Employee was adjusting valves on the bag pump when the hose popped off of the fitting. The water blew EE's safety glasses off and the hose hit EE near EE's eye. Employee stated several days later the eye still hurt and EE went to the hospital.

Mostrando los 26Employee was lifting a wire saw tree with it rested on employee's arms and hurt left bicep. Employee called the following day and said employee needed a day to rest the arm.
Employee was coming down from a ladder and missed the last step. When they stepped down to the ground, they rolled their ankle slightly. Employee stated that they did not think it was an issue and filed a report when they could not put much weight on it the next day.
Pulling an oversized tarp to fold it up. Drove to portal 5 and when they got out of the truck they felt a sharp pain on the right side of left knee.
Employee was securing water pipe to roof when tension on the pipe released pushing against shoulder which caused pain.
Employee left a wrench on a drill and forgot about it. Employee then turned the machine on causing the wrench to spin and come off and hit the employee in the ankle.
Employee was walking over to see another employee and fell forward breaking both arms.
Employee was measuring the depth of a saw cut and got EE's fingers caught between the marble and a piece of rebar pinching EE's fingers.
IE and another EE were removing the steels from a Fast 65 hole. The bottom steel didn't come loose and when it turned the wrench hit IE in the left ankle.
Cut/crushed left pointer finger picking up a bar.
Employee was sitting between the blocks while one was being trimmed. The trim piece separated and fell on his legs. There was a ladder lying by him. Co-worker had the guys bring a piece of steel to try and take the pressure off his legs. They brought in a loader and picked up the edge of the block a few inches so they could get his legs out.
Employee was removing a pin from a wire saw when another employee hit it with a sledgehammer. The pin came loose and hit him in the right knee.
EE was setting up the wire saw and went to show another ee the controls. He tripped on the diamond wire and hurt his left wrist.
Ice
Blowing out water line and the pressure knocked off his safety glasses and he got something in his eye.
Employee was kicking diamond wire to free it from the ice and hurt his foot.
Washing trackhoe
Employee was getting his lunch box out of the pickup truck and got his foot stuck in the mud. When he went to leave he twisted his right knee.
Employee was moving his saw when the guard fell down hitting him on the head. Employee said his neck was sore and he had a headache. Employee was taking Advil and refused to go to the doctor. On June 2 employee said when he stopped taking the pain medication his neck hurt. We made him an appointment to go to the doctor.
EE was holding a ladder for another ee to the side of the loader forks when a rock fell off of the forks hitting ee in the right pointer finger and right leg.
Employee was walking around the connex area when a rock rolled out from under his foot. He fell and hurt his lower back.
EE pulled a pump off of a block and slipped on the ice and fell and hurt his back.
Employee was walking outside the mine and stepped in some mud and pulled his foot out and twisted his knee.
EE was moving wire when another employee attempted to start the wire saw twisting EE's finger. EE said the accident happened on 9-16-2013 but did not report the accident to anyone until 12-16-2013, 3 months later. He was at work everyday since 9-16-2013 and continues to work up to the time of this report. EE went to the doctor on and they said he might need surgery.
EE was using a jackhammer when the jackhammer spun around and the handle hit EE in the chin.
EE was cleaning out mud out of the cut and a piece of marble fell about ten feet striking him on his hardhat.
