. Employee complained of right shoulder pain after incident.
