Using improper tool. Employee was using a sledgehammer and a king pin to remove a pin in the conveyor when a sliver of the king pin broke off cutting their arm and embedding in their arm.

Employee was helping string a conveyor belt. When pulling slack out of the roll with a rope, the knot secruring it came loose, causing them to fall onto a steel beam. Employee has a small fracture to the ninth rib.
EE was removing hardware from sand screw using an impact wrench. EE believed that sand fell from the screw and EE felt slight pain in EE's left eye. After finishing the project, the EE's eye was still irritated so EE flushed it with water. The EE went to the eye doctor on Monday morning. Doctor found metallic material in EE's left eye.
Employee was walking around a skid steer and tripped over the corner of the bucket, causing him to fall to the ground. Employee requested to be seen by physician for leg pain. Employee was seen by a doctor for pain to his upper left leg. Physician determined there was no broken bone and restricted employee to limited walking. Employee was released to full duty on 5/4.
