Mostrando los 5On 1/10/26, EE was cleaning a plugged chute and experienced pain in EE's right shoulder. EE evaluated by doctor immediately and was not given any work restrictions that prevented EE from completing EE's normal job duties. On a follow-up appointment on 1/29/26, EE was issued a restriction to be off work.
While dismounting from a portable ladder and area during replacement of a conveyor counterweight, an object fell striking miner in the head requiring sutures in two places. Employee was cleared to return to work without any restrictions.
Employee was washing out elevated areas with a high-pressure hose and material fell striking the miner. Employee received a non-conclusive diagnosis of a possible concussion but out of caution was given work restrictions for return as if EE did receive a concussion.
Employee was dismounting haul truck to use the restroom and felt a pain in back on the final step. Employee visited doctor and was given restrictions from driving a haul truck. Employee worked restricted duty during this time until employee was cleared to return driving the haul truck.
