Was attempting to remove a rock that was fouled in the #3 head. While using a pry bar to lift the rock, the bar slipped as I was pushing down causing my right hand to strike and get pinched by the bar on the edge of the #2 tail guard.

Mostrando los 16Employee was exiting dozer and stepped on the track and lost his footing. He fell on his butt and bruised his lower back and butt. Employee was using hand rail.
During preshift examination April 30. 2015, a roof fall was discovered. The fall was in the #5 entry intake 2 breaks inby spad #63. The fall was immediately called in to MSHA and the WV OMHST. Both departments came and inspected the fall. A plan was then devised and agreed upon by all parties to clean up the fall.
Employee was throwing rocks out of the feeder. He grabbed a hold of a big piece of sharp slate and threw it but before turning loose of it he caught his fingers between it and a solid piece of metal.
Employee was standing on a piece of metal laying on a ladder he pulled the ladder out and the metal tilted causing him to fall 5-6 feet into a chute. Employee said he was ok but while shoveling on 1/12/15 he reported his back was hurting employee went to doctor on 1/13/15 and returned to work on 1/19/15
Employee was shoveling coal in basement when he felt a sharp pain on his right side.
EE said he was looking back to see which scoop man was coming behind him. When a rock fell out the top and hit him in the back. He rolled out of the deck and grabbed the rock through it out of the deck against the rib got back in the miner and backed it out to take it to a new place. After a few minutes went by ee than said he was hurting and wanted to leave
Employee was cleaning in draw off tunnel and stepped on bar gate over a drain sump that slipped out of its frame causing the employee to fall in the sump.
Employee was installing fly boards in the #5 entry after drilling his starter hole. He was attempting to put his stack together when a piece of draw rock came out of the top smashing his left middle finger against the top opening at his pusher resulting in a severe laceration.
Had a double crack in 2 left that we were putting additional roof support up in the form of T3 straps (16 ft. long) when the boss left to go tend to the section for a moment. Employee attempted to install the T3 strap by himself, resulting in a strain of the back.

Employee was walking from back door of the plant to the front parking lot to get oil. He had stopped to scratch his head and turned into the belt structure beam and put a scratch on his head, on 7/25/14 he went to ldrndman clinic because he had a head-ach and was released.
On 7/21/14 called in stating he was going to the Doctor because he had blister on the top of his feet, on 7/14/14 Employee had helped repair a caustic soda line and reported he had spilt the chemical on his boots, over the next few days he developed blisters on the top of his feet (Employee is a diabetic)
Employee was using a chain hoist and a 3/8 chain to lift a side of a screen to change the springs on the right side the chain around a beam holding the hoist broke and the screen fell forcing a spring out hitting him in the right knee causing him to fall.
Employee was using a pry bar to help remove a pitman arm from a steering box on a Mack truck. The bar slipped causing the employee to trip and fall twisting his right ankle. Employee was taken to E.R. X-rays showed his ankle was cracked in 2 places.
Employee was using hoist to lower a section of pump to the ground. He was using his hands to guide the section to the resting place when it turned & mashed the end of his left pinky finger between his section and another section of the pump
